Öğe Massive Pulmonary Embolism with Mobile Right Atrial Thrombus in a Pregnant Patient Using Valproic Acid(Aves, 2012) Soydinc, Hatice Ender; Yuce, Murat; Sari, Emine; Yavuz, Sema; Sari, Ibrahim; Davutoglu, Vedat; Yigiter, RemziPulmonary embolism is a life threatening condition. Pregnancy is among the risk factors of pulmonary embolism. There is reluctance about using thrombolytics s during pregnancy due to potential teratogenic effects. Valproic acid is one of the most commonly used antiepileptic agents, hovewer, its use during pregnancy is controversial because of the teratogenic potential. Among adverse effects of valproic acid use, elevated liver enzymes, thrombocytopenia have been reported. Moreover, product literature warns that pregnant women using valproic acid should be monitored for low fibrinogen and platelets and the potential for bleeding. The present case report describes successful treatment of a massive pulmonary embolism with tissue plasminogen activator (t-PA), caused by a mobile right atrial thrombus in a 30-year-old first trimester pregnant women using valproic acid. This has not been reported previously. During follow-ups, she was free of any signs or symptoms and gave birth to a baby by spontaneous vaginal delivery. The baby was completely healthy without any problem or deformity.Öğe Mean platelet volume in functional and thrombotic prosthetic mitral valve(Lippincott Williams & Wilkins, 2012) Tekbas, Ebru; Akdemir, Ilyas; Soydinc, Serdar; Alici, Hayri; Yavuz, Fethi; Yuce, Murat; Cakici, MusaThe mean platelet volume (MPV) values reflect platelet size and are accepted as marker of platelet activation. We sought to test the hypothesis that platelet activation occurs independently from presence or absence of thrombus in prosthetic mitral valve. A total of 168 patients were included in the study. Study participants were divided in three groups: group 1 (n=62) - patients with normal prosthetic mitral valve; group 2 (n=37) - patients with prosthetic mitral valve thrombosis; and group 3 (n=69) - healthy individuals. MPV values were significantly higher in normal and thrombotic prosthetic mitral valve patients than in healthy individuals (P=0.008 and P=0.01, respectively). MPV values were not different between normal prosthetic mitral valve and thrombotic prosthetic mitral valve. This is the first study indicating that increased MPV is present in normal and thrombotic prosthetic mitral valve, implying that platelet reactivity occurs in prosthetic mitral valve irrespective of development of thrombus formation. No reductions were observed in the thrombus size or mean mitral gradient on transesophageal echocardiography (TEE) with 1 week of unfractioned heparin therapy. As the thrombus on TEE imaging was hypermobile and fragile, in addition to a higher dose of enoxaparin (80 mg twice daily), trofiban infusion 0.20 mu g/kg per minute was administered for another 1 week. The thrombus on the valve was reduced in size, mobility and fragility of the thrombus diminished, and mean valve gradient decreased on TEE. As complete thrombus resolution was not observed and limitation of valve mobility continued, tissue plasminogen activator (tPA) was given to the patient. A complete thrombus resolution was observed on this therapy. The patient is presented for being the first case in literature whose valvular thrombus reduced with trofiban therapy.Öğe Staphylococcus Hominis Native Tricuspid Valve Endocarditis Following Spontaneous Abortus(Emergency Medicine Physicians Assoc Turkey, 2012) Soydinc, Hatice Ender; Yuce, Murat; Sari, Emine; Sari, Ibrahim; Yildirim, Cuma; Davutoglu, Vedat; Ercan, SuleymanThe patient had a spontaneous abortion in the fifth week of her pregnancy and had nonspecific treatment due to fever and chills that had started 3 days after the spontaneous abortion. She was diagnosed and treated as endocarditis as there was vegetation on the tricuspid valve on transthoracic echocardiography, and blood culture revealed Stphylococcus Hominis. The patient completely recovered from endocarditis after antibiotic treatment and had another pregnancy ending with normal birth without any complication. Stphylococcus Hominis is a coagulase negative microorganism that normaly exists in skin flora, causing endocarditis more commonly in immunesup-pressed subjects, patients with prosthetic valves and acessory devices. Here in this case report, we present a 31 year-old woman who had the complication of native tricuspid valve endocarditis after spontaneous abortus in her fifth week of pregnancy. To our knowledge, the present case is the first to be reported as a native valve endocarditis caused by Stphylococcus Hominis associated with pregnancy or abortus.Öğe Twin Circumflex Arteries with Left Sinus of Valsalva Origin: A Case Report(Thieme Medical Publ Inc, 2011) Tekbas, Ebru Onturk; Cakici, Musa; Yuce, Murat; Alici, Hayri; Davutoglu, VedatCoronary artery anomalies are not uncommon. The importance of coronary anomalies varies from unimportant to life threatening. Herein, we report for the first time twin circumflex coronary arteries originating separately from the left sinus of Valsalva.
