    Biochemical and Histopathological Examination of the Effect of Cigarette Smoking on Rat Kidneys

    OBJECTIVE: To study the negative effects of cigarette smoke on the kidney, and to determine if those effects are reversed after smoking cessation. STUDY DESIGN: A total of 40 rats were divided into 5 groups of 8 each: group I, control group; group II, rats exposed to cigarette smoke; group III, rats that discontinued smoking for 1 month; group IV, rats that discontinued smoking for 3 months; and group V, rats that discontinued smoking for 5 months. Serum urea, creatinine, cotinine, IL-18, NGAL, and KIM-1 levels were measured in the blood samples. RESULTS: Cotinine levels in all groups were similar to those of the control group, but only the nicotine group had a significant increase. The active smoker group exposed to cigarette smoke displayed dense vacuolization. As a result of the exposure to cigarette smoke, increasing levels of urinary creatine, microalbumin, and beta-2 microglobulin and urea in serum led to changes in KIM-1 and NGAL values, which result in dysfunction of glomeruli and tubules. CONCLUSION: We observed that smoking cessation may lead to improvement after 3 months in both biochemical parameters and glomerular and tubular changes and approaches their normal values/structures 5 months later.

    Cerebrospinal fluid levels of sortilin-1, lipocalin-2, autotaxin, decorin and interleukin-33 in patients with idiopathic intracranial hypertension

    Background Idiopathic intracranial hypertension (IIH) is characterized by increased cerebrospinal fluid (CSF) pressure of unknown cause. It has been suggested that the inflammatory process plays a role in the pathophysiology of the disease. Sortilin-1, lipocalin-2, autotaxin, decorin, and interleukin-33 (IL-33) are among the factors involved in inflammatory processes. Objective To investigate the CSF levels of sortilin-1, lipocalin-2, autotaxin, decorin, and IL-33 in patients with IIH. Methods A total of 24 IIH patients and 21 healthy controls were included in the study. Demographic characteristics of the patients and of the control group as well as CSF pressures were evaluated. Sortilin-1, lipocalin-2, autotaxin, decorin and IL-33 levels in the CSF were measured. Results The CSF levels lipocalin-2, sortilin-1, autotaxin, IL-33 and CSF pressure were significantly higher in the patients group compared with the control group (p < 0.001). Decorin levels were reduced in patients (p < 0.05). There was no correlation between the autotaxin and IL-33 levels and age, gender, CSF pressure, and body mass index. The results of our study showed that inflammatory activation plays an important role in the development of the pathophysiology of IIH. In addition, the fact that the markers used in our study have never been studied in the etiopathogenesis of IIH is important in explaining the molecular mechanism of this disease. Conclusion Studies are needed to evaluate the role of these cytokines in the pathophysiology of the disease. It is necessary to evaluate the effects of these molecules on this process.

    Clinical significance of increased serum neopterin in chronic kidney failure as a biomarker of cell-mediated immunity

    Background: Neopterin is a pyrazino-pyrimidine compound which is used as a marker of cell-mediated immunity in a variety of diseases. It is known that neopterin levels increase in diseases where interferon-gamma (IFN-g) stimulation is present, and also as a result of deficiencies in renal function, given that the primary means of elimination of neopterin is through the kidneys. In this study, we aimed to investigate the role of increased neopterin levels as a prognostic biomarker in patients with impaired renal function. Methods: A total of 90 individuals including 63 patients with chronic kidney failure (CKF) and 27 healthy volunteers were included in the study. Serum neopterin concentrations were measured using the enzyme-linked immunosorbent assay. A Mann-Whitney U test and a Pearson Correlation Test were used in the statistical analysis, with a p value of <0.05 being considered statistically significant. Results: The mean age was 52.21±0.16 years in the patient group and 56.55±0.32 years in the control group. In the CKF patients, serum neopterin levels increased to a significantly greater degree than in the control group (p<0.001), while no statistically significant correlation was identified between serum neopterin levels and age (p>0.05). Conclusions: A significant increase was found in the serum neopterin levels in the CKF patients, due to both the triggering of the disease and the reduction of neopterin elimination.

Genetik, çevresel faktörler ve yaşam tarzı değişikliklerinin etkileşimi nedeniyle insülin salgılanması,insülin etkisi veya her ikisinde oluşmuş defektlerden kaynaklanır. Son derece ciddi ve ilerleyici bir hastalık olmasının yanı sıra, kontrol sağlanamadığında akut ve kronikkomplikasyonlara yol açıp morbidite ve mortaliteyi olumsuz etkilemesiyle hem birey hem de toplum için büyük bir sağlık sorunu olarak karşımıza çıkmaktadır.Diyabetin toplumun her kesiminde önlenmesi için azami gayret gösterilmelidir. Bu konuda en büyük görev birinci basamak hizmetlerindeki eşsiz rolleri nedeni ile aile hekimlerine düşmektedir. Bu yüzden birinci basamak sağlık çalışanları, hastaları diyabet risk faktörleri açısındandeğerlendirmeli ve diyabet gelişimini önleyici tedbirleri almalıdır. Gerekli kişilerde diyabet taraması yapmalıdır. Di- yabeti olan hastalarda ise komplikasyonları önlemek içintedavi ve kontrollerini takip etmelidir. Önleme ve tedavi multidisipliner yaklaşım gerektirdiğinden hastayı nefroloji, göz, kardiyoloji vb. bölümlere de sevk etmelidir. Bu amaçla makalede diyabet ve diyabetik hastaya nasıl yaklaşılması gerektiği incelenmiştir.

    Evaluation of acute stroke patients applying to a university emergency service in terms of risk factors

    Aim: Stroke is a group of diseases that causes morbidity and loss of function at the highest rate worldwide that mostly affects the quality of life and is the second leading cause of death after heart diseases. In the present study, we aimed to determine the risk factors of the patients who applied to the emergency service and their distributions. Material and Method: Our study was formed retrospectively by evaluating the follow-up files of 194 patients who applied to Dicle University Medical Faculty Emergency and Traumatology Polyclinic and were diagnosed with stroke and were followed between June1, 2014 and June 1, 2016. Results: Of the 194 patients included in the study, 98 (51%) were female and 96 (49%) were male, the mean age was 69.6 +/- 13.4. One hundred and seventeen of patients (60%) were diagnosed with ischemic stroke, 77 of them (40%) were diagnosed with a hemorrhagic stroke. The mean age of men and women were 69.9 +/- 10 and 69.3 +/- 15.7 years respectively. According to the types of stroke, the mean age for those with ischemic stroke was 69.9 +/- 12.6, for those with hemorrhagic stroke was 69.3 +/- 14.8. In both types of stroke, the values of systolic and diastolic blood pressure were higher than normal. When the age of the patients was compared in terms of both sex and type of stroke, no significant difference was determined in terms of age in the groups. The incidence of stroke was greater between the ages of 45 and 84 in 164 patients (84.5%). Discussion: The average age of the patients was in the advanced group and their average tension values were higher than normal. For this reason, advanced age and high blood pressure were considered as risk factors contributing to stroke development in our study group. It is significant to evaluate the risk factors of the patients during the continuous health services given to the patients and to provide appropriate treatment and regular follow-ups for those with risk factors.

    Evaluation of empathy and biopsychosocial approaches of medical faculty assistant doctors

    INTRODUCTION AND AIM: Biopsychosocial approach suggests that the individual's health condition is partly influenced by the interactions of biological, psychological, and social processes. It also concludes that the clinical method should be patient-centered, which is based on the physician's involvement in the patient's world and the physician's ability to see the illness from the patient's view. In this study, we aimed to determine the extent to which medical faculty assistant doctors use the biopsychosocial approach in their clinical practice. MATERIALS AND METHODS: Our study is a cross-sectional type, descriptive, and analytical research. The sample size consisted of assistant doctors who met the inclusion criteria which were accepted to participate in the study between December 1, 2017, and March 1, 2018, at the medical faculty hospital of Dicle University. Two hundred and three physicians participated in our study. The Patient-Physician Orientation Scale (PPOS), The Jefferson Scale of Physician Empathy (JSPE), and a survey study including questions in relation with sociodemographic characteristics were conducted in our study. The reliability of the Patient-Physician Orientation Scale (PPOS) and JSPE used in our study was tested with Cronbach's alpha and found to be 0.878 and 0.931, respectively. Questionnaires were administered by a face-to-face interview method during interview. Information on the purpose and method of the study was given. RESULTS: A total of 203 physicians participated in our study. One hundred and forty-three (70%) participants were male and 60 (30%) were female. According to medical disciplines, eight physicians (3, 9%) from psychiatry, 21 (10, 3%) from family medicine, 104 (51, 2%) from other internal medical disciplines, and 70 (34, 5%) from surgical medical disciplines participated in the study. In the first subgroup of the PPOS, which is used in our study, the average score of psychiatry physicians was 3.67 +/- 0.99, family medicine assistant doctors 3.92 +/- 0.67, other internal medical disciplines 3.98 +/- 0.65, and surgical medical discipline 3.83 +/- 0.61. In the second subgroup, which is defined as the care in the same scale, the average score of psychiatry assistant doctors was found to be 3.20 +/- 0.45, family medicine assistant doctors 3.68 +/- 0.54, other internal medical disciplines 3.49 +/- 0.60, and surgical medical discipline 3.68 +/- 0.58. The results of the Jefferson Scale used to determine the level of empathy in our study are found as psychiatry 78.25 +/- 14.78, family physician 67.71 +/- 14.69, other internal medical disciplines 72.03 +/- 13.46, and surgical medical discipline 70.87 +/- 15.24. CONCLUSIONS: The biopsychosocial approach is primarily an elementary approach model in medical discipline such as family medicine and should be applied to other internal and surgical medical disciplines as well. With conducting this study, the awareness of this area will be increased and related issues in the literature will be enriched.

    Evaluation of serum adipocytokine and interleukin-18 levels in patients with epilepsy

    Objective: Epilepsy is a neurological disease characterized by recurrent seizures. The underlying pathophysiological mechanisms in epilepsy are not fully known. Our aim is to investigate the relationship between serum adipocytokine and interleukin (IL)-18 levels in epilepsy patients receiving and not receiving antiepileptic therapy. Method: Our study was established as three groups. I: Epilepsy patients receiving antiepileptic therapy (n=30), II: Newly diagnosed epilepsy patients (n=30) and III: Control group (n=30). Serum adipocytokine and IL-18 levels were measured by enzyme-linked immunoassorbent assay method. Results: It was determined that serum adipocytokine and IL-18 levels were increased in epilepsy patients who received topiramate treatment and did not receive antiepileptic therapy compared to the control group. Serum glucose, total protein, cholesterol and albumin concentrations of patients who received antiepileptic treatment were decreased compared to the control group (p0.05).     Gebelikteki aneminin doğum şekli ve yeni doğan üzerine etkileri

    Amaç: Çalışmamızın amacı gebelikteki aneminin doğum şekli ve yeni doğan (YD) üzerine etkilerini değerlendirmektir. Yöntemler: Haziran-Ekim 2009 arasında doğum yapan 307 gebede hemoglobin (Hb), hematokrit (Hct) değerleri ve doğum şekli retrospektif olarak değerlendirildi. Ayrıca 1.dakika ve 5.dakika apgar skorları, doğum ağırlıkları ve yeni doğanların kordon kanından elde edilen Hb, Hct ve bilirubin değerleri kaydedildi. Gebeler, Hb değeri 11,1 g/dl ve üzerinde olanlar anemik olmayan ve 11g/dl altında olanlar ise anemik olmak üzere iki gruba ayrılarak sınıflandırıldı. Ayrıca Hb değeri 10,1–11 g/dl, 9,1-10 g/dl ve 9 g/dl’nin altında olanlar şeklinde anemik grup kendi içinde 3 gruba ayrıldı. Bulgular: Çalışmaya alınan gebelerin 146’sı anemik iken, 161’i anemik değildi. Düşük doğum ağırlıklı bebek oranları anemik gebelerde anlamlı olarak daha yüksekti (p=0,029). Anemik gebelerin yeni doğanlarında Hb (P=0,026) ve Hct P=0,006) değerlerinin anlamlı olarak daha düşüktü. Düşük doğum ağırlığı sıklığı, anne Hb değeri 10g/dl ve altında olduğunda anlamlı olarak artmaktaydı (%62,5 duyarlılık, %74,7 özgüllük). Sonuç: Anemik gebelerin yeni doğanlarında düşük doğum ağırlığı ile düşük Hb ve Hct değerleri daha sık görülmektedir. Bu nedenle gebelik esnasında anemi muhtemel olumsuz sonuçları nedeniyle taranmalı ve tedavi edilmelidir.

    Introduction: Defensive medicine is the practice of physicians to use the unnecessary procedures for diagnosis and treatment in order to protect themselves against medical malpractice cases, or to avoid taking the responsibility of high-risk medical practices which are likely to result in a malpractice case. The present study was carried out to reveal the knowledge and attitudes of research assistants working in internal and surgical medical sciences at Dicle University Medical Faculty Hospital about the defensive medicine applications and the factors affecting work-related stress and burnout levels. Method: The sample of this descriptive-cross-sectional type of study was designed by proportional layer method. The questionnaire, which included the sociodemographic data form, the defensive medicine applications attitude scale and the maslach burnout ınventory, was applied to 200 physicians. Results: Of all the participants whose mean age was 29.4±3.0, 70.5%(n=141) were male, 56%(n=112) were married. The frequency of positive and negative defensive medicine was found as 98%(n=196) and 92 % (n=184) respectively. The points of defensive medicine were significantly high in the males, those working at surgical departments, those in the first two years of the specialist training in medicine, those the malpractice case opened about and smokers. The level of work-related tension was significantly high in the ones working at surgical departments, ones who keep watch for eight times or more per month, those the malpractice case opened about, smokers and alcohol users. The average points which the physicians took from Maslach Burnout Inventory were determined as 31.57±11.67 for emotional exhaustion, 11.75±6.49 for desensitization and 29.46±7.72 for personal accomplishment. Conclusion: In our study, it was concluded that most of the research assistants were not satisfied with the choice of profession and specialty, their knowledge of defensive medicine concept was inadequate and their high burnout rates were found in all burnout dimensions. It was seen that the burnout levels of those who applied more to defensive medicine were higher.

    High Social Anxiety and Poor Quality of Life in Patients With Pulmonary Tuberculosis

    Pulmonary tuberculosis (PT) has been previously related with various psychosocial adverse consequences including stigmatization and social isolation. Social anxiety is a psychiatric condition that may be associated with social isolation and fear of social exclusion. To date no study has investigated social anxiety and its impact on quality of life (QoL) among patients with PT. Therefore, we aimed to determine the severity of social anxiety in a group of patients with PT. Among patients who were recently discharged from hospital with the diagnosis of PT 94 patients and 99 healthy control subjects who had similar demographical features have been included in the study. A psychiatrist interviewed all participants and a semistructured interview form, which was prepared by the authors, Liebowitz Social Anxiety Scale (LSAS), and Short Form-36 were administered to them. Patients with PT showed higher levels of performance avoidance and social avoidance than healthy control subjects. They reported lower QoL scores across all dimensions. Among patients women showed higher levels of LSAS subscale scores and total score. Fear of social exclusion was predicted by perceived illness severity and emotional role difficulty. On the other hand, perceived illness severity was predicted by fear of exclusion and sedimentation level. PT patients seem to experience higher levels of social anxiety and associated fear of social exclusion that add to their worse QoL during the earlier months of their disease. Among them fear of social exclusion is related with perceived illness severity.

    Hipertansif Hastalarda Tip II Diabetes Mellitusun Renal Parametreler Üzerine Etkisi: Karşılaştırmalı Bir Çalışma

    Amaç: Diabetes mellitus ve hipertansiyon, toplumda yaygın olarak görülen iki hastalık grubu olup bir çok sistemi etkileyerek, mortalite ve morbiditeyi arttırmaktadır. Bu çalışmanın amacı, hipertansiyon ile hipertansiyona ek olarak Tip 2 diabetes mellitus tanısı olan hastaların, renal fonksiyonlarının incelenmesi ve bu hastalıkların böbrek üzerindeki etkilerinin karşılaştırılarak değerlendirilmesidir. Yöntem: Çalışmaya, İstanbul Eğitim Araştırma Hastanesi dahiliye ve diyabet polikliniklerinde ayaktan takip edilen, 35-70 yaşları arasında 85 hipertansiyon, 85 Tip 2 diabetes mellitus ve hipertansiyon tanılı, toplam 170 adet hasta arşiv dosyaları retrospektif olarak taranarak dahil edildi. Her iki gruptan, klinik bulgular ve demografik özellikler taranarak kayıt edildi. Bulgular: Tip 2 diabetes mellitus ve hipertansiyon grubunda, albüminüri oranı (%30,6), hipertansiyon grubundan (%16,5) anlamlı olarak daha yüksekti (p=0,047). Glomerüler filtrasyon hızı

    Increased visinin-like protein-1, YKL-40, lipocalin-2, and IL-23 levels in patients with migraine

    Background: Migraine is a type of primary headache caused by changes in the trigeminal system and has been reported to be associated with neurovascular inflammation of cerebral and extracerebral vessels.Objective: It is known that inflammation is an important process in the pathogenesis of migraine. It has been shown that the molecules of visinin-like protein 1 (Vilip-1), YKL-40, lipocalin-2 and interleukin (IL)-23 play a role in the inflammatory process. Our aim is to investigate the role of this molecule in the metabolic pathway of migraine disease.Methods: Fifty migraine patients with and without aura in the interictal period were included in the study. Vilip-1, YKL-40, lipocalin-2, and IL-23 levels were measured by ELISA method.Results: Serum vilip-1, YKL-40, lipocalin-2, and IL-23 levels were found to be significantly higher in migraine patients compared to the control group. We found that this molecule increased significantly in migraine subgroups compared to the control group (p < 0.001). A positive significant correlation was found between vilip-1 level and YKL-40 and lipocalin-2 levels in migraine patients. In addition, a positive correlation was observed between visual analogue scale score, number of days with pain and vilip-1 level (p < 0.01). The results of our study showed that activation of inflammatory mediators may play a role in the pathogenesis of migraine disease. In addition, our study is valuable in that inflammatory molecules are high in the interictal period and these biomarkers have never been analyzed in migraine patients. However, we still believe that larger studies are needed to explain the role of vilip-1, YKL-40, lipocalin-2, and IL-23 in the molecular mechanism of migraine disease.

    Investigation of serum vaspin, visfatin, chemerin and IL-18 levels in migraine patients

    Background: Migraines are headaches caused by changes in the trigeminovascular metabolic pathway. Migraine headache attacks are associated with neurovascular inflammation, but their pathophysiological mechanisms have not been fully explained. Objective: To investigate the relationship between serum vaspin, visfatin, chemerin and interleukin-18 (IL-18) levels and the frequency of attacks in migraine headache. Methods: Three groups were established: migraine with aura (n = 50), migraine without aura (n = 50) and control group (n = 50). The migraine diagnosis was made in accordance with the International Classification of Headache Disorders-III beta diagnostic criteria. The analyses on serum vaspin, visfatin, chemerin and IL-18 levels were performed using the enzyme-linked immunosorbent assay method. Results: The serum vaspin, visfatin, chemerin and IL-18 levels were found to be significantly higher in the migraine patients than in the control group (p 0.01). No statistically significant differences in serum vaspin, visfatin, chemerin and IL-18 levels were found among the migraine patients during attacks or in the interictal period (p 0.05). The serum visfatin and chemerin levels of the migraine patients were positively correlated with their serum IL-18 levels (p < 0.01), while their serum chemerin and visfatin levels were positively correlated with their serum vaspin levels (p < 0.05). Conclusions: This study showed that these biomarkers may be related to migraine pathogenesis. Nonetheless, we believe that more comprehensive studies are needed in order to further understand the role of vaspin, visfatin, chemerin and IL-18 levels in the pathophysiology of migraine headaches.

    A microbiological assessment of the oral hygiene of 24-72-month-old kindergarten children and disinfection of their toothbrushes

    Background: The objective of this study is to assess the index of decayed, missing and filled teeth (DMF-T), habit of brushing teeth, and the microbiological agents accumulating on the children's toothbrushes for 4 weeks and response of these agents to disinfection via a chlorhexidine solution, then compare those results with the education and income levels of the children's parents. Method: Included in the study were 187 children (96 in the control group and 91 in the experiment group - chlorhexidine) chosen randomly from 600 kindergarten children whose ages ranged from 24 months to 72 months. The children selected had not taken any antibiotics, antimicotics for three months and dental treatments during this trial. The distribution of these children to the groups was also done randomly. After performing a survey for the education, occupation, and income status of the parents, the children were examined and the number of decayed teeth was recorded. The children were given toothbrushes, toothpaste (with fluroide), and the solutions (including distilled water and chlorhexidine) for four weeks under the condition that toothbrushes were returned at the end of each week. The 14 different microbiological agents observed as a result of the assessment of the samples taken in the first week were also included in the assessments of the samples taken over the four-week period. Results: The decrease in the DMF-T index was found to be meaningful according to the differences in education, income, and occupation status of the parents. Of all the samples taken from the toothbrushes, the bacteria with the greatest rate of reproduction included Streptococcus mutans, Escherichia Coli, Pseudomonas aeuroginosa, Enterococcus spp, Staphylococcus epidermidis and Candida albicans. Except for Candida albicans, the other microorganisms taken as samples from the toothbrushes reproduced less overall. In the group using the solution with chlorhexidine, a meaningful decrease in bacterial reproduction was discovered compared to the control group. Conclusion: The findings of this study show that the education, occupation, and socioeconomic situations of the parents should be considered when discussing children's oral and dental health. Moreover, the study shows that disinfection of toothbrushes in order to prevent reinfection and contamination oral flora with the bacteria again is important in terms of preventive medicine and family-children health.

    Potential prognostic role of immune system activation marker neopterin in patients with type 2 diabetes

    Background: An increase in neopterin concentrations is known in some pathologies due to interferon-gamma (INF-γ) activation. These include viral and bacterial infections, auto immune diseases, metabolic diseases, psychiatric disorders, tissue and organ rejections, and different malignancies. The aim of this study was to evaluate the role of neopterin as a prognostic biomarker in type 2 diabetes, which is a metabolic disease with a high worldwide prevalence. Methods: The study included a total of one hundred thirtynine individuals including one hundred and six patients admitted to a family medicine outpatient clinic and diagnosed with type 2 diabetes and thirty-three healthy volunteers. Serum neopterin concentrations were measured using the enzyme-linked immunosorbent assay. Results: Serum neopterin levels significantly increased in type 2 diabetes patients, compared to the control group (p<0.001). Conclusions: Early diagnosis of diabetes and determination of the appropriate therapeutic options are of utmost importance, as diabetes is also associated with other systemic diseases. The risk of developing secondary diseases is high in untreated patients. Our study results suggest that serum neopterin may be a useful biomarker in patients with type 2 diabetes.

    The prevalence of diabetes mellitus, dysglycaemia and factors that affect them in public employees of Kahramanmaras

    Aim: Our aim is to determine the prevalence of diabetes mellitus, impaired fasting glucose and sociodemographic characteristics of employees in public institutions and organizations in Kahramanmaras province and to assess the effect of these variables in the development of diabetes mellitus. Methods: The work, which was planned as an observational, cross-sectional and an analytical type study, was conducted with 1201 male and female participants between 01/10/2013 and 01/12/2013 in the city of Kahramanmaras. The age, gender, body mass index (BMI), systolic and diastolic blood pressure, marital status and educational status of participants were recorded. The association between these parameters and diabetes mellitus or impaired fasting glucose (IFG) were evaluated Results: The mean age of the study participants 42.86 ± 9.56, of those 79% were male, 21% were women.DM was observed in 120(10%) persons, BAG was observed in 373 (31%) persons, hypertaension was observed in 218 (18,1%) persons. Marital status single / widowed patients with DM and IFG rate of sequentially 4.7% and 24.3% , In the married respectively; 10.7% and 31.7% (p< 0,05). DM and IFG ratio were significantly different in participants who were graduated from primary school 15% , IFG is 27.9% , in the high school graduates respectively; 11.6% and 29.8% while university graduates were respectively; 6.6% and 29.9% (p< 0,05). The rate of DM and BAG were respectively; 15% , 35.3% in first-degree relatives in the story of DM, while there were 6,8% , 28.2 % and in patients with no DM family story (P<0.05). Conclusion: An increase in the level of education to reduce the incidence of diabetes and family history, marital life were found to increase the rate of DM and IFG. We obtain the finding that increased the level of DM and IFG when increse the Body Mass Index (BMI).Firstly, the identification of risk groups and lifestyle changes, such as; exercise, appropriate training, etc., were eligible to be given to this group by the current practice of family medicine units.

    The relationship between neopterin and hepatitis B surface antigen positivity

    Hepatitis B is a life-threatening viral liver infection caused by the hepatitis B virus. Neopterin is regarded as an immunologic biomarker of several diseases related to activation of the cellular immune system. Hepatitis B infection is associated with increased production of cellular immune system markers. We aimed to investigate whether there is a relationship between hepatitis B surface antigen-positivity (HBsAg +) and neopterin to determine the role of neopterin in the early diagnosis of hepatitis B infections. Seventy-two HBsAg (+) patients with normal liver function tests and forty-three controls were included in the study. Neopterin levels were 17.6 ± 0.13 nmol/L in HBsAg (+) patients; and 9.12 ± 0.09 nmol/L in infection-free controls, respectively. Compared to the control group, a statistically significant increase (p < 0.001) in the serum neopterin levels of the patients was observed. No significant relationship was determined between neopterin levels and age/sex (both, p > 0.05). With overstimulation of interferon-gamma, the production of neopterin increases by monocytes/macrophages. Likewise with other diseases associated with an activated cellular immune system, this study shows that neopterin can be a predictive biomarker for persistent carriers of hepatitis B infection.

    The Relationship between Obesity and Cardiovascular Risk in Postmenopausal Women

    Objective: The aim of this study was to investigate the relationship between obesity and cardiovascular risk in postmenopausal women. Methods: The study included 43 postmenopausal women who were admitted to Dicle University Medical School Family Medicine Outpatient Clinic. Body mass index (BMI), waist and hip circumferences, body fat mass and percentage, 24-hour arterial tension measurements, homocysteine (Hcy), lipid and vitamin B12 levels, and Framingham risk score (FRS) were compared between obese (BMI >= 30 kg/m(2)) and non-obese (BMI<30 kg/m(2)) patients. Results: The patients included 25 (58.1%) obese and 18 (41.9%) non-obese patients. Abdominal obesity was seen in 29 (67.4%) and truncal obesity in 18 (41.9%) participants. Family history of cardiac disease was seen in 25 (58.1%) women, most of whom were obese. Based on FRS, moderate cardiovascular risk was assessed in 21.6%, but none of the participants were at high risk. FRS was positively correlated with Hcy and waist/hip ratio. Conclusions: The results of this study indicate that age- and gender-dependent differences must be taken into consideration for cardiovascular risk assessments and postmenopausal women should be informed about obesity and hypertension in order to improve their quality of life.

    Vaginal Birth After Repeated Cesrean Section: A Case Report

    Introduction: Cesarean section is a risky procedure. In most of the world, trial of vaginal birth after cesarean section (VBAC) is suggested and performed. In this case report, we aimed to show the possibility of vaginal labor in a patient who underwent repeated cesarean section and to review the risk of cesarean section. Case Presentation: A female patient aged 32 years, who had cesarean section three times (gravida 5, parity 3, abortion 1) with 3 children, came to the delivery service when she went into final labor pain. On examination, it was seen that there was full dilatation of cervix and the fetus was in the vertex position. With episiotomy given and normal spontaneous vaginal delivery, a live baby boy of 3280 grams, 50 cm and Apgar score of 6-8 (at first and fifth minutes) was delivered. Having normal vital findings, the patient was discharged from the hospital on these conditions postpartum day 1. Conclusion: Cesarean section increases maternal, prenatal mortality and morbidity. In health centers which have operative conditions, the option of normal spontaneous delivery should not be ignored for appropriate pregnant women.
