dc.description.abstractAmaç: Çalışmamızı; Küçük hücre dışı akciğer kanserli olgularda, maksimum standart uptake değerlerinin (SUVmax); sağkalım ile korelasyon olup olmadığını belirlemek ve olguların demografik, klinik ve laboratuvar verilerinin sağkalım üzerine etkileri araştırmak amacıyla planladık. Gereç ve Yöntem: Çalışmamız, Dicle Üniversitesi Tıp Fakültesi Göğüs Hastalıkları ve Onkoloji Anabilim Dalı polikliniklerine Mayıs 2006 ile Mart 2011 yılları arasında başvuran, kesin tanı almış ve evrelemesinde PET /BT kullanan 101 küçük hücre dışı akciğer kanserli olgunun dosyaları geriye dönük incelenerek oluşturuldu. Çalışmaya alınan olguların dosyalarından başta18 fluorodeoxyglucose pozitron emissiontomography ( FDG PET) deki kitlenin SUVmax değeri olmak üzere diğer prognozu etkileyen klinik, histopatojik, laboratuar ve tedavi paremetreleri kaydedilerek istatistik değerlendirme yapıldı. Bulgular: Hastaların 88’i erkek (%87,1) ve 13’i kadındı (%12,9) .Vakaların ortalama takip süresi 12.68 ± 11 ay idi. Tüm seride ortalama sağkalım süresi 10,62 ± 8,55 (1- 49 ay ) idi. Tüm hasta popülâsyonun kitlenin SUVmax medyan değeri; 12,0 (2,7 – 35) idi. Hastaların demografik, klinik, laboratuar ve radyolojik parametreleri SUVmax 'ın medyan değeri (12,0) ile iki gruba ayrılarak karşılaştırıldı. SUVmax <12 olan 50 hastanın medyan sağ kalımı 10,5 ay (1,5- 43,5), SUVmax ?12 olan 51 hastanın medyan sağkalım 8 ay (1–49) idi. Aralarında sağkalım acısından, istatistiksel olarak anlamlı bir fark yoktu (p=0.807). Çalışmamızda tek değişkenli analiz ile prognostik faktörlerin sağkalıma etkisini incelendiğimizde; ileri evre, metastaz varlığı, yüksek LDH düzeyi, yüksek WBC düzeyi, inoperabl olması, düşük albümin düzeyi ve düşük perfomans düzeyi olması olumsuz etkilediği görüldü. Cox regresyon yöntemi kullanılarak yapılan çok değişkenli analizde ise, erken evre ve cerrahi uygulanması sağkalım için bağımsız bir faktördü (p=0.002, 0.031) . Sonuç: Küçük hücreli dışı akciğer kanserli olguların, sağkalım süresinde SUVmax’ın önemli rolü olmadığını düşündürtmüştür. Hastaların tümör evresine ek olarak performans durumu, serum LDH, WBC ve albümin değerlerin bilinmesi daha iyi prognostik değerlendirme sağlayabilir. dc.description.abstractObjective: We planned our work in order to investigate whether in non-small-cell lung cancer patients the maximum standard uptake values (SUVmax) correlate with survival or not and to investigate the effect of demographic, clinic and laboratory data on survival. Materials and Methods: Our study was created by the way that we retrospectively have reviewed files of patients who have applied at the Dicle University School of Medicine Chest diseases and Oncology Department between the dated of May 2006 and March 2011 who have had a definite diagnosis and using PET /BT in the staging with 101 small cell-exterior cancer patients. Of the files of the patients included in the study a statistic was made by initially recording the SUVmax value of the mass in 18 fluorodeoxyglucose pozitron emissiontomography (FDG PET) and of all other clinical, histopathological, laboratory and treatment parameters affecting the prognosis. Findings: 88 of the patients were men (%87,1) and 13 were women (%12,9). The average follow up period of the cases was 12.68 ± 11 months. In all series the average survival period was 10,62 ± 8,55 (1- 49 months). The SUVmax mdian value of the mass in all patient population was 12,0 (2,7 – 35). The demographical, clinical, laboratory and radiological parameters of the patients an the median value (12,0) of SUVmax were divided into two groups and compared. The median survival 50 patients with SUVmax <12 has been 10,5 months (1,5- 43,5), the median survival of 51 patients with SUVmax ?12 has been 8 months (1–49). In terms of statistics of survival there was no significant difference (p=0.807). When we examined the effect of prognostic factors on survival with a single variable analysis it was seen that metastasis occurrence of advanced stage, high LDH level, high WBC level, that it was not surgery, that the low albumin level and low performance level did negatively affect it. In the multi variable analysis with the Cox regression method the early stage and application of surgery were independent factors for survival (p=0.002, 0.031). Result: It was considered that in the survival period of non-small-cell lung cancer patients SUVmax does not play an important role. Knowing additionally the performance condition of the patient in the tumor stage, serum LDH, WBC and albumin values may provide a better prognostic evaluation.
