    Adjuvant chemoradiotherapy for cancer of the rectum: Postoperative or preoperative?

    Cancer of the rectum is a frequent malign disease of both sexes and its primary treatment is surgery. Postoperative chemotherapy is the generally preferred adjuvant treatment modality but its results are not still satisfactory. Recent studies have reported that total mesorectal excision technique with preoperative chemoradiotherapy is superior to postoperative adjuvant treatment in terms of local disease control. The question of timing of the adjuvant treatments like radiotherapy, chemotherapy and chemoradiotherapy will be discussed in light of the randomized clinical trials with sufficient patient number. In conclusion, current recommendation is preoperative concomitant chemotherapy plus radiotherapy.

    Development of AML in a patient who was in complete remission after limited stage small cell carcinoma of the lung: A case report

    Concomitant thoracic irradiation and cisplatin and etoposide has been started at a 62 years old male patient with limited stage small cell carcinoma of the lung. Complete remission was achieved clinically and radiologically after 46 Gy irradiation and 4 cycle chemotherapy and prophylactic cranial irradiation was also applied. The patient was on complete remission for more than 6 months when he complained of malasia and hemogram revealed pancytopenia. Diagnosis of acute myeloblastic cell leukemia (AML) was reached with blood smear and bone marrow aspiration biopsy. AML-M2 diagnosis was confirmed with flowcytometry. Chemotherapy with cytosin-arabinoside and idarubicin was started. The patient has died at the 35th day of therapy with acute respiratory arrest. Death was dedicated to intracranial hemorrhage which caused by thrombocytopenia.

    Should internal mammary nodes be irradiated?

    The optimal treatment of regional lymph nodes in the setting of postoperative adjuvant radiotherapy in unknown. Treating internal mammary lymph nodes is also controversial. Increased overall survival has been reported with post-mastectomy radiotherapy but contribution of internal mammary nodes radiotherapy was not specifically addressed. Irradiation of the cardiac structures with photon beams resulted in increased cardiac-related mortality but this is not the case for electron beams as shown in the literature. In this present review, we summarized the literature concern-ing internal mammary nodes irradiation.

    Surveillance of breast cancer after primary treatment

    Breast cancer occupies a serious part of routine oncological practice and medical care costs because of its high prevalance and long survival. Hence, surveillance after primary treatment should be based on evidence-based and cost-effective approach. Breast cancer recurrence can be seen as locoregional or systemic relapse, or both. The majority of relapses occur within first few years after treatment. Except mammography, laboratory and radiographic tests are not sensitive and specific enough for detecting recurrence. And early diagnosis of distant metastasis did not improved overall survival and quality of life in randomized clinical trials. Thus follow-up with clinical history, physical examination, patient education and annual mammography and pelvic examination is satisfactory for asymptomatic breast cancer patients after primary treatment.
