dc.description.abstractHp duodenal ülser patogenezinde en önemli faktördür. Hp eradikasyonu, duodenal ülser nüksünün azalmasına neden olmaktadır. Bu nedenle, omeprezol, amoksisilin ve klaritromisinden oluşan üçlü kombinasyonun ülser tedavisine ve Hp eradikasyonuna etkisi araştırılmıştır. Omeprazol (2x20 mg / gün), amoksisilin (2x1000 mg/gün) ve klarit romisin (2x250 mg/gün) tedavisi; Hp (+) olan 24 duodenal ülserli (22 erkek, 2 bayan 21-57 yaş) hastaya 2 hafta süreyle uygulanmıştır. Kontrol grubu olarak, omeprazol (2x20 mg/gün), amoksisilin (2 xl000 mg/gün) tedavisi uygulanmış ve Hp (+) olan 28 duodenal ülserli hasta (18 erkek, 10 bayan, 22-55 yaş) alınmıştır. Teşhis, GÎS endoskopisi ile konmuş ve antrumdan HE, Giemsa ve hızlı üreaz testi için biopsilen alınmıştır. Tedavi öncesi tedaviden 15 ve 45 gün sonra tekrar endoskopiler yapılmıştır. 3 testinde Hp için negatif olması, eradikasyon olarak kabul edilmiştir. 3. endoskopide Hp % 8 olguda (+), eradikasyon oram ise % 92 olarak bulunmuştur. Kontrol grubunda eradikasyon oram % 79 olmuştur. Bu sonuçlar, 2 haftalık 3' lü tedavinin duodenal ülser tedavisinde ve Hp eradikasyonunda çok iyi etkili, iyi tolere edilebilen bir tedavi yöntemi olduğu kanıtlamıştır.
dc.description.abstractHelicobacter pylori (Hp) is the most importand factor in the pathogenesis of duodenal ulcer (DÜ). Eradication of Hp does lower the recurrence rate of DÜ. The aim of this study was to evaluate the efficacy, tolerabiliry and compliance of a triple therapy with Omeprazole and Amoxicillin plus duodenal ulcer and Hp infection 24 patients (22 males, 2 females, age range 21-57) with duodenal ulcer vere treated for 2 week with omeprazol 20 mg bi.d, amoxicilline 1 gr b.i.d and Claritromicine 250 mg b.i.d, 28 Hp (+) duodenal ulcer cases which are treated for 2 with Omeprazole 20 mg b.i.d, amoxicilline 1 gr b.i.d. are taken as control groups. Diagnosis was performed by upper G.Î. endoscopy and multiple biopsies of antral mucosa. Standart H.E, Giemsa stains and rapid urease test were obtained and observed by one microbiologist. They were endoscoped befor and the end of therapy and 1 month after completion of therapy. Eradication was defined as all tests negative at the final endoscopy. At the third endoscopy Hp was found 8 %. The eradication was there fore reached in 22 patients (92 %). At the control group, the eradication rate was 79 % (P< 0,001). It is concluded that 2 weeks administration of triple therapy wjth omeprazole, claritromicine and amoxicillin is very effective and well tolerated for the eradication of Hp infection.
