Öğe Clinical and radiologic results of open reduction and fixation with locked plate screws in proximal humerus fracture-dislocation(Derman Medical Publ, 2019) Degnek, Orhan; Atic, Ramazan; Alemdar, Celil; Aydin, Abdulkadir; Yildirim, Azad; Ozkul, EminAim: Proximal fracture dislocations of the humerus are rarely seen in society compared to other fractures. In our study, we evaluated the clinical and radiological results of patients who underwent open reduction and locked plate-screw fixation with proximal humerus fracture-dislocation. Material and Method: Between January 2009 and January 2016, 17 patients were treated with open reduction and locking plate screws in the Department of Orthopedics and Traumatology at the Faculty of Medicine, Dicle University. Patients were divided into two groups according to age. Group 1 consisted of 6 patients over 65 years of age and the mean age was 77.5 (69-87). Group 2 consisted of 11 patients under 65 years of age and the mean age was 41.6 (24-60). Group 1 consisted of all female patients and Group 2 consisted of all male patients. Patient fractures were classified according to the Neer classification. Oxford Shoulder Scale, DASH Score, and Constant Murley Score were used in the clinical evaluation of the patients. Avascular necrosis phase was performed according to Cruess phase. Results: The mean follow-up period of the patients was 13.8 months (range 10-38). The mean duration of surgery was 1.11 days (range 0-4). According to the Neer classification, 11.8% of the cases were two-part fracture dislocation, 64.7% were three-part fracture dislocation, and 23.5% were four-part fracture dislocation. There was a statistically significant difference between Oxford and DASH scores in the clinical outcome according to age groups (p = 0.001, p=0.049). Avascular necrosis was observed in 14 of 17 (82.3%) patients. Additional complications such as wound infection, nonunion were not observed. Discussion: In proximal humerus fracture-dislocations, the first surgical choice should be open reduction and internal fixation in young patients, whereas internal fixation in addition to arthroplasty should be considered in elderly patients.Öğe Evaluation of postoperative early mobilization in patients with repaired flexor tendons of the wrist, the spaghetti wrist(Ios Press, 2010) Yildirim, Azad; Nas, KemalPurpose: The hands and wrists are very important for performing the activities of daily life independently. The spaghetti wrist may involve major nerves and arteries, as well as the wrist and finger flexors. The purpose of this study was to evaluate the rehabilitation results of postoperative early mobilization in patients with repaired flexor tendons of the wrist. Methods: Thirty-three patients with repaired flexor tendon injuries were included. Patients were divided into two groups due to ethylogy. Group 1 included 23 patients who incurred tendon injuries during fights with their family members or friends or due to broken glass after fighting. Two patients had cut their hands with a razor. The other patient had cut his hands during discussion. The Group 2 included 10 patients. The injuries in this group were due to work and home accidents. Results: The functional result was excellent in 46% of fingers, good in 22%, fair in 17%, and poor in 15% in the Group 1 patients. The results were excellent in 55% of fingers, good in 17%, fair in 18%, and poor in 10% in the Group 2 patients. Conclusion: Early primary repair and effective rehabilitation are of great importance during the postoperative period for successful results in the treatment of extensive volar wrist lacerations.Öğe Four decades without diagnosis: Sheehan's syndrome, a retrospective analysis(Taylor & Francis Ltd, 2016) Gokalp, Deniz; Alpagat, Gulistan; Tuzcu, Alpaslan; Bahceci, Mithat; Tuzcu, Sadiye; Yakut, Fatime; Yildirim, AzadAim: Sheehan's syndrome (SS) remains a frequent cause of hypopituitarism in undeveloped and developing countries, but due to improvements in obstetric care, it is rare in developed countries. We aimed to share the results of a retrospective study analyzing the demographic, clinical, imaging, and hormonal characteristics of a large group of patients with SS, and also increase awareness of this syndrome especially in developed countries. Methods: The medical records of 124 patients with SS patients who were followed up in the Endocrinology Department of Dicle University between 1995 and 2015 were assessed retrospectively. Results: The mean period of diagnostic delay was 20.37 +/- 8.34 years on average. 5.7% of patients with SS were literate; 62% of patients delivered at home. Anemia was identified in 64.5% of SS patients. Mean blood sodium levels were 129.8 +/- 11.3 mEq/L. The mean urine densities were 1013 +/- 6.5. Osteoporosis and osteopenia were found in 44 (35.4%) and 71 (57.2%) patients, respectively, According to pituitary magnetic resonance imaging (MRI) analyses, 92 (74.2%) patients with SS had completely empty sella, 29 (23.3%) had partially empty sella, and 1 patient had microadenoma, and 2 had normal pituitary MRI results. Conclusions: Improved obstetric care and effective interventions for postpartum hemorrhage have limited the prevalence of SS in developed countries. However, in developing countries like Turkey, SS due to postpartum bleeding remains common. Thus, physician's awareness of the symptoms of SS is urgently required to avoid the associated morbidity and mortality.Öğe Neglected monteggia fracture-dislocations in children(Derman Medical Publ, 2018) Atic, Ramazan; Yildirim, Azad; Yigit, Sehmus; Aydin, AbdulkadirAim: Presently, there are few large-scale studies and no clear consensus on the type of osteotomy or ligamentoplasty to be performed in neglected Monteggia fractures-dislocations cases. The goal of this study was to report the results from neglected Monteggia fractures and dislocations. Material and Method: Thirteen children and adolescents with neglected Monteggia fractures and dislocations were treated between 2009 and 2012. There were 11 males and two females with a mean age of 8.5 (range 2-15) years. Based on the BADO classification, 11 patients had type 1, one patient had type 3, and one patient had type 4 injuries. Time delay from symptom onset to fracture was 8.84 (3-24) months. The data were analyzed using R version 3.1.1 and multiple statistical analyses were conducted. Results: A postoperative Mayo Elbow Performance Index (MEPI) at the time of follow-up was recorded. Nine patients received an excellent grade, while three had good and one had a fair result with no poor results recorded. Radiographically, there were nine good, four fair, and no poor score results. The length of hospitalization was a mean of 3 days. The mean duration of follow-up was 2.9 (range 1.5-4.5) years from the initial presentation, and only one patient developed subluxation for which no additional intervention was performed. Discussion: This case series demonstrates that early diagnosis increases the success rate in patients younger than 10 years and less than a 1-year interval between the trauma and diagnosis. Unless it is kept for a prolonged time, a condyloradial pin can be applied in necessary cases.
