The presence of these autoantibodies can be shown by an autologous serum test consisting of the papule and erythema reaction arising from the injection of the serum into the dermis. It has been suggested in recent years that autologous plasma skin test would be more appropriate than the autologous serum skin test in chronic urticaria. The present study evaluated the test results of autologous serum skin test and autologous plasma skin test in chronic urticaria patients. Materials and Methods: The study registered 34 chronic urticaria patients who presented at dermatology outpatient clinic from July 2009 to November 2010, and a control group consisting of 30 healthy volunteers. Chronic urticaria patients and control group were tested by using autologous serum and autologous plasma skin tests. Urticaria activity scores of chronic urticaria patients were calculated. be positive in 67.6% of chronic urticaria patients and 26.7% of control group. Autologous plasma skin test, on the other hand, was established to be positive in 79.4% of chronic urticaria patients and 30% of control group. In case of chronic patients whose disease duration ranged between 1-5 years, both tests produced positive results at higher rates. Urticaria activity scores of the chronic urticaria patients whose autologous serum and autologous plasma skin tests were positive were higher than those of the chronic urticaria patients with negative test results. Conclusion: The fact that autologous plasma skin test was found higher in comparison to autologous serum test in chronic urticaria patients may bring new treatment alternatives to the fore in the light of the new developments in the disease pathogenesis.Öğe A case of orf (ecthyma contagiosum) with multiple lesions(Pakistan Medical Assoc, 2013) Turan, Enver; Yesilova, Yavuz; Ucmak, DeryaOrf, also known as ecthyma contagiosum or contagious pustular dermatitis, is a viral zoonotic disease resulting from the direct or indirect contact of damaged skin with infected animals. The causative microorganism is the orf virus, an epitheliotropic DNA virus from the Parapoxvirus group, which generally infects sheep, goats, and various other domestic and wild ovine animals. A male patient presented to our outpatient clinic with multiple nodular lesions on his right wrist after incurring an injury during ovine slaughter. Diagnoses other than ecthyma contagiosum were eliminated by punch biopsy. Orf generally manifests as solitary lesions on hands and fingers, but it may rarely present as multiple nodular lesions. Here we present the case of a 42-year-old man diagnosed with orf based on clinical and histopathological findings.Öğe A comparative evaluation of the effects of bevacizumab and 5-fluorouracil on wound healing in rat model(Drunpp-Sarajevo, 2012) Gunay, Ahmet; Atilgan, Serhat; Yesilova, Yavuz; Yaman, Ferhan; Ozgoz, Mehmet; Lacin, NihatPurpose: Our aim is to evaluate the use of combined bevacizumab with 5-Fluorouracil (5-FU) on post-operative scarring survival following experimental skin incision surgery in comparison to the agents alone. Material and methods: Skin incision surgery was performed on 28 female Spraque-Dawley rats. The rats were allocated to one of four treatments: 5-FU combined with Bevacizumab, 5FU alone, bevacizumab alone and phosphate buffered saline (PBS). A single subcutaneous injection was administered immediate postoperatively. Histological staining determined the presence of cutaneous fibrosis and mRNA expression of collagen I and fibronectin in the tissue was quantified. Results: Relative quantity of Colla1 mRNA transcript 5 fold increased in Group 1 (PBS) in comparison to control group. However, Colla1 level in Group 2 (5-FU) and 3 (bevacizumab) is decreased. The lowest level of Colla1 was detected in Group 4 (combination of 5-FU and bevacizumab). On the other hand, relative quantity of Fn1 is slightly elevated in Group 1 (PBS) and decreased in Group 2 (5-FU) and group 3 (bevacizumab) in comparison to control group. The lowest level of Fn 1 was detected in Group 4 (combination of 5-FU and bevacizumab). Conclusions: Bevacizumab in combination with 5-FU resulted in a greater anti-fibrotic effect compared to monotherapy with 5FU or bevacizumab alone, as evidenced by the attenuation in fibronectin and mature collagen I expression and deposition. (P<0.05) The results provide compelling evidence that combined bevacizumab and 5-FU offers superior anti-fibrotic effect over monotherapy. A synergistic effect is suggested to be present.Öğe Determination of dermatology life quality index, and serum C-reactive protein and plasma interleukin-6 levels in patients with chronic urticaria(Termedia Publishing House Ltd, 2013) Ucmak, Derya; Akkurt, Meltem; Toprak, Gulten; Yesilova, Yavuz; Turan, Enver; Yildiz, IsmailIntroduction: C-reactive protein (CRP) and interleukin-6 (IL-6), which is one of its most important simulators, were determined in great amounts in the sera of patients with chronic urticaria (CU). Aim: To determine the levels of IL-6 in patients with urticaria, and evaluate its relationship with urticaria activity scores and Dermatology Life Quality Index (DLQI). Material and methods: Fifty-three patients with CU were included in the study successively by determining their urticaria activity scores (0-3) and DLQI (0-5). The CRP and IL-6 were measured by immune assay methods. Thirty-two healthy subjects were included as a control group. Results: Serum levels of IL-6 and CRP were significantly higher in patients with CU compared to healthy controls (p < 0.001, p = 0.026 respectively). There was a statistically significant correlation among urticaria activity scores and IL-6 and CRP concentration (p = 0.004, p = 0.042). This correlation was more significant in patients who had moderate and severe disease activity scores than in those who had mild disease activity score (p < 0.001, p < 0.001, respectively). There was a statistically significant association between DLQI and IL-6 (p = 0.025). This correlation was very significant in patients who had severe and very severe disease activity scores (p < 0.001, p < 0.001, respectively). DLQI scores and serum levels of IL-6 were significantly different in the very severe group compared to healthy controls (p = 0.024). Conclusions: The levels of CRP and IL-6 are increased in patients with CU. A relationship of DLQI and urticaria activity scores with CRP and IL-6 was found. These findings support the relationship between the inflammatory process in CU and the clinical findings.Öğe Early-onset basal cell carcinoma(Turkish J Pediatrics, 2013) Turan, Enver; Yurt, Nurdan; Yesilova, Yavuz; Turkcu, GulBasal cell carcinoma (BCC) is a common skin cancer that is responsible for approximately 75% of non-melanoma skin cancers. BCC occurs on sun-exposed areas in fair-skinned individuals; frequency increases with age and peaks in the seventh decade of life. Although it usually occurs in the elderly population, it has been reported in children as well. Here, we present a case of BCC in an eight-year-old child without any predisposing characteristics.Öğe Effects of isotretinoin on obsessive compulsive symptoms, depression, and anxiety in patients with acne vulgaris(Taylor & Francis Ltd, 2012) Yesilova, Yavuz; Bez, Yasin; Ari, Mustafa; Kaya, Mehmet Cemal; Alpak, GokayBackground: Depression, anxiety, and obsessive compulsive symptoms are known to be common among patients with acne vulgaris. In contrast to earlier reports, a number of recent studies suggested isotretinoin to be beneficial for depression and anxiety symptoms in acne patients. Moreover, its effects on obsessive compulsive symptomatology are still widely unknown. Objectives: To examine the effects of isotretinoin treatment on obsessive compulsive symptoms, depression, and anxiety in acne vulgaris patients. Methods: The Hospital Anxiety and Depression Scale, Maudsley Obsessive Compulsive Questionnaire, and Sheehan Disability Scale were administered to 43 patients with acne vulgaris who accepted isotretinoin treatment and agreed to participate in the study. Severity of acne was evaluated with Global Acne Grading System. Thirty-three patients completed 6 months treatment with oral isotretinoin at 0.5-1.0 mg/kg daily doses. Measurements were repeated at the end of 6 months. Results: Depression, anxiety, and obsessive rumination symptoms were significantly improved after isotretinoin treatment. On the other hand, obsessive doubting of acne patients was worse at the end of the treatment. There was no significant change in disease related disability of patients. Conclusion: Besides worsening of obsessive doubting, successful treatment with oral isotretinoin seems to be related with improvement in depression, anxiety, and obsessive rumination symptoms in acne vulgaris patients. These changes were not reflected in the disability level that acne patients have reported.Öğe High social phobia frequency and related disability in patients with acne vulgaris(John Libbey Eurotext Ltd, 2011) Bez, Yasin; Yesilova, Yavuz; Kaya, Mehmet Cemal; Sir, AytekinAcne is an easily recognizable abnormality which may cause some adverse psychosocial consequences. We aimed to determine the social phobia frequency, social anxiety level, and disease related disability in a group of acne vulgaris patients. One-hundred and forty acne vulgaris patients and 98 healthy control subjects were included in the study. Acne severity was determined by the Global Acne Grading System (GAGS). A psychiatrist interviewed each participant and the Liebowitz Social Anxiety Scale (LSAS) was administered to all participants, who also completed the Hospital Anxiety and Depression Scale (HADS) and Sheehan Disability Scale (SDS). Social phobia was diagnosed in 45.7% of acne vulgaris patients and in 18.4% of control subjects. Acne vulgaris patients demonstrated higher performance avoidance and total avoidance scores in LSAS than controls. Acne vulgaris patients without social phobia had higher scores in HADS and LSAS than the acne vulgaris patients with social phobia. They were more disabled in their occupational, social, and familial lives. Social phobia diagnosis predicted disability at work, whereas education level predicted the disability in family life of acne vulgaris patients. The awareness of paraneoplastic dermatoses among clinicians and pathologists is of great importance in early diagnosis and management of internal malignancy, in monitoring for tumor recurrence, and for clarifying the pathophysiology. The signs and symptoms of dermatological lesions are easily detectable, therefore, they may be the reason for a cancer patient to refer to a physician. Cutaneous paraneoplastic syndromes usually have a course parallel to the underlying malignancy. In this review, the most common cutaneous paraneoplastic syndromes are overviewed.Öğe Predictive Value of Obsessive Compulsive Symptoms Involving the Skin on Quality of Life in Patients with Acne Vulgaris(Acta Dermato-Venereologica, 2013) Bez, Yasin; Yesilova, Yavuz; Ari, Mustafa; Kaya, Mehmet Cemal; Alpak, Gokay; Bulut, MahmutAcne is one of the most common dermatological diseases, and obsessive compulsive disorder is among the most frequent psychiatric conditions seen in dermatology clinics. Comorbidity of these conditions may therefore be expected. The aim of this study was to measure obsessive compulsive symptoms and quality of life in patients with acne vulgaris, compare them with those of healthy control subjects, and determine whether there is any predictive value of obsessive compulsive symptoms for quality of life in patients with acne. Obsessive compulsive symptoms and quality of life measurements of 146 patients with acne vulgaris and 94 healthy control subjects were made using the Maudsley Obsessive Compulsive Questionnaire and Short Form-36 in a cross-sectional design. Patients with acne vulgaris had lower scores for physical functioning, physical role dysfunction, general health perception, vitality, and emotional role dysfunction. They also had higher scores for checking, slowness, and rumination. The only predictor of physical functioning and vitality dimensions of health-related quality of life in these patients was rumination score. Obsessive compulsive symptoms in patients with acne vulgaris are higher than in controls, and this may correlate with both disease severity and quality of life for patients.Öğe Reduced serum paraoxonase-1 levels in vitiligo: further evidence of oxidative stress(Taylor & Francis Ltd, 2012) Yesilova, Yavuz; Turan, Enver; Ucmak, Derya; Selek, Sahabettin; Yavuz, Brahim Halil; Tanrikulu, OsmanVitiligo is a common disorder that results in depigmented areas of the skin. The pathogenesis of the disease remains unclear, but oxidative stress is one suggested cause. Oxidative stress may be induced by increasing the generation of reactive oxygen species and other free radicals. The generation of reactive oxygen species is known to be associated with a decrease in antioxidant levels. This study examined oxidative stress index in active lesions of generalized vitiligo patients. We analysed serum levels of paraoxonase 1, arylesterase, catalase, ceruloplasmin, total antioxidant capacity, and oxidative stress index in patients with active lesions of generalized vitiligo, as well as in matched, healthy controls. Serum oxidants and oxidative stress indexes were higher, and serum antioxidants were lower, in vitiligo patients compared with healthy controls. Our findings suggest that oxidative stress may play an important role in the pathogenesis of vitiligo. Paraoxonase 1 can be used as an indicator in determining oxidative stress existent in the pathogenesis of vitiligo diseases.Öğe Thermal pillow: an unusual causative agent of erythema ab igne(Turkish J Pediatrics, 2013) Turan, Enver; Yesilova, Yavuz; Ucmak, Derya; Celik, Ozgur IlhanErythema ab igne (EAI), also known as toasted skin syndrome or heat-induced circumscribed dermal melanosis, presents with pigmented reticular skin lesions with telangiectasias due to prolonged thermal radiation exposure. This article describes a 16-year-old male patient with EAI due to the use of a thermal pillow, and discusses a potential causative agent of EAI. The potential hazards of misusing new technological equipment and the importance of consumer education are also discussed.
