Özet
Giriş ve amaç: Psöriatik artrit (PsA) psöriasis ile ilişkili inflamatuar bir hastalıktır. PsA sinovit, daktilit, entezit ve spondilit ile karakterizedir. PsA hastalarında uyku kalitesi ve bununla ilişkili olarak yaşam kalitesi etkilenmektedir. Bu çalışmanın amacı PsA’nın uyku kalitesi üzerindeki etkisini belirlemek; uyku kalitesi ile klinik parametreler, yaşam kalitesi ve psikolojik durum arasındaki ilişkiyi ortaya koymaktır. Materyal ve metod: Çalışmaya 41 psöriatik artrit hastası ve 38 sağlıklı gönüllü dahil edildi. Demografik ve klinik özellikler; yaş, cinsiyet, hastalık süresi, eritrosit sedimentasyon hızı (ESH), C reaktif protein (CRP) ölçümleri kaydedildi. Hasta grubunda PsA yaşam kalitesi indeksi (PsAQoL), psöriasis alan şiddeti indeksi (PASI) skorları ölçüldü. Generalize ağrı vizüel analog skala (VAS) ile değerlendirildi. Hasta ve kontrol grubunda anksiyete ve depresyon skorları hastane anksiyete depresyon skalası (HADS) ile uyku kalitesi Pittsburgh uyku kalitesi indeksi ile (PUKİ) değerlendirildi. Bulgular: Psöriatik artritli hastalarda, öznel uyku kalitesi, uykuya dalma süresi, uyku süresi, alışılmış uyku etkinliği, uyku bozukluğu, gündüz işlev bozukluğu ve total PUKİ skoru kontrol grubuna göre anlamlı olarak yüksekti. Total PUKİ skoru; anksiyete, generalize ağrı, PsAQoL, entezit, CRP ve ESH seviyeleri ile anlamlı korele idi (p<0,05). Sonuçlar: Psöriatik artiritli hastalarda uyku kalitesi bozulmuştur. PsA tanılı hastalarda uyku bozukluğu özellikle generalize ağrı, anksiyete, entezit, ESH ve CRP seviyeleri ile ilişkilidir. Introduction: Psoriatic arthritis (PsA) is an inflammatory disease associated with psoriasis. PsA is characterized by synovitis, dactylitis, enthesitis and spondylitis. Related to sleep quality, quality of life is being affected in patients with PsA. The aim of this study is to determine the influence of PsA on sleep quality and to reveal the relationship of sleep quality with the clinical parameters, quality of life and the psychological state. Material and method: Fourty one patients with PsA and 38 healthy volunteers were included into the study. Demographic and clinical characteristics including age, sex, disase duration, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) were recorded. Psoriatic Arthritis Quality of Life (PsAQoL), Psoriasis Area and Severity Index (PASI) scores were measured. Generalized pain assessed by visual analog scale (VAS). Anxiety ve depression scores were evaluated with Hospital Anxiety and Depression Scale (HADS) and sleep quality was evaluated with Pittsburgh Sleep Quality Index (PSQI) in both patient and control group. Result: Subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, daytime dysfunction and total PSQI were signifanctly higher in patients with PsA compared to control group. Total PSQI score was significiantly correlated with anxiety, generalized pain, PsAQoL, enthesitis, CRP and ESR levels (p<0,05). Conclusion: The sleep quality is disturbed in patients with PsA. Sleep disturbance is especially associated with generalized pain, anxiety, enthesitis, ESR and CRP levels in patients with PsA.
