Öğe 46, XX, t(8;9)(q22;p24) balanced translocation carrier with hypogonadotopic hypogonadism(2010) Akbaş H.; Yalinkaya A.; Alp M.N.; Budak T.Balanced autosomal translocations are unusually observed in the patients with hypogonadotropic hypogonadism. The patient with hypogonadotropic hypogonadism had balanced reciprocal translocation involving chromosomes 8 and 9. The patient's father, two brothers and a sister have the same chromosomal translocation with no apparent physicial abnormalities. In this case report; the relation of autosomal translocations mentioned above and hypogonadotropic hypogonadism are discussed.Öğe Can combined spinal-epidural analgesia be an alternative to epidural analgesia alone in labour?(2003) Ölmez G.; Da? I.H.; Özyilmaz M.A.; Yalinkaya A.Currently, the final point in labour analgesia has been accepted as combined spinal-epidural analgesia (CSE). In this prospective and randomised study, we aimed to investigate whether this approach can be an alternative to epidural analgesia (EA) as a reference method. The study was performed with 50 primigravid pregnant women who planned to have vaginal delivery. The patients were allocated randomly to one of the two groups: In Group I, CSE block was applied in 25 patients and epidural block in Group II with 25 patients. A solution of 0.125% bupivacaine with 2 ?g mL-1 fentanyl was prepared for using in patient controlled epidural analgesia (PCEA). Epidural catheters were introduced following the spinal administration of bupivacain (2.5 mg) plus fentanyl (25 ?g) in patients in Group1. PCEA was started after the VAS scores exceeded the level of 3. In Group 11, PCEA was started shortly after introducing of epidural catheters. Only with the first subarachnoidal dosage in GI sufficient analgesia was obtained in all patient. Duration of spinal analgesia was 89.80±9.94 min. At the fifth minute, VAS pain scores were significantly lower in CSE group than in EA group (p<0.0001). The amount of total bupivacaine consumption was 47.96±11.42 mg in GI and was 69.72±11.54 mg in GII (p<0.0001). These amounts for fentanyl consumption was 97.21±18.76 ?g in GI and 111.52±18.50 ?g in GII (p<0.05). There were no significant differences in the other variables. We conclude that CSEA technique can be an alternative approach in conditions of required fast analgesia and short term labour action in delayed deliveries and in multipartaous pregnants.Öğe A comparative study of a gonadotropin-releasing hormone agonist and finasteride on idiopathic hirsutism(2000) Bayhan G.; Bahçeci M.; Demirkol T.; Ertem M.; Yalinkaya A.; Erden A.C.Objective: To compare the efficacy of finasteride and GnRH agonist in the treatment of idiopathic hirsutism. Methods: Sixty women with hirsutism were randomly assigned to receive either 5 mg of finasteride or long-acting GnRH agonist (depot leuprolide 3.75 mg) intramuscularly monthly for six months. Main outcome measures: Hirsutism scores were measured according to the Ferriman-Gallway scoring system, and side-effects were monitored for six months of treatment. Blood samples were taken at each visit for assessment of endocrine (FSH, LH, estradiol, progesterone, total and free testosterone, androstenedione, DHEAS-S, 17-OH-P, SHBG), biochemical, and hematologic parameters. Results: All of the patients treated with finasteride or GnRH agonist showed neither menstrual abnormalities nor side-effects. The mean percent change (±SD) in hirsutism scores in the GnRH and finasteride groups was 36%±14% and 14%±11% at six months, respectively. Serum total testosterone, free testosterone, androstenedion and DHEA-S showed a meaningful decrease in patients treated with GnRH agonist. On the other hand, only serum total testosterone and free testosterone levels decreased with finasteride treatment (p<0.05 and p<0.0001, respectively).Öğe A middle aged woman with mosaic turner syndrome: A case report(2011) Simsek S.; Yalinkaya A.; Oral D.; Turkyilmaz A.; Tekes S.; Budak T.Turner syndrome is a disorder of females is characterized by the absence of all or part of a normal second sex chromosome. Turner syndrome occurs in 1 in 4000 live-born girls and approximately 5 to 10 percent of them have mosaic isochromosome 45,X/46,X,i(Xq). Turner syndrome is associated with reduced adult height and with gonadal dysgenesis, leading to insufficient circulating levels of female sex steroids and to infertility. Osteoporosis and high risk of fractures are features in adults with Turner syndrome. In this study, we present a delayed case of Turner syndrome with primary amenorrhea, short stature, osteoporosis and high risk of fractures. This case has ignored due to social and economic conditions, therefore we think that the patient can be considered for publication.Öğe Misoprostol-induced termination of secondtrimester pregnancy in women with a history of cesarean section: A retrospective analysis of 56 cases(Via Medica, 2013) Turgut A.; Özler A.; Görük N.Y.; Karaçor T.; Yalinkaya A.Objective: To assess the effectivity and safety of misoprostol induced termination of pregnancy in the second trimester in women with a history of previous caesarean section. Materials and Methods: Retrospective analysis of case records from the obstetrics and gynecology department of a tertiary care center between January 2009 and February 2012 was performed. Data derived from 219 women,who underwent a second trimester termination of pregnancy, was analyzed in terms of demographics, clinical findings, laboratory and procedural data. The study group consisted of 56 women with a previous caesarean section and the control group was composed of 163 women without such a history. Termination of pregnancies was conducted by administration of misoprostol at doses of 50-600 mcg intravaginally or by surgical evacuation in cases of failure of medical measures. Results: There was no statistically significant difference between two groups in terms of demographics such as age, menarche, number of pregnancies or live births, smoking habit and co-morbidities. Necessity for blood transfusion (p=0.05) and additional procedure for abortion (p=0.056) were found to be similar in both groups. However, laparotomy (p=0.004), uterine rupture (p=0.016), hysterotomy (p<0.001) were performed more frequently in the study group; while abortion was more likely to occur within 24 hours in the control group (p=0.031).Conclusion: Medical abortion must be carefully used for the termination of second trimester pregnancies in women with a history of CS. Increased possibility of uterine rupture and requirement of interventions such as laparotomy or hysterotomy is more likely in these patients.Öğe Mucinous tumors of the ovary: Analysis of 38 cases(2001) Yaldiz M.; Uzunlar A.K.; Kilinç N.; Yalinkaya A.; Akkuş M.; Tunik S.Ovarian mucinous tumors stem from ovarian surface epithelium and are divided into benign, borderline and malignant. It is difficult to differentiate borderline and malignant mucinous tumors. Thirty-eight cases of ovarian mucinous tumors which were diagnosed at the Pathology Department of Dicle University Medical Faculty were reviewed. Of these, 18 (47.3%), six (15.7%) and 14 (36.8%) were benign, borderline and malignant, respectively. The patients' ages ranged from 18 to 67 (average 44.5) years. Bilaterality was detected in 1/18 (5.5%), 0/6 and 4/14 (28.5%) of benign, borderline and malignant mucinous tumors, respectively. Mean tumor size was 26.4 cm. Microscopically, there was no stratification in the benign tumors. The borderline tumors had papillary infoldings and 2-3 layers of atypical epithelial cells but no invasion of the stroma. A 64 year old, postmenopausal woman applied to our clinic with severe acute abdominal pain. On abdominal examination, there were abdominal tenderness, defense and rebound. On ultrasonographic examination, we detected a 6 cm of pelvic mass. Because she had acute abdomen we performed laparotomy by midline incision and excised a 6 cm ovarian mass on right ovary. The mass had been reported as ovarian leiomyoma on frozen section by pathology department. © 2014 Old City Publishing, Inc.Öğe Rectal perforation following anal intercourse in a sixteen-year-old young married woman(2006) Kale A.; Kuyumcuoglu U.; Erdemoglu M.; Akdeniz N.; Yalinkaya A.Background: Many men and women practice a broad range of voluntary sexual activities. Most of which are harmless. Many minor injuries of the genital, and anal areas do occur but most require only symptomatic therapy. Rectal perforations and sphincter injuries are uncommon but have important medicolegal implications. Case Report: A-16-year-old nullipara presented with vaginal bleeding after sexual intercourse. Pelvic examination revealed full-thickness longutidunal perforation of the rectum. Full-thickness perforation of the rectum was sutured primarily under general anesthesia. The patient was discharged 2 days later. Conclusions: Rectal perforation due to sexual intercourse in a 16-year-old married woman has not been previously reported.Öğe Slowly growing large size acrochordon of vulva in an adolescent girl(2006) Kale A.; Akdeniz N.; Yalinkaya A.; Erdemoglu M.; Kilinc N.Background: Vulvar achrochordon is a relatively uncommon benign polypoid tumor of the vulva commonly known as skin tag or occasionally fibroepithelial polyps. The size of the lesion is generally 1-2 cm, but it rarely can reach extremely large size. Case Report: In our case A 20 years old girl had been suffering from slowly growing a vulvar achrochordon tumor for six years. The tumor was arising from the labium majus, and measured 11 cm in its largest diameter. Histologically, papillomatosis and hyperkeratosis of polypoid lesion was reported. Six month following surgical excision, the patient did not manifest any signs of recurrence. Conclusions: This vulvar lesion represents a very unusual example of large size achrochordon in an adolescent girl. We present this case as a rarely seen benign tumor of vulva.
