Öğe Assessment of the underlying causes of the immune thrombocytopenia: Ten years experience(Pakistan Medical Assoc, 2017) Dal, Mehmet Sinan; Karakus, Abdullah; Dal, Tuba; Aydin, Berrin Balik; Hattapoglu, Elif; Ekmen, Mehmet Onder; Ulas, TurgayObjective: Immune thrombocytopenia (ITP) is an immune haematologic disorder causing platelet destruction mediated by anti-platelet antibodies. In this study we aimed to evaluate the clinical and laboratory variables of ITP patients in southeast of Turkey. Methods: In this retrospective study 167 ITP patients between 2005 and 2015 were evaluated. All patients were screened for immunological parameters including ANA (antinuclear antibodies), anti dsDNA (anti-double-strandedDNA), ACA(anti-cardiolipin) IgM and IgG, LA (lupus anticoagulants). All patients were screened for Helicobacter pylori, HBsAg (Hepatitis B surface antigen), anti-HCV (hepatitis C virus antibody), and anti-HIV 1/2 (HIV antibody) and brucellosis. Results: Among the patients, 50 (29.9%) patients were male, 117 (70.1%) were female. The age range of patients was 18-86 (mean 38.16+/-14). In 56 patients (33.5%) splenectomy was performed. 36 patients (21.6%) were positive for ANA, 5 (3%) were positive for anti dsDNA, 14 (8.4%) for ACA Ig G, and 14 (8.4%) patients for ACA IgM. LA was tested in 165 patients and 30 (18%) patients were positive for LA. Microbiologic evaluation was as follows: 16 patients (9.6%) were positive for HbsAg, 109 (65.3%) positive for Anti-HBs, 5 positive for anti-HCV (3%), 56 (33.5%) patients were positive for Helicobacter pylori antigen, 5 (2.9%) for Brucella and one patient was positive for anti-HIV 1/2. Conclusion: Immune thrombocytopenia patients have to be evaluated according to their demographic characteristics and laboratory results. Secondary causes of ITP were HIV, HCV, Helicobacter pylori, brucellosis, tuberculosis, and autoimmune diseases in our region. As the pathophysiology of the disease got clear, thrombopoietin receptor (TPO-R) agonists have been preferred in recent years, which seems to be an effective option in the treatment of resistant cases. Eltrombopag, a TPO-R agonist, is a second or third line medical treatment option for adults with chronic ITP. Objective: To determine the efficacy and safety of long-term eltrombopag treatment in treatment-refractory patients with primary ITP was the aim of this study. Materials and Methods: Retrospective data of 34 patients with refractory ITP who were treated with eltrombopag were examined. Efficacy and safety of the eltrombopag treatment was evaluated. Results: The total rate of response was 82%, and the median duration of response defined as the number of the platelets being over 50x10(9)/L was 14 (interquartile range: 7-28) days. In two patients, thrombosis was observed with no other additional risk factors due to or related to thrombosis. Conclusion: In spite of the fact that the responses to eltrombopag were satisfactory, patients need to be monitored closely for increasing platelet counts as well as thromboembolic events.Öğe Effects of eradication on proteinuria: a prospective study(Springer Wien, 2012) Aydogan, Timucin; Ulas, Turgay; Selcoki, Yusuf; Alkan, Rabia; Yilmaz, Omer Caglar; Yalcin, Kadir Serkan; Inan, OsmanBackground Helicobacter pylori ( H. pylori) is one of the most common bacterial infections, seen in humans worldwide and its possible relationship to different diseases is a focus of attention nowadays. The aim of this study was to analyse the effects of eradication on proteinuria. Ninety-nine patients suffering from dyspeptic complaints were recruited in this prospective study. The patients were divided into two groups according to the presence of infection. Thus, a total of 67 positive and 32 negative patients were studied. The positive patients' group was divided into two groups according to response to eradication treatment. A total of three groups were formed, viz; group 1 comprises of patients who are positive and responds positively to eradication therapy, group 2 comprises of patients who are positive and responds negatively to eradication therapy and group 3 is the control group and comprises of patients that are negative. Urine samples to obtain the protein/creatinine ratio were collected initially and at the end of the study from all patients. Mean difference levels (pre- and post-treatment difference) of urine protein/creatinine ratio was 0.055 +/- 0.13 in group 1. The ratio was -aEuro parts per thousand 0.0007 +/- 0.0067 in group 2 and -aEuro parts per thousand 0.0022 +/- 0.008 in group 3. A statistically significant difference was found in group 1 compared to the other groups in terms of mean difference levels of protein/creatinine ratios (p < 0.001). As a result of our study, treatment of eradication significantly reduced the proteinuria within the normal limits.Öğe Evaluation of oxidative stress parameters and metabolic activities of nurses working day and night shifts(Univ Sao Paolo, 2013) Ulas, Turgay; Buyukhatipoglu, Hakan; Kirhan, Idris; Dal, Mehmet Sinan; Ulas, Sevilay; Demir, Mehmet Emin; Eren, Mehmet AliThe aim of this study was to evaluate the oxidative stress and metabolic activities of nurses working day and night shifts. Intensive care unit (ICU) (n=70) and ordinary service (OS) nurses (n=70) were enrolled in the study. Just before and the end of the shifts, blood samples were obtained to measure the participants' oxidative stress parameters. Metabolic activities were analyzed using the SenseWear Armband. Oxidative stress parameters were increased at the end of the shifts for all OS and ICU nurses compared to the beginning of the shifts. Compared to the OS nurses, the ICU nurses' TAS, TOS, and OSI levels were not significantly different at the end of the day and night shifts. The metabolic activities of the OS and ICU nurses were found to be similar. As a result, the OS and ICU nurses' oxidative stress parameters and metabolic activities were not different, and all of the nurses experienced similar effects from both the day and night shifts.Öğe Evaluation of sleeping energy expenditure using the SenseWear Armband in patients with overt and subclinical hypothyroidism(Canadian Soc Clinical Investigation, 2012) Ulas, Turgay; Buyukhatipoglu, Hakan; Eren, Mehmet A.; Dal, Mehmet S.; Torun, Aysenur; Aydogan, Timucin; Demir, Mehmet E.Purpose: The aim of the present study was to evaluate the average sleeping energy expenditure (EE) levels using the SenseWear Armband (SWA) in patients with overt and subclinical hypothyroidism. Methods: Sixty patients with hypothyroidism and 30 healthy individuals were recruited for the study. Hypothyroid patients were divided into two groups: group 1 (n = 30) consisted of patients with overt hypothyroidism and group 2 (n = 30) consisted of patients with subclinical hypothyroidism. Lastly, group 3 (n = 30) consisted of healthy subjects. The average EE and metabolic equivalent of task (MET) values during sleep of all the hypothyroid participants were analyzed at baseline and at the end of the study. Data were also obtained from the healthy subjects at baseline. Results: The average sleeping EE and METs values were not significantly different at baseline. Similarly, these values did not change significantly after achieving a euthyroid state via thyroid hormone replacement (both p > 0.05). H. pylori may cause some extra intestinal manifestations some of which are dermatological conditions, including Henoch-Schonlein purpura (HSP), chronic urticaria and atopic dermatitis. We describe a 49-year-old man who presented with HSP triggered by gastric H. pylori infection. Treatment of H. Pylori infection was accompanied by prompt resolution of the gastrointestinal manifestations and purpuric rashes. These findings suggest a causative role for H. pylori in the occurrence of HSP.Öğe Urotensin-II and endothelin-I levels after contrast media administration in patients undergoing percutaneous coronary interventions(Isfahan Univ Med Sciences, 2013) Ulas, Turgay; Buyukhatipoglu, Hakan; Dal, Mehmet S.; Kirhan, Idris; Kaya, Zekeriya; Demir, Mehmet E.; Tursun, IrfanBackground: Contrast induced kidney injury is an acute renal dysfunction that is secondary to the administration of radio contrast media. The purpose of this study was to evaluate the levels of urotensin-II (UT-II) and endothelin-I (ET-I) after contrast media administration in patients undergoing percutaneous coronary interventions. Materials and Methods: In this prospective cohort study, we evaluated 78 patients with coronary artery disease who were scheduled for and ultimately underwent percutaneous coronary interventions. As a contrast material, nonionic contrast media was used in various amounts (70-480 mL). Blood and urine samples were obtained to measure U-II, ET-I just before and at the twenty-fourth hour of percutaneous coronary interventions. Results: Compared to baseline, twenty-fourth hour creatinine levels were significantly increased (P < 0.001). The twenty-fourth hour serum and urine levels of both UT-II and ET-I were also significantly increased compared to baseline (P < 0.001 for all) and 24th hour serum and urine UT-II (r = 0.322, P = 0.004; r = 0.302, P = 0.007 respectively), ET-I (r = 0.511, P < 0.001; r = 0.266, P = 0.019 respectively) levels were significantly correlated with the amount of contrast media.
