    A case of primary pancreatic lymphoma followed in remision in the 9th year

    Primary pancreatic lymphoma (PPL) is a rare form of ekstranodal, lymphomas, comprising 0.2-4.9% of pancreatic masses and <1% of all malignant lymphomas. Due to the rarity of PPL and nonspecific clinical symptoms and imaging, differentiation of PPL from adenocarcinoma is difficult without histopathologic diagnosis. Many patients with primary pancreatic lymphoma are diagnosed after radical surgery due to difficulties in diagnosis. Many patients with primary pancreatic lymphoma can be diagnosed after radical surgical treatments due to diagnostic difficulties. A 70-year-old patient presenting with abdominal pain was detected to have pancreatic mass. The post-operative pathologic outcome was reported as T-rich B-cell lymphoma. The patient was followed with complete remission after receiving six cycles of chemotherapy. This case report emphasizes the importance of distinguishing between pancreatic lymphoma and adenocarcinoma because of the different treatment strategies and prognoses.

    Efficacy of Ga-68-PSMA PET/CT-derived whole-body volumetric parameters in predicting response to second-generation androgen receptor axis-targeted therapy, and the prognosis in metastatic hormone-refractory prostate cancer patients

    Introduction The present study investigates the role of Ga-68-PSMA PET/CT-derived whole-body metabolic and volumetric parameters in the prediction of treatment response and prognosis among metastatic hormone-refractory prostate cancer patients undergoing second-generation androgen receptor axis-targeted therapy (abiraterone or enzalutamide). Materials and methods This retrospective study included 44 metastatic hormone-refractory prostate cancer patients undergoing Ga-68-PSMA PET/CT, including 29 enzalutamide-treated and 15 abiraterone-treated patients. Results Of the 44 patients included in the study, 29 received enzalutamide and 15 received abiraterone. During treatment, the changes in PET parameters were correlated with the PSA (biochemical) response. More specifically, a positive correlation was noted between PSA response and percent change in TLP (Delta TLP) response, and there was concordance between the results (r = 0.652, k = 0.42, P < 0.001). Baseline PSA (P =0.05), high MTVw (P = 0.005), the increase in Delta PSA (P = 0.036), Delta TLP (P = 0.039) and percent change in MTV (Delta MTV) (P = 0.049) values were identified as factors associated with mortality risk.Multivariate analysis showed that PSA1 [odds ratio (OR): 1.005, 95% confidence interval (CI) 1.002-1.008, P = 0.004], Delta PSA (OR: 14.7, 95% CI 1.50-143.7, P = 0.02) and MTVw1 (OR: 11.4, 95% CI 1.11-116, 6, P = 0.04) were independent prognostic factors associated with mortality risk. Conclusion A statistically significant concordance and correlation was noted between Ga-68-PSMA PET/CT-derived whole-body metabolic parameters (Delta TLP and Delta MTV) and Delta PSA. In addition, the baseline PSA, Delta PSA, Delta TLP, Delta MTV and TMTV were identified as predictive factors for mortality risk.

    Hungry Bone Syndrome in Patients with End-Stage Renal Disease Receiving Hemodialysis

    Objective: Hyperparathyroidism develops in the majority of patients with end-stage renal disease receiving hemodialysis. Parathyroidectomy can be performed in cases who cannot be managed with medical therapy and a portion of the patients develop hungry bone syndrome (HBS) in the postoperative period. In this study, we investigated the factors that influence the development of HBS and the hospitalization times in hemodialysis patients who underwent parathyroidectomy. Method: This study included 49 patients. Demographic, clinical and laboratory parameters of these patients were retrospectively evaluated. Results: Patients’ median age was 46 years (22-62). The parathyroid gland that showed hyperplasia the most frequently was the left inferior 79.6% (n=39), followed by the right inferior 77.6% (n=38) gland. Rate of four-gland hyperplasia was 32.7% (n=16). Of the 49 operated patients, 34 (69.4%) developed HBS. In patients with postoperative PTH 4 ampules during the first 24 hours, hospitalization times were prolonged and the need for parenteral calcium infusion was elevated. Conclusion: The need for parenteral calcium replacement during the first 24 hours, postoperative PTH and calcium levels during the first 24 hours were determined to be factors indicating the severity of HBS in the postoperative period.

    A Low body fat mass ratio predicts poor prognosis in patients with advanced non-small cell lung cancer

    Purpose The aim of this study was to investigate the effect of the body fat mass ratio on survival and prognosis in advanced non-small-cell lung cancer patients. Methods The study includes 200 patients who were diagnosed with advanced non-small-cell lung cancer between 2014 and 2018 and whose body fat mass percentage and body mass index (BMI) were determined using the Tanita Body Composition Analyzer during admission. Results All patients had advanced incurable non-small-cell lung cancer (30% had locally advanced disease, 70% were stage IV). In the univariate and multivariate analyses, age, gender, histopathological type, smoking history, comorbidities, weight loss in the last six months and body mass index had no statistically significant effect on survival (p > 0.05). However, the performance status (p = 0.008), metastatic status (p = 0.003) and body fat mass ratio (p = 0.01) were found to have a significant effect on overall survival (OS): the median OS was 16.4 mo, in patients with the BFM ratio <= 22% and 29.2 mo, in those with > 22% (p = 0.01). Conclusion In this study, it was found that the body fat mass ratio was an important prognostic factor in patients with advanced non-small-cell lung cancer.

    Mikroalbüminürili esansiyel hipertansif hastalarda renovasküler hipertansiyon sıklığı

    Amaç: Renovasküler hipertansiyon sekonder hipertansiyon nedenleri arasında önemli bir yere sahiptir. Bu çalışmamızda amacımız mikroalbuminürili esansiyel hipertansif hastalarda renovasküler hipertansiyonun sıklığını araştırmaktır. Materyal ve Metod: Bu çalışmaya 2009 yılında Dicle Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Nefroloji kliniğinde yatan veya nefroloji ve genel dahiliye polikliniklerine başvuran rutin takip ve tedavileri yapılan mikroalbuminürili esansiyel hipertansiyonu olan 108 hasta dahil edildi. Hastalardan biyokimyasal, hormonal, hematolojik parametreler için kan örnekleri alındı. Nefropati tayini açısından 24 saatlik idrarda protein, kreatinin ve mikroalbüminüri düzeyi çalışıldı. Hastaların glomerüler filtrasyon hızları MDRD (Modification of Diet in Renal Disease) formülü kullanılarak hesaplandı. Bütün hastalarda EKG ve ECHO bakıldı. Tüm hastalarda renal doppler USG ve renal USG bakıldı. 3 hastada MR Angiografi çekildi. Bulgular: Çalışmamızda 307 hipertansif hasta tarandı. 108’inde (% 35.1) mikroalbuminüri tespit edildi. Çalışmamızda 108 mikroalbuminürili esansiyel hipertansif hastanın 3’ünde (%2.8) renal arter doppler USG’yi takiben MR angiografi ile doğrulanan renal arter stenozu tespit edildi. Çalışmamızda RAS tespit edilen hastaların yaş ortalaması 33.67+/- 18.14, diastolik kan basıncı 95+/-8.6 idi. RAS’u olan hasta grubunda istatistiksel analizde sistolik kan basıncı (p=0.028) ve üre yüksekliği (p =0.048) anlamlı olarak tespit edildi. Sonuç: Mikroalbuminürili esansiyel hipertansif hastaların % 2.8’inde renovasküler hipertansiyon bulunduğunu tespit ettik. Bu değer mikroalbuminüri çalışılmadan yapılan araştırmaların sonuçları ile aynıdır. Hipertansiyonun progresyonunda prediktif değeri olan mikroalbuminürinin tanısal değer taşımadığını, bununla beraber bu konuda geniş ölçekli klinik çalışmaların yapılması gerektiği kanaatindeyiz.     The protective effect of Aspalathus linearis against the experimental gastric ulcer induced by ethanol in rats

    This study aimed to investigate the effect of Aspalathus linearis on the expression levels of major histocompatibility complex (MHC) molecules and their ability to present antigens to CD8, CD68, and T cells in an experimental rat model of gastritis. Furthermore, changes in tumor necrosis factor–alpha (TNF–α), myeloperoxidase, superoxide dismutase, and malondialdehyde levels due to the antioxidant properties of the plant were evaluated. The study included 24 female Wistar albino rats (Rattus norvegicus). A. linearis was administered to the treatment groups of an ethanol–induced gastritis rat model via oral gavage for 5 days the blood and stomach tissues were sampled for analysis. Biochemical evaluation revealed that TNF–α and myeloperoxidase levels decreased and superoxide dismutase activity increased in the A. linearis–treated groups compared with the ethanol–treated group. Histochemical evaluation revealed that the severity of inflammation decreased in the rats with gastritis treated with A. linearis. In addition, the number of cells expressing CD8 and CD68 decreased. The number of MHC–II immunopositive cells was higher in the A. linearis–treated group compared with the control and other treatment groups. A. linearis exhibits a protective effect on gastric mucosa by reducing the severity of oxidative stress owing to its antioxidant, anti–inflammatory, and antiapopitotic properties.

    The protective effect of Pomegranate extract against the experimental gastric ulcer induced by ethanol in rats

    The Pomegranate (Punica granatum) is a commonly found fruit in the Mediterranean and Iran, which has a variety of uses including medicinal purposes, cosmetics, and as a spice in culinary applications. Pharmacological functions of Pomegranate include antioxidation, anti–tumor, anti–hepatotoxicity, anti–lipoperoxidation and antibacterial properties. The aim of this study was to evaluate the therapeutic efficacy of Pomegranate extract by utilizing its antioxidant activity in an experimental rat model of gastritis induced by ethanol. In the study, 24 female Wistar albino rats (180–200 g) were used. Gastritis in rats was induced using Ethanol. In experimental groups, Tumor necrosis factor–alpha, Myloperoxidase, Superoxide Dismutase and Malondialdehyde were examined for biochemical analyzes. Streptavidin peroxidase immunohistochemistry method was applied to gastric tissues with gastritis. A statistically significant difference was observed between Superoxide Dismutase and Meloperoxidase levels. CD8 and CD68 immunoreactivity was higher in the Ethanol group compared to the other groups. A decrease was observed in CD8 and CD68 positive immunoreactivity in Ethanol+Pomegranate extract group compared to Ethanol group. The study found that the immunoreactivity of MHC–I and MHC–II was found in specific locations, namely intraepithelial lymphocytes located in the epithelium, some capillary vessel endothelium, and connective tissue. Changes in anti–oxidative stress markers such as Superoxide Dismutase and Myloperoxidase contributed to the mucosal protective effect of Pomegranate extract in Ethanol–induced gastritis.

    The relationship between infection parameters and urine volume in acute kidney injury

    Aim: Acute kidney injury (AKI) is a clinical syndrome that can cause disturbances in fluid-electrolyte and acid-base balance, resulting in the accumulation of nitrogen and uremic toxins along with the loss of kidney functions within hours or days. In this study, it was aimed to retrospectively examine patients with acute kidney injury to determine whether there is a relationship between infection parameters and urine volume. Materials and Methods: The study included a total of 144 patients with (n=74) and without infection (n=70) out of 294 patients with AKI who received treatment between 1 January 2020 and 31 December 2021 in the nephrology clinic of a tertiary university hospital. Results: The mean age was 66.4±15.7 (range:19-95) in patients with infection and 63.8±15.2 (range:36- 93) in non- infected patients. 51.4% (n=38) of those with infection and 52.9% (n=37) of those without infection were women. There was no difference between the individuals with and without infection in terms of age and gender (p>0.05). Infection was present in 51.4% (n=74) of the patients included in the study. Urinary tract (31.3%) and respiratory tract infections (13.2%) were the most common in those with infection. A moderate negative correlation was observed between admission CRP and discharge creatinine level in patients with infection. There was no correlation between PCT and sedimentation rate, urine volume and admission/discharge creatinine level. Moderate positive correlations were found between admission/ discharge PCT and admission/discharge urine volume in patients without infection. In addition, moderate negative correlations were found between admission/discharge sedimentation rate and admission urine volume. Conclusions: No correlation was found between PCT and sediment (incoming/exit) and outflow urine volume in patients with infection.

    The Role of monocyte to high-density lipoprotein cholesterol ratio in predicting the severity of proteinuria and renal dysfunction in primary nephrotic syndrome

    Introduction: Monocyte to high-density lipoprotein (HDL) cholesterol ratio (MHR) has emerged as a novel marker of endothelial injury, inflammation, and oxidative stress. This study aimed to investigate the effect of MHR on primary nephrotic syndrome (NS) and its relationship with the severity of proteinuria. Methods: This study enrolled 161 patients newly diagnosed with primary NS and 100 healthy individuals. Demographic characteristics of the patients, primary NS diagnosis, basal laboratory parameters, the amount of 24-hour urinary protein excretion, and MHR were recorded. The groups were compared regarding these parameters. Results: MHR was significantly higher in patients with primary NS compared with the healthy group (12.89 +/- 4.86 and 9.71 +/- 2.30, respectively; p < 0.001). There was no difference between the groups in terms of age and sex. The amount of protein in the 24-hour urine in patients with a diagnosis of primary NS was 6.91 +/- 3.73 g/day. The correlation analysis showed a positive correlation between MHR and the amount of proteinuria (r = 0.519, p < 0.001) and creatinine level (r = 0.167, p = 0.034). The multivariate regression analysis found that the severity of proteinuria was independently correlated to MHR (p < 0.001). According to the receiver operating characteristic curve analysis, the optimal cut-off level for MHR in NS was 10.08 (area under the curve of 0.704, sensitivity of 68%, and a specificity of 62%). Conclusion: Our study is the first to compare the severity of proteinuria and renal functions with MHR in patients with primary NS. We believe that MHR can be used as a biomarker to determine inflammation, endothelial injury, and the level of oxidative stress, and may be useful to predict prognosis in patients with primary NS.

    Thiol-disulphide homeostasis and ischemia-modified albumin level and its relationship with clinicopathological features of breast cancer

    Objective: To investigate the status of thiol-disulphide homeostasis and ischemic-modified albumin and their association with clinicopathological parameters in breast cancer. Study Design: A cross-sectional descriptive study. Place and Duration of Study: Department of Internal Medicine and Department of Medical Oncology, Dicle University, Turkey, from April to September 2021. Methodology: Forty treatment-naive female patients diagnosed with breast cancer who presented to the Oncology Clinic of the hospital and 33 healthy women with no comorbidities were included. Serum levels of native thiol (NT), disulphide (Ds), total thiol (TT), IMA (ischemic modified albumin) at diagnosis and disulphide/native thiol (Ds/NT), disulphide/total thiol (Ds/TT), and the ratios of native thiol/total thiol (NT/TT) were analysed by the colorimetric method. Results: Median age at diagnosis was 44 (29-70) years. The majority of patients had stage II-III disease (77.5%). Mean serum levels of TT were significantly lower in breast cancer patients (462.45 100.2 umol/L) compared to healthy controls (507.28 75.72 umol/L) (p=0.038). Mean serum levels of Ds were significantly lower in breast cancer patients (20.25 5.94 umol/L) compared to healthy controls (22.99 3.56 umol/L) (p=0.018). Meanwhile, mean IMA levels were significantly higher in breast cancer patients (0.81 0.05 umol/L) compared to healthy controls (0.73 0.19 umol/L) (p=0.016). NT and TT levels showed a moderate correlation with the percentage of fat mass and body mass index (BMI), and a weak correlation with age (p0.05). Univariate and multivariate analyses examining the association between thiol-disulphide levels and patient clinical characteristics demonstrated that NT and TT levels had a statistically significant relationship with body mass index and menopausal status (p0.05), with lower levels of NT and TT in postmenopausal patients and patients with high BMI. Conclusion: Decreased TT and Ds levels and increased IMA levels were determined in patients diagnosed with breast cancer compared to the healthy control group. Thiol-disulphide levels were observed to be associated with clinical characteristics such as menopausal status and BMI.

    Two malignant mesothelioma cases with brain metastasis

    A 32-year-old male patient presented to our clinic in February 2014 with complaints of chest pain and dyspnea, which lasted for the last six months. In the xray examination,the right sinus was closed and there was the pleural effusion (Figure 1).
