Kardiyopulmoner resüsitasyon ilişkili kot ve sternum kırıklarının incelenmesi

Özet
Amaç: Özellikle travma sonrası otopsi olgularında tespit edilen bazı bulguların primer travmaya bağlı veya kardiyopulmoner resüsitasyon (KPR) ile ilişkili olduğu konusunda adli hekimler yanılabilmektedir. Bu nedenle KPR ilişkili olabilecek komplikasyonlar iyi tanınmalı ve adli otopsi esnasında akılda tutulmalıdır. Bu çalışmada non-travmatik adli otopsi olgularında, KPR ile ilişkili olabilecek komplikasyonların ortaya konulması amaçlandı. Gereç ve Yöntem: Mevcut çalışmada Adli Tıp Kurumu Diyarbakır Grup Başkanlığı tarafından 2005-2014 yılları arasında 10 yıllık periyotta adli otopsi veya sadece ölü muayenesi yapılmış olan 9174 adet adli ölüm vakasının dosyası taranarak veriler elde edildi. Bu adli ölüm vakalarından 6309 adetine adli otopsi yapılmış olup adli otopsi yapılanlar arasından non-travmatik nedenlerle öldüğü kesinleşmiş ve KPR uygulanmış olan 403 vaka incelendi. Bulgular: Mevcut çalışmada KPR uygulanan vakalar incelendiğinde 92 hastada ekimoz ve cilt altı kanama (%22.82), 16 olguda 1-3 arası sayıda kot kırığı (%3.97), 49 hastada 4 veya 4’ten fazla kotta kırık (%12.15), 78 olguda sternum kırığı (%19.35), 13 olguda iç organ hasarı (%3.22) tespit edildi. Sonuç: Çalışmanın sonucuna göre tespit edilen komplikasyon oranları literatüre göre düşük bulunmakla beraber, bu düşüklüğün bazı hastalarda oluşan komplikasyonların gözden kaçabileceği şeklinde yorumlanmıştır.
Objectives: Forensic physicians may be mistaken about whether some findings especially in post- traumatic autopsy cases are related to primary trauma or cardiopulmonary resuscitation (CPR). Therefore, complications that may be CPR should be well recognized and kept in mind during forensic autopsy. In this study, it is aimed to reveal the complications that can be related to CPR in non-traumatic death cases. Material and Methods: In the present study, the data were obtained by scanning the files of 9174 forensic death cases who had forensic autopsy or only corpse examination in a 10-year-long period between 2005 and 2014 by the Council of Forensic Medicine, Diyarbakir Group Chairmanship. Of these forensic death cases, forensic autopsy was performed on 6309 and among those who had forensic autopsy, 403 cases who were confirmed to die due to non-traumatic causes and to have undergone CPR were examined. Results: In the study, when the cases where CPR was applied, were examined, ecchymosis and subcutaneous haemorrhage was found in 92 (22.82%) cases, 1-3 rib fractures were found in 16 (3.97%) cases, fractures in 4 or more than 4 ribs were found in 49 (12.15%) cases, sternal fracture was found in 78 (19.35%) and internal organ damage was found in 13 (3.22%) cases. Conclusion: Although the complication rates according to the results of the study were found to be low compared to the literature, this low rate may be interpreted as that the complications occurring in some patients could have been overlooked.
