Örtülü kapanış ve bruksizm görülen hastaya yeni protetik yaklaşım: Olgu sunumu

Eski protezlerini değiştirmek üzere kliniğimize başvuran 35 yaşındaki erkek hastanın yapılan muayeneler neticesinde örtülü kapanışa sahip olduğu tespit edilmiş ve hastaya bruksizm tanısı konulmuştur. Çalışmamız, bruksizm ve örtülü kapanış sonucunda hastada oluşabilecek temporamandibular eklem rahatsızlıklarının önüne geçmeyi ve aynı zamanda protezlerini yenileyerek, estetik ve fonksiyonunu tekrar kazandırmayı amaçlamaktadır. Bu doğrultuda örtülü kapanış nedeniyle oluşan azalmış yüz yüksekliği ve bruksizm sonrası oluşan ağrı, literatürlerin desteğiyle dikey boyut arttırılarak tedavi edilmeye çalışılmıştır. Hastanın tedavi planlaması, konulan teşhisler, materyal seçiminde sosyoekonomik durum ve hastanın onayı göz önünde bulundurularak yapılmıştır. Hastaya bütün bu etkenler neticesinde metal alt yapılı porselen restorasyonlar yapılmasına karar verilmiştir. Dikey boyut yükseltmek ve bu işlem yapılırken herhangi bir komplikasyonla karşılaşmamak için hasta, geçici akrilik restorasyonlarla bekletilmiş ve temporamandibular eklemin göstereceği reaksiyon gözlemlenmiştir. Her dental tedavide olduğu gibi preprotetik aşamada diğer bölümlerinde tedaviye katkısıyla protetik aşama da tamamlandıktan sonra hastanın yüz yüksekliğinde bir artış elde edilmiştir. Artan dikey boyut sayesinde bruksizm sonrasında masseterde hissedilen yoğun ağrıda azalma olduğu hasta tarafından ifade edilmiştir. Bunun yanında hastanın kliniğimize asıl başvuru sebebi olan eski protezleri yenilenmiştir. Hastanın estetik ve fonksiyonel beklentileri de sağlıklı bir şekilde giderilmiştir.

A 35-year-old male patient, who applied to our clinic to replace his old prostheses, was found to have an overbite as a result of the examinations, and the patient was diagnosed with bruxism. Our study aims to prevent temporamandibular joint disorders that may occur in the patient as a result of bruxism and covert bite, and at the same time to restore aesthetics and function by renewing the prosthesis.The patient's treatment planning, diagnoses, material selection, socioeconomic status and patient's consent were taken into consideration. As a result of all these factors, it was decided to make porcelain restorations with metal substructures. In order to increase the vertical dimension and avoid any complications during this procedure, the patient was kept with temporary acrylic restorations and the reaction of the temporamandibular joint was observed. Every dental treatment, an increase in the patient's face height was obtained after the prosthetic phase was completed with the contribution of the treatment in the other parts of the preprosthetic phase. Bruxism due to increased vertical size It was stated by the patient that there was a decrease in the intense pain felt in the masseter afterward. In addition, the old prostheses, which were the main reason for the patient's application to our clinic, were renewed. The aesthetic and functional expectations of the patient were also met in a healthy way.
