Özet
Merkezi sinir sistemi enfeksiyonları çocuklarda morbidite ve mortaliteye
yol açması nedeni ile önemini korumaktadır. Bu çalışmada Ocak 2006 ile
Ocak 2007 tarihleri arasında kayıtlarına ulaşılabilen 73 merkezi sinir
sistemi enfeksiyonlu hastanın bulguları retrospektif olarak incelendi. Yaş
ortalaması 4.7±3.7 yıl (3 ay-13 yaş) ve erkek kız oranı (E/K) 2.3 idi.
Başvurudaki en sık şikayetler ateş, kusma ve baş ağrısı idi. Fizik inceleme
bulgularında en sık meninks irritasyon bulguları ve bilinç değişikliği
bulundu. Klinik ve laboratuar bulguları açısından hastalar
sınıflandırıldığında %53.4 bakteriyel, %36.9 aseptik ve %9.5 tüberküloz
menenjit olarak saptandı. Aseptik menenjitli 6 hastada kabakulak
enfeksiyonu, bakteriyel menenjitli 8 hastada posttravmatik menenjit ve 4
hastada tekrarlayan enfeksiyonu mevcut idi. En sık komplikasyonlar
konvulziyon, hidrosefali ve uygunsuz ADH sendromu idi. Mortalite oranı
%4.1 olarak bulundu. Çocuk yaş grubunda merkezi sinir sistemi enfeksiyonlarının erken tanı
ve tedavisi gelişebilecek komplikasyonları ve mortaliteleri önleme açısından
önemlidir.
Central nervous system infections maintain their importance since they result in morbidity and mortality. In this study findings of 73 patients with central nervous system infection were evaluated retrospectively. Male female rate was determined to be 2.3, and the mean was 4.7±3.7 years (ranging from 3month to13 years). The most common symptoms on admission were fever, vomiting and headache. On physical examination, the most frequently encountered symptoms were meningeal irritation findings and apathy. When patients were classified in terms of clinical and laboratory findings; bacterial meningitis was determined 53.4%, aseptic in 36.9% and tuberculous meningitis in 9.5%. There was mumps infection in 6 patients with aseptic meningitis, post-traumatic meningitis in 8 patients with bacterial meningitis and recurrent central nervous system infection in 4 patients. The most frequent complications were found to be convulsion, hydrocephalus and the syndrome of inappropriate ADH secretion. The mortality rate was determined as 4.1%. Early diagnosis and prompt treatment of central nervous system infections are of utmost importantance for preventing complications and mortalities in children.
