Öğe A Case of Lymphocytic Interstitial Pneumonitis Associated With Sjgren's Syndrome With Atypical Radiological Findings(Bilimsel Tip Publishing House, 2008) Yildiz, Tekin; Akyildiz, Levent; Topcu, Fusun; Yilmaz, FahriThe 42 year old female patient admitted to our hospital with the compliants of cough, sputum, loss of weight, hoarseness and dryness of the eyes and skin. On the chest x-ray, opacification was found in the right lower zone. The chest CT scan demonstrated nodular lesions, including air bronchograms showing a partial alveolar pattern in the middle and lower right lobes and bilateral pleural plaques. The diagnosis of Sjogren's Syndrome was made on the basis of bilateral Schirmer test positivity in the ophthalmologic examination and anti-SSA: (+++) positivity, with a titration of 1/100. The cytologic examination of bronchoalveolar lavage fluid and histopathological evaluation of transbronchial biopsy were not diagnostic. Lymphocytic interstitial pneumonia was diagnosed by evaluation of a transthoracic fine needle biopsy (tru cut). In this case report, we present a case of lymphocytic interstitial pneumonia associated with Sjogren's syndrome with atypical radiological findings, who responded favourably to steroid and cyclophosphamide treatment.Öğe Differential Expression of CD34, S100, and c-Kit in Interstitial Cells of Cajal in Infantile Hypertrophic Pyloric Stenosis - Immunochemical Study(Wroclaw Medical Univ, 2009) Ozturk, Hulya; Ozturk, Hayrettin; Yilmaz, Fahri; Okur, Hanifi; Otcu, Selcuk; Dokucu, Ali IhsanBackground. The pathogenesis of infantile hypertrophic pyloric stenosis (IHPS) is poorly understood although many hypotheses have been proposed. Objectives. Assessment whether the differential expression of c-Kit, CD34, and S100 may be involved in the development of IHPS. Material and Methods. Specimens from 14 infants with IHPS and seven control subjects were immunohistochemically stained for c-Kit, CD34, and S100. The numbers of CD34(+), S100(+), and c-Kit(+) cells in five random fields per specimen were compared via light microscopy (x200). Results. In normal pyloric tissue, specific and intense c-Kit immunoreactivity was observed in the muscle layers and moderate staining was observed around the myenteric plexus. In IHPS patients, c-Kit+ cells were either absent or markedly reduced around the myenteric plexus. In control and IHPS patients, CD34(+) cells were not observed around the myenteric plexus. In the vascular endothelium, moderate CD34 staining was observed in specimens from control subjects, whereas intense staining was observed for IHPS patients. In normal pyloric tissue, moderate S100 immunoreactivity was observed in the muscle layers and intense staining was observed in the myenteric plexus. In IHPS patients, few S100(+) cells were observed in the pyloric muscle layers and S100 immunoreactivity decreased markedly around the myenteric plexus. Conclusions. These results suggest that the numbers of c-Kit(+) and S100(+) cells are markedly decreased in the pyloric muscle layers and around the myenteric plexus in IHPS patients. Thus a lack of c-Kit and S100, but not CD34, expression may be a critical factor in the pathogenesis of IHPS and may serve as a useful prognostic tool in the treatment of this disease (Adv Clin Exp Med 2009, 18, 1, 33-39).Öğe Effect of ELF magnetic fields on lipid peroxidation, sperm count, p53, and trace elements(Int Scientific Literature, Inc, 2006) Akdag, M. Zulkuf; Dasdag, Suleyman; Aksen, Feyzan; Isik, Birgul; Yilmaz, FahriBackground: Some epidemiological and laboratory studies suggest a possible connection between extremely low-frequency (ELF) magnetic fields and certain illnesses, such as cancer, immune suppression, as well as reproductive toxic effects and abnormalities. Therefore, the aim of this study was to investigate the effects of ELF magnetic fields (1.35 mT) on sperm count, malondialdelivde concentration, the histology of such organs as the testes, brain, liver, and kidney tissues, p53 immunoreactivity of bone marrow, and the serum concentrations Of Cu2+, Zn2+, Mn2+, and Fe3+ in rats. Material/Methods: Sixteen Sprague-Dawley male rats were divided into two groups. The rats in the experimental group were exposed to an ELF magnetic field 2 hr/day for 2 months (7 days a week). The rats in the control group were not exposed to the ELF magnetic field. The exposure was performed in a Faraday cake (130x65x80 cm) with grounded shielding against the electric component. The Mann-Whitney U-test was used for the statistical analysis of the data. Results: Magnetic field measurements showed that, under the experimental conditions, the magnetic field-exposure system produced a stable. flux density of 1.35 +/- 0.018 mT and a stable frequency of 50 Hz, with negligible harmonics and no transients. However, no statistically significant alteration was observed in the parameters measured in this study except in Mn2+ concentrations (p < 0.001). Conclusions: The present study found no evidence of an adverse effect of ELF magnetic fields on the measured parameters except for significantly increased Mn2+ concentrations (p < 0.001).Öğe The effects of molsidomine on hypoxia inducible factor alpha and Sonic hedgehog in testicular ischemia/reperfusion injury in rats(Springer, 2009) Dokucu, Ali Ihsan; Ozturk, Hulya; Ozturk, Hayrettin; Tuncer, Mehmet Cudi; Yilmaz, FahriThis study was designed to determine the effect of molsidomine (MO), a precursor of nitric oxide (NO) donor, on hypoxia inducible factor alpha (HIF-1 alpha) and Sonic hedgehog (Shh) levels considered to be involved in the development of testes ischemia/reperfusion (I-R) injury. Torsions were created by rotating ipsilateral testes 720A degrees in a clockwise direction for 6 h and 1-h detorsion of the testis was performed. A sham operation was performed in group 1 (control, n = 7). In group 2 (I-R/Untreated, n = 7), following 6 h of unilateral testicular torsion, 1-h detorsion of the testis was performed. No drug was given. In group 3 (I-R/MO), after performing the same surgical procedure as in group 2, a NO donor MO was given at the starting time of reperfusion. In group 4 (I-R/L-NAME), after performing the same surgical procedure as in group 2, L-NAME was given at the starting time of reperfusion. Testes malondialdehyde (MDA) levels were determined as well as examining the testes histologically. Treatment of rats with MO produced a significant reduction in the levels of MDA and histopathological score compared to testes I-R groups. The Sonic hedgehog (Shh) expression in the basement membrane of the tubuli seminiferi, and sertoli and germinal cells in testicular tissue, were greatly increased in the I-R/MO group compared to groups 1, 2 and 4. Additionally, the HIF-1 alpha expression in the interstitial spaces in testicular tissue were greatly increased in the I-R/MO group. The results suggest that MO has a protective effect against ischemia/reperfusion injury in rat testes and may affect Shh and HIF-1 alpha signaling pathway.Öğe Grading of the ovarian serous cystadencarcinomas, and its correlation with BRCA1 and p53 immunoreactivity(Ortadogu Ad Pres & Publ Co, 2007) Guenes, Nedim; Yilmaz, Fahri; Uzunlar, Ali KemalObjective: Ovarian cancer is a gynecological malignancy with the higest mortality. The surface epithelial tumors of the ovary are the most common cancer types. The purpose of this study was to grade the ovarian serous cystadenocarcinomas and to evaluate BRCA1 and p53 proteins immunohistochemically. Material and Methods: Fifty-eight cases diagnosed as serous cystadenocarcinomas of the ovary were selected for this study. The cases were graded according to two-tier grading system, FIGO and Shimizu-Silverberg grading system and the results were compared. The BRCA1 and p53 proteins were studied immunohistochemically. Results: The cases were distributed as follows: according to the two-tier grading system low grade 12 (21%) cases and high grade 46 (79%) cases; according to the FIGO grading system grade 19 (16%) cases, grade B 29 (50%) cases, and grade 11120 (34%) cases and according to the Shimizu-Silverberg grading system grade 111 (19%) cases, grade II 29 (50%) cases, and grade 11118 (31%) cases. Positive p53 staining was detected in 57% (33/58) and BRCA1 was detected in 67% (39/58) of the cases. The relationship between p53 and BRCA1 proteins and histopathologic grade was analyzed. High grade ovarian serous carcinomas had a higher rate of p53 expression (65%) compared to low grade (25%) tumors. BRCA1 immunstaining was detected in 92% (11/12) of low grade and 61% (28/46) of high grade tumors according to two-tier grading system. Conclusion: There is a good correlation between the two-tier grading system and the FIGO and the Shimizu-Silverberg grading systems. In addition, the application of the two-tier grading system for ovarian serous carcinomas is easy because this system is based on easily understood two criteria and includes two categories (low grade, high grade). In contrast to p53, high grade tumors had a lower rate of BRCA1 expression (61%) compared to low grade (92%) tumors. In high-grade carcinomas, no statistical correlation was present between the p53 and BRCA1 positivity (p= 0.666).Öğe Intussusception due to inflammatory fibroid polyp located at ileum: Report of two cases(Tubitak Scientific & Technological Research Council Turkey, 2007) Korkmaz, Ozguer; Yilmaz, Hatice Guelsen; Yilmaz, FahriInflammatory fibroid polyps are uncommon gastrointestinal tract lesions. The stomach and ileum are the most commonly affected sites. Clinical symptoms depend on the location and size of the lesion. Obstruction or intussusception is the most frequent initial symptom when an inflammatory fibroid polyp is located in the small intestine. We describe two cases of inflammatory fibroid polyps of the small intestine causing ileocecal and ileoileal intussusception.Öğe Unusual presentation of cervical spinal intramedullary arachnoid cyst in childhood: Case report and review of the literature(Karger, 2007) Guzel, Aslan; Tatli, Mehmet; Yilmaz, Fahri; Bavbek, MuradThe authors report a 7-year-old girl who presented with progressive quadriparesis which had started a month before admission. Magnetic resonance imaging of the spine revealed an intramedullary cystic lesion extending from C-2 to C-4. After performing a C2-5 laminectomy, the cyst was drained and anatomically fenestrated with the subarachnoid space by a 1-cm vertical median myelotomy without using a stent. The histopathological findings revealed arachnoid cyst. In the first month after operation, the neurological deficits disappeared gradually, except for a slight left upper paresis. Cervical spinal intramedullary arachnoid cyst is extremely rare and should be considered in the differential diagnosis of intramedullary cystic lesions in childhood. Recovery is possible after subtotal excision and fenestration of the cyst to allow communication with the subarachnoid space even if neurological deficits are present for a long time. Zulkuf; Kilinc, NihalThe purpose of the present study was to investigate the anti-apoptotic bcl-2 protein in rat brain and testes after whole-body exposure to radiation emitted from 900 MHz cellular phones. Two groups (sham and experimental) of Sprague-Dawley rats of eight rats each were used in the study. Exposure began approximately 10 min after transferring into the exposure cages, a period of time when rats settled down to a prone position and selected a fixed location inside the cage spontaneously. For the experimental group, the phones were in the speech condition for 20 min per day for I month. The same procedure was applied to the sham group rats, but the phones were turned off Immunohistochemical staining of bcl-2 was performed according to the standardized avidin-biotin complex method. The results of this study showed that 20 mn of the radiation emitted from 900 MHz cellular phones did not alter antiapoptotic bcl-2 protein in the brain and testes of rats. We speculate that bcl-2 may affects of radiation on the brain and testes of rats. not be involved in the effects of radiation on the brain and testes of rats.
