    Comparative preventive efficacy of recombinant tissue plasminogen activator (rt-PA), urokinase, hyaluronic acid and enoxaparin in post laminectomy epidural fibrosis in rats

    Aim: We aimed to determine the effects of rt-PA and enoxaparin on post laminectomy epidural fibrosis and to compare the effects of these two medications with hyaluronic acid and urokinase in an experimental model of spinal epidural fibrosis. Method: Forty Spraque-Dawley rats were anaesthetized by intraperitoneal anesthesia (ketamin 8 mg/100 g) and randomly allocated to five groups. All groups were underwent L1-2 total laminectomies. Topical medications were applied to group 2-5 right after the laminectomy space. Hence, in group 1 (G1=control) only laminectomy was constituted, in group 2 (G2) laminectomy + topical Urokinase, in group 3 (G3) laminectomy + topical rt-PA, in group 4 (G4) laminectomy + topical enoxaparin and in group 5 laminectomy + topical Hyaluronic acid (G5) application were performed. The potentials of local urokinase, rt-PA, hyaluronic acid and enoxaparin in preventing the production of epidural fibrosis were assessed. Result: In inter group comparison, G1-G2, G1-G3, G1-G4, G1-G5 groups differed from each other for total scar score, middle scar score and deep scar score (p<0.05). However, G2-G3, G2-G4, G2-G5, G3-G4, G3-G5 and G4-G5 were not different from each other. Conclusion: In rats, the topical thrombolysis with fibrinolitic agents is safe and efficacious in preventing of post laminectomy epidural fibrosis. Thrombolytic therapy with topical hyaluronic acid, rt-PA, urokinase and enoxaparin may come to play an important role in the prevention of post operative spinal epidural fibrosis and arachnoiditis. Based on the results we found that rt-PA and enoxaparine inhibit the formation of epidural scar adhesions and arachnoiditis compared with control group.

    Differential expression of CD34, S100, and c-Kit in interstitial cells of Cajal in infantile hypertrophic pyloric stenosis - Immunochemical study

    Background. The pathogenesis of infantile hypertrophic pyloric stenosis (IHPS) is poorly understood although many hypotheses have been proposed. Objectives. Assessment whether the differential expression of c-Kit, CD34, and S100 may be involved in the development of IHPS. Material and Methods. Specimens from 14 infants with IHPS and seven control subjects were immunohisto-chemically stained for c-Kit, CD34, and S 100. The numbers of CD34+, S 100+, and c-Kit+ cells in five random fields per specimen were compared via light microscopy (x200). Results. In normal pyloric tissue, specific and intense c-Kit immunoreactivity was observed in the muscle layers and moderate staining was observed around the myenteric plexus. In IHPS patients, c-Kit+ cells were either absent or markedly reduced around the myenteric plexus. In control and IHPS patients, CD34+ cells were not observed around the myenteric plexus. In the vascular endothelium, moderate CD34 staining was observed in specimens from control subjects, whereas intense staining was observed for IHPS patients. In normal pyloric tissue, moderate S 100 immunoreactivity was observed in the muscle layers and intense staining was observed in the myenteric plexus. In IHPS patients, few S100+ cells were observed in the pyloric muscle layers and S100 immunoreactivity decreased markedly around the myenteric plexus. Conclusions. These results suggest that the numbers of c-Kit+ and S100+ cells are markedly decreased in the pyloric muscle layers and around the myenteric plexus in IHPS patients. Thus a lack of c-Kit and S100, but not CD34, expression may be a critical factor in the pathogenesis of IHPS and may serve as a useful prognostic tool in the treatment of this disease. © Copyright by Wroclaw Medical University.

    Echinococcosis multilocularis in south-eastern Anatolia, Turkey

    Objective: To quantify the human prevalence, elucidate possible risk factors and present other epidemiological parameters of human alveolar echinococcosis in the south-eastern region of Turkey. Design: A retrospective study. Settings: South-eastern region (Anatolia) of Turkey by two medical centres: Dicle University Medical Faculty in Diyarbakir and Harran University Medical Faculty in Urfa. Subjects: Forty seven cases of human alveolar echinococcosis were presented between the period of 1980 and 2000. Methods: For the majority of our patients, personal anamnestic, clinical data and addresses were available from their files, and we visited their villages to investigate the risk factors for human alveolar echinococcosis. Epidemiological parameters were evaluated. Results: The 47 cases consisted of 18 (38.3%) males and 29 (61.7%) females. The average age was 34.6 years. In all cases, lesions occurred in the liver. No village or district had more than one case. The majority of patients (73.8%) were farmers and 33 had a history of dog ownership. Conclusion: This region has a higher incidence than Turkey as a whole, and domestic dogs and free-ranging dogs seem to be the main source of infection.

    The effect of vitamin E on small intestine damage of ischemia- reperfusion in young rats

    The effect of antioxidant agents on ischemia-reperfusion is well known. In this study 300 mg/kg of vitamin E was administered I.M to young rats for three consecutive. On day three, the mesenteric superior artery was held with atraumatic clamps for 30 minutes to create ischemia, and then exposed to 45 minutes of reperfusion. We took sections from the jejunum for histological examination, then observed the negative effect of ischemia reperfusion and protective effect of vitamin E.

    The effects of Extremely Low Frequency Magnetic Field on inner ear, parotis and submandibular gland

    Experiments using sixteen male Spraque-Dawley rats were carried out to determine possible effects of Extremely Low Frequency Magnetic Field (ELF MF) on inner ear, parotis and submandibular glands. The rats were seperated into two groups of 8, one control and one experimental group. The rats in the experimental groups were exposed to ELF MF (1.35 mT) 2 hour/a day during two months. Eight rats of sham group were treated like experimental group except ELF MF exposure. The rats were sacrificed under ketalar anesthesia immediately after last exposure. Then parotis, submandibular gland and total ear of rats were removed for histopathological examination. The histopathological results obtained indicate that ELF magnetic field didn't affect parotis, submandibular glands and inner ear.

    Giant sebaceous naevus: A case report

    Sebaceous naevi are circumscribed hamartomatous lesions composed predominantly of sebase glands at birth or early childhood. The esions may be round, oval, or linear, varying in length from about 1 to 10 cm. We described a 19 year-old female with a giant sebaceous naevus measuring 10x17 cm on the scalp, forehead and left cheek.

    Heavy metal levels in some macrofungi

    This study was done on edible, inedible and poisonous macrofungi collected around the Balikesir-Manisa highway from two different areas (roadside and background area) in 1998-1999. Cu, Zn, Fe, Mn, Co, Cd, Ni and Pb contents were determined by atomic absorption spectrophotometer in 256 samples belonging to 24 macrofungi species. The habitat, edibility and distribution of the taxa in the families were listed. According to mean dry weight (DW), Mn, Co and Cd contents were high in Omphalotus olearius (DC.: Fr.) Fr., which is a poisonous macrofungus species compared to the others; however, Fe levels were also extremely high. The lowest Cu, Mn and Fe contents were found in Laetiporus sulphureus (Bull.: Fr.) Murr., which is an edible macrofungus. The highest Pb and Zn contents were determined in Lycoperdon perlatum Pers. as 6.5 mg/kg and 274 mg/kg respectively. The contents of Ni and Cd seemed to be lower the near road.

    Malignant ovarian germ cell tumors: Analysis of 32 cases

    Objective: In this study, some clinicopathologic characteristics and the outcome of patients with malignant ovarian germ cell tumors (MOGCT) were evaluated. Materials and Methods. The clinical charts and pathologic reports of 32 patients with MOGCT treated at the Department of Obstetrics and Gynecology, and diagnosed at the Department of Pathology, Medical Faculty of Dicle University, Turkey from 1983 to 1999 were reviewed. Results: Thirteen patients (40.6%) had dysgerminoma, nine (28.1%) had immature teratoma (four grade 1, three grade 2, and two grade 3), eight (25%) had endodermal sinus tumor, and two (6.3%) patients had mixed germ cell tumors. Site of involvement was unilateral in 30 (19 on the right and 11 on the left) and bilateral in two. All patients underwent primary surgery and 26 patients combination chemotherapy. There seemed to be a relationship between pathologic findings and clinical outcome, and MOGCT histologic types may affect the prognosis. Conclusion: Dysgerminoma had a better prognosis than the nondysgerminomatous group (p < 0.05). This study provides additional data in confirmation of previous reports that management of MOGCT with fertility preservation is safe.

    Nevoid hyperkeratosis of the nipple and areola: Case report

    Nevoid hyperkeratosis of the nipple and areola is a rare benign condition of unknown etiology, Its characteristic properties are verrucous thickening and brownish discoloration of the nipple and areola. In microscopic examination, hyperkeratosis, papillomatosis and acanthosis with keratotic plugging were observed and nevoid hyperkeratosis was diagnosed. We discussed this rare condition in the light of current literature.

    Post- pemphigus acanthomata

    Pemphigus is a group of vesiculobullous disorders in which the blisters usually heal with hyperpigmentation. The appearance of acanthomata at sites of previous blisters has been noted in some cases. We described a 48-year- old patient with pemphigus vulgaris, who was treated with systemic steroids, and who developed acanthomata on the sites of some healed blisters. The acanthomata were seen as verrucous lesions at sites of healed blisters, and were observed in the trunk. Histopathology of lesions showed suprabasal cleft associated with hyperkeratosis, acanthosis, and papillomatosis. Acantholytic cells were seen intraepidermally within the cleft. Acanthomata occurring in patients whose pemphigus is in remission is not uncommon. Since post- pemphigus acanthomata shows histopathologic activity, these cases should be carefully followed-up.

    Primary gastrointestinal lymphomas: A clinicopathologic study of 55 cases

    Purpose: To study primary gastrointestinal (GI) non-Hodgkin's lymphomas (NHLs) in our region of the southeast Turkey, and to compare our results with those found in the relevant literature. Materials and methods: Fifty-five patients with surgical resection of primary GI NHLs were reviewed retrospectively, and their histologies were classified according to the Working Formulation. Clinical data obtained from pathology and endoscopy reports was also reviewed. Results: The cases of thirty-two male and 23 female patients were studied. GI NHLs were most frequent in the first and fifth decade of life (mean 30.9 years, range 3-75 years). Abdominal pain, weight loss and bleeding were the most common presenting symptoms. The primary sites of the tumors were: 16 in the stomach, 26 in the small intestine, 3 in the iliocecal region, and 10 in the large intestine. Eight (15%) of the 55 GI NHLs were of low-grade malignancy (5 lymphocytic and 3 follicular small cleaved cell), 38 (69%) were of intermediate-grade malignancy (9 diffuse small cleaved cell, 6 diffuse mixed, 23 diffuse large cell) and 9 (16%) were of high-grade malignancy (1 immunoblastic, 3 diffuse small noncleaved cell and 5 lymphoblastic). Conclusion: Clinicopathologically, our cases of primary GI NHLs seem to resemble the "Mediterranean" type of NHLs.

    Protruding urethral leiomyoma obscuring vaginal introitus: Case report

    Leiomyoma is quite rare in the female urethra. We report an additional case to 29 reported in the literature. A multiparous woman presented with a complaint of a gradually enlarging mass causing dyspareunia for the last seven months. The mass was originating from the upper part of the distal urethra with a relatively thin stalk covered by squamous epithelia, and immunohistopathological examination confirmed it as leiomyoma.

    Visceral leishmaniasis with multiple nodular lesions of the liver and spleen: CT and sonographic findings

    Visceral leishmaniasis is a severe disease caused by the intracellular protozoa Leishmania donovani. Diagnosis is based on examination of bone marrow or serology. The role of imaging techniques as diagnostic tools remains to be established in visceral leishmaniasis. We report multiple nodular lesions in the liver and spleen on ultrasonography and computed tomography in a patient with visceral leishmaniasis. To our knowledge, this is the first reported case of multiple nodular hepatosplenic lesions in visceral leishmaniasis.
