Öğe Acute brucellosis associated with leukocytoclastic vasculitis and splenic infarct(EDIMES Edizioni Medico Scientifiche, 2014) Uçmak F.; Uçmak D.; Beştaş R.; Anli R.A.; Adanir H.Brucellosis is globally the most prevalent multisystem infection of zoonotic origin, while it is still one of the most important public health problems in Turkey as non-pasteurised milk and dairy products are consumed. Early diagnosis is vital to prevent the possibly lethal complications caused by the disease. However, diagnosis might be delayed as the disease does not have a single and typical manifestation and presents with various symptoms of different systems. Brucellosis and associated splenic infarct have rarely been studied, there being few cases in the literature. One of the rare involvements in this disease is dermatological involvement, which has been found in less than 10% of brucellosis cases. In this study, we discuss a 17-year-old male patient who was admitted to our hospital due to fever, abdominal pain, arthralgia and rash on legs, diagnosed with brucellosis through brucellosis tube agglutination test and found to have splenic infarct upon examination and leukocytoclastic vasculitis according to the skin biopsies in the light of the present literature. © 2014, Le Infezioni in Medicina. All rights reserved.Öğe Clinical and histopathological features of patients with cicatricial marginal alopecia(Turkiye Klinikleri, 2014) Akkurt Z.M.; Uçmak D.; Do?ruk Kaçar S.; Alabalik U.; Karabulut P.; Türkçü G.; Arica M.Objective: The aim of this study was to investigate the clinical and histopathologic characteristics of patients presenting with band-shaped alopecia of the temporal and frontal areas who had no history of traction. Material and Methods: Patients with cicatricial marginal retrospective study were included in this retrospective study. Severity of disease was classified as mild, moderate and severe according to the density of hair and size of lesions. Classification of the alopecia was made as follows: "bitemporal" in patients with involvement of only the temporal areas, "band-like" in patients with involvement of both temporal areas and the frontal area, and "other" in patients which did not fit the previous classifications. Dermoscopic findings were noted. Previous biopsy samples were restained with Masson Trichrome, CD3, CD1a, CD20 and CD68 and re-evaluated. Results: A total of 20 patients were included in the study. The mean age of the patients was 27,68 (15-41) years and the mean disease duration was 2,33 years. The most frequently reported symptoms were moderate-to-severe pruritus and mild pain. The disease was mild in nine, moderate in seven and severe in four patients (45,0%, 35,0% and 20,0%, respectively). The frontal and temporal hairline was intact and had in all patients. Eight patients had a bitemporal, eight had a band-like pattern of disease, whereas the rest were of other patterns. No signs of inflammation, including erythema, pustules, were observed on the lesions. On dermoscopy, decreased hair density, decreased number of follicles and hair shafts with reduced diameters were seen in all patients. Histopathologic findings of the 14 patients who had consented to biopsy were all similar. Decrease in number of hair follicles, perifollicular fibrosis and retention of sebaceous glands were notable findings. Inflammatory cells were very scarce. Conclusion: Cicatricial marginal alopecia is a type of alopecia seen in females which can seriously affect the cosmetic well-being of patients. An intact hairline and lack of inflammation are characteristic properties of the disease. Copyright © 2014 by Türkiye KlinikleriÖğe Evaluation of clinical and epidemiological features of children with facial viral warts(Dermatologia Pediatrica, 2014) Uçmak D.; Akkurt Z.M.; Dal T.; Acar G.; Arica M.Verrucae are viral lesions commonly encountered in the pediatric population. They occur relatively rarely on the face. The occurence of verrucae in the facial region leads to limitation in treatment modalities and increases patient discordance. The clinical and epidemiological data of 37 patients (26 females) seen at the Dicle University Faculty of Medicine Dermatology outpatient clinics from January 2012 to January 2013 were reported. The mean age of the patients was 10.43±4.51 (range 4-18). The mean duration of lesions was 16.59±14.90 months. Number of lesions ranged from 1 to 50 (mean: 15.92). The most common localization on the facial area was the cheek in 23/37. Extrafacial involvement was present in 5/37 patients. The most common type of verruca in the facial region was verruca plana (50%). Before admission to our clinic, 56.8% had not received any form of treatment. Treatment modalities included reassurance - 16% -, cryotherapy -10.8% -, topical treatment with tretinoin cream, salicylic acid + 5-FU solution - 5.4% - and cauterization -1%-. Twelve of the 21 patients who had not previously received therapy had periorbital lesions. Positive correlation was present between age and duration of lesions (p < 0.01, r = 0.42).Öğe Evaluation of endothelial functions of psoriasis patients by flow mediated dilatation method(Duzce University Medical School, 2015) Oylumlu M.; Yüksel M.; Yıldız A.; Uçmak D.; Akkurt Z.M.; Karamanlıoğlu M.; Aydın M.Aim: Flow mediated dilatation (FMD) is a good sonographic indicator of early atherosclerotic changes which reflects endothelial functions by measuring dilatation of peripheral arteries in response to physical stressors. In this study, we aimed to measure FMD on brachial artery of patients with psoriasis which reflects endothelial dysfunction and compare to that of healthy volunteers. Methods: 30 patients with psoriasis and 31 control subjects matched in terms of age, gender, and demographic characteristics were included to this observational, case-control study. FMD was measured on the nondominant arm of the subjects by using sonographic techniques. Results: The patients and controls were similar in terms of sex, gender, smoking, arterial blood pressure, and body mass index. FMD values, reflecting endothelial functions, were tended to be lower in psoriasis group than healthy controls; however this difference was not statistically significant (7.7 ± 3.1 vs. 8.6 ± 2.8, respectively; p = 0.247). Conclusion: These results support that presence of psoriasis is not a risk factor for future cardiovascular disease development. Prospective, multi-center, large scale studies including control group are needed in order to clarify this issue. © 2012 Düzce Medical Journal.Öğe Generalized pustular psoriasis in two children: Acitretin therapy: Case report(Turkiye Klinikleri, 2014) Uçmak D.; Akkurt Z.M.; Türkçü G.; Acar G.; Akdeniz S.; Arica M.Von Zumbusch type pustular psoriasis is rarely seen in pediatric patients. Attacks of pustular psoriasis are characterized by fever that lasts several days, followed by sudden onset of pustules. The pustules are disseminated over the trunk and extremities, including the nail beds palms, and soles. Acitretin, alone, or combined with oral corticosteroids, is the first-line treatment method. Methotrexate or cyclosporine may also be used. Acitretin is a second generation aromatic retinoid. It takes effect by regulating keratinocyte proliferation and differentiation. A two-yearold girl and a three-year-old boy, presented with widespread pustules on an erythematous background, accompanied by fever and leukocytosis. Skin and blood cultures were negative. A diagnosis of Von Zumbusch type pustular psoriasis was made. We present two pediatric cases of generalized pustular psoriasis responding to acitretin rapidly and effectively. Copyright © 2014 by Türkiye Klinikleri.Öğe Herpes simplex superinfection with hailey-hailey disease: Case report(Turkiye Klinikleri, 2013) Uçmak D.; Harman M.; Akkurt Z.M.; Türkçü G.; Acar G.Hailey-Hailey disease (familial benign pemphigus) is an autosomal dominant acant-holytic dermatosis. The disease shows painful erosions and vesicles situated on an erythematous background especially in the intertriginous area. Herpes simplex infection associated with Hai-ley-Hailey disease differs from eczema herpeticum in that it is more localized. A 55-year-old female presented with vesicular lesions on the edge of wide inguinal erosions. A Tzanck smear revealed single hypertrophic nucleated acantholytic round cells with basophilic cytoplasm and one or two marked nucleoli at the periphery of the nucleus, adjacent to the nuclear membrane. Herpes simplex superinfection of Hailey Hailey disease is very rare and may not come to mind in diagnosis. A Tzanck smear, which is faster and easier than other tests for diagnosis, should be undertaken when there is clinical suspicion of herpes infection and antiviral treatment should be administered without delay. It is a rare autosomal dominant disease of early onset. Pyoderma gangrenosum, acne and hidradenitis suppurative (PASH) syndrome has been recently identified as nonhereditary and characterized by absence of arthritis different from PAPA syndrome. No mutations have been detected to date in patients with PASH syndrome. Only three patients with PASH syndrome were reported. Herein, we report a new patient with PASH syndrome. He had severe disseminated pyoderma gangrenosum and multiple depressed scars resulting from related acne and hidradenitis suppurative but lacked any episodes of pyogenic arthritis. He was successfully treated with systemic corticosteroids. Material and Methods: Seventy-two sexually active male patients with Behçet's Disease and 62 healthy control subjects were included in this study. Patients' demographic and clinical characteristics were noted. Sexual function was assessed by means of the International Index of Erectile Function scoring system. In addition, disease-related quality of life was measured by means of the Nottingham Health Profile, and anxiety and depression levels were assessed by means of the Hospital Anxiety and Depression Scale. Results: Patients with Behçet's Disease scored significantly lower in each of the 5 parameters of International Index of Erectile Function (p < 0.001) and significantly higher in Hospital Anxiety and Depression Scale compared to healthy control subjects (p < 0.001). International Index of Erectile Function scores correlated with age, duration of disease and Hospital Anxiety and Depression Scale and Nottingham Health Profile scores in patients with Behçet's Disease (p < 0.05). Conclusion: Sexual function is impaired in male patients with Behçet's Disease, which might be associated by age, duration of disease, psychological status and quality of life.
