Öğe 18F-FDG PET/CT parameters for prediction of response to neoadjuvant therapy and prognosis in rectal cancer(Wolters Kluwer Health, 2023) Ebinç, Senar; Güzel, Yunus; Oruç, Zeynep; Kömek, Halil; Kalkan, Ziya; Can, Canan; Taşdemir, Bekir; Urakçı, Zuhat; Kaplan, Muhammet Ali; Küçüköner, Mehmet; Işıkdoğan, AbdurrahmanObjective This study aims to investigate the role of F-18 fluorodeoxyglucose PET/computed tomography (18F-FDG PET/CT) parameters in the prediction of treatment response and the prognosis in locally advanced rectal cancer. Methods We investigated the relationship of 18F-FDG PET/CT parameters [rectal metabolic tumor volume (MTV), rectal total lesion glycolysis (TLG), rectal standard uptake value (SUV) max, rectal highest peak SUV, lymph node MTV, lymph node TLG, lymph node highest peak SUV] with the pathological response and disease-free survival (DFS) in 60 patients who received neoadjuvant therapy for a diagnosis of locally advanced rectal cancer. Patients with a total score of 0 were assigned to the low-risk group, patients with a score of 1 were assigned to the intermediate-risk group and patients with a score of 2 were assigned to the high-risk group. Results The multivariate analysis revealed that, from baseline PET CT parameters, lymph node highest peak SUV strongly predicted the pathological response at a cutoff value of 2.23. DFS was predicted by the lymph node highest peak SUV at a cutoff value of 3.13 and by the MTV value at a cutoff value of 27 cm3. The risk scoring performed with regard to rectal MTV and lymph node highest peak SUV values determined a median DFS of 19 months in patients with a risk score of 2, whereas the median DFS was not reached in patients with risk scores of 0 and 1 (P < 0.001). Conclusion This study determined that rectal MTV and lymph node highest peak SUV predicted the response to neoadjuvant therapy and DFS.Öğe Comparison of thyroid scintigraphy and ARFI-elastography in autoimmune thyroid diseases(MediHealth Academy Yayıncılık, 2022) Sezgin, İlhan; Taşdemir, Bekir; Kılınç, Faruk; Hamidi, Cihad; Çoraplı, MahmutAim: To compare the ARFI elastographic and scintigraphic quantitative data obtained from thyroid gland and to evaluate diagnostic value of ARFI elastographic measurements in autoimmune thyroid diseases. Material and Method: In the study a total of 61 patients (41 women, 20 men) including 24 Graves’ disease, 15 Hashimoto disease and 22 control subjects. The patients mean age was 33.9±10.9 years. Patients with thyroid nodules were not included in the study. Thyroid uptake was performed with Tc 99m pertechnetate and the elastosonography score of the thyroid gland was determined with ARFI. Findings of both disease groups were statistically compared with each other and control group Results: For thyroid uptake test; a statistically significant difference was found between two disease (p < 0.001) and between each disease and control group (p < 0.001 for Hashimoto, p <0.001 for Graves). However, for ARFI method; a statistically significant difference was not found between two disease (p=0.336) and between each disease and control group (p=0.14 for Hashimoto, p=0.08 for Graves). Conclusion: Thyroid scan and thyroid uptake measurements are extremely valuable in differential diagnosis of autoimmune thyroid diseases and ARFI elastosonographic measurements don’t seem to replace scintigraphic datas.Öğe Effect of imaging time on post stress left ventricular ejection fraction and volume measures by gated myocardial perfusion single photon emission computed tomography(2010) Çil, Habib; Arıtürk, Zuhal; Tekbaş, Ebru; Dostbil, Zeki; Taşdemir, Bekir; Elbey, Mehmet AliAmaç: Efor sonrası sol ventrikül ejeksiyon fraksiyonu (LVEF) ve sol ventrikül volüm değerlerinin koroner arter hastalığı bulunan hastalardaki kardiyak ölüm tahmininde katkısı vardır. Bu çalışmada, efor sonrası görüntüleme zamanının Quantitative Gated SPECT (QGS-Cedars-Sinai) programı ile hesaplanan efor sonrası LVEF, end-diastolik volüm (EDV) ve end-sistolik volüm (ESV) üzerine etkisini araştırmayı amaçladık. Hastalar ve yöntem: Bu çalışma 2008 ve 2009 yıllarında Elazığ Eğitim ve Araştırma Hastanesi Nükleer Tıp Birimine göğüs ağrısı nedeniyle Kardiyoloji Kliniğinden gönderilen 36 hasta üzerinde yapıldı. Tüm hastalara efor esnasında yapılan 99mTc-Mibi enjeksiyonunu takiben 20. ve 40. dakikalarda EKG-gated (8-bin frame modunda) miyokard perfüzyon SPECT sintigrafisi uygulandı. Kardiyak kantifikasyon yazılımı olan QGS programı kullanılarak LVEF, EDV ve ESV hesaplandı. İlk ve ikinci görüntülemelerden elde edilen LVEF, EDV ve ESV değerleri istatistiksel olarak birbirleri ile karşılaştırıldı.Bulgular: İlk ve ikinci görüntülemelerden elde edilen LVEF, EDV ve ESV değerleri arasında istatistiksel olarak anlamlı farklılık bulunmadı (p>0.05).Sonuç: Efor sonrası sol ventrikül fonksiyon parametreleri koroner arter hastalığı bulunan hastalardaki prognoz değerlendirmesinde önemlidir. Within the scope of this research, we aimed to elucidate the pathology reports and determine whether the metabolic parameters detected by PET/CT differ in MSI-positive and negative patients. Methods: A total of 35 patients were analyzed retrospectively. The patient population consisted of patients who applied to the Nuclear Medicine Department with a diagnosis of colon or rectum cancer, underwent PET/CT imaging for staging purposes, and were operated on. Results: A total of 35 colon or rectum cancer patients were included in this retrospective analysis. When microsatellite instability was analyzed among the patients, it was found that female patients comprised 4 microsatellite instability-positive and 16 microsatellite instability-negative individuals. On the other hand, 5 of the males were microsatellite instability positive, and 10 were microsatellite instability negative. The mean SUVmax value was 16.4±8.2, SUVmean was 8.1±1.9, TLG was 392.4±520.8, and MTV was 26.5±25.4 in the microsatellite instability-positive individuals. On the other hand, the mean SUVmax value was 22.7±9.7, SUVmean was 5.2±2.2, TLG was 316.4±325.7, and MTV was 21.7±21.7 in the microsatellite instability-negative individuals. Conclusion: With the advancement of image analysis technology, MTV, and TLG, volumetric indexes derived from 18F-FDG PET have been proposed for risk stratification of cancer patients. SUBJECTS AND METHODS: This retrospective cross-sectional study included a total of 708 patients with a diagnosis of malignancy. Fluorine-18-FDG PET/CT images acquired between January 2021 and December 2022. Exclusion criteria comprised prior chemotherapy, radiotherapy, hematological malignancies, or liver/spleen tumors. Statistical analysis included correlation analysis, univariate, and multivariate regression analysis. RESULTS: C-reactive protein levels demonstrated a significant positive correlation with 18F-FDG uptake in the spleen (r=0.104, P=0.006) and bone marrow (r=0.112, P=0.003). Albumin showed a negative correlation with liver 18F-FDG uptake (r=-0.220, P<0.001). Regression analysis revealed ESR's impact on spleen-to-liver (P=0.023) and bone marrow-to-liver (P=0.012) 18F-FDG uptake. CONCLUSION: This study demonstrates the association between inflammatory markers and 18F-FDG uptake in liver, spleen and bone marrow. C-reactive protein and ESR showing significant correlations with spleen and bone marrow 18F-FDG uptake, and albumin correlated with liver 18F-FDG uptake negatively. Erythrocyte sedimentation rate had significant impact on spleen and bone marrow 18F-FDG uptakes. These findings suggest the potential of 18F-FDG PET/CT in diagnosing inflammatory conditions, warranting further investigation into its clinical implications.Öğe Is there a relationship between the liver SUVmax values in FDG-PET/CT imaging and non-alcoholic fatty liver disease score?(2021) Yalçın, Kendal; Güzel, Fatih; Kaplan, İhsan; Güzel, Yunus; Taşdemir, Bekir; Uyar, Ali; Tuzcu, Şadiye AltunAim: Non-alcoholic fatty liver disease is one of the most common causes of liver disease worldwide with an estimated prevalence of 20%–30% in adult population. Following the widespread utilization of PET in the evaluation of malignant diseases, F-18 FDG have also been reported to be used in non-malignant processes. The aim of this study is to elucidate whether the FDG SUVmax values determined by PET/CT in different adipose tissue samples and the liver change according to NAFLD score. During our desktop research we did not find any published article therefore, it is the first study in this field. Materials and Method: A total of 230 patients who applied to Dicle University Faculty of Medicine, Department of Nuclear Medicine between March and April 2020 and who have been conducted FDG PET/CT for diagnosis, staging, restaging and evaluation of response to treatment were included in the study. Patients were divided into three groups according to their NAFLD score as patients with fibrosis score <-1,455 (the group in which severe fibrosis was excluded) as group-1, and those with NAFLD score between -1.455-0.676 (inter-mediate score) as group-2. and patients with a NAFLD score >0.676 (severe fibrosis group) group-3. Results: Liver SUVmax levels were found to be significantly higher in group-3 than group-1. No significant difference was observed between group-2 and group-3. SUVmax levels measured from supracalvicular region, posterior scapular region and mesentery region were not different from each other in all three groups. Glucose-corrected liver SUVglu levels were found to be significantly lower in group-1 than group-3 (p=0.001). In terms of liver SUVglu levels, group-1 and group-2 and group-2 and group-3 did not differ statistically from each other. Supracalvicular SUVglu, posterior scapular SUVglu and mesenteric SUVglu groups were not different from each other. Conclusions: The most important result of this study could be elaborated with increased FDG uptake in NAFLD. Liver FDG uptake increases as the severity of NAFLD increases as demonstrated by the NAFLD score.Öğe Laparoskopik splenektomi klinik deneyimimiz: 38 hastanın sonuçları(Dicle Üniversitesi Tıp Fakültesi, 2015) Bozdağ, Zübeyir; Türkoğlu, Ahmet; Taşdemir, Bekir; Oğuz, Abdullah; Dal, Sinan; Karakuş, Abdullah; Budak, Hıdır; Gümüş, MetehanAmaç: Laparoskopik splenektomi hematolojik hastalıkların tedavisinde son dönemlerde yaygın olarak kabul görmeye başlamıştır. Bu çalışmada laparoskopik splenektomi uyguladığımız hastalarımızın sonuçlarını sunmayı amaçladık. Yöntemler: Kliniğimizde 2012 – 2015 yılları arasında hematolojik hastalıklar nedeniyle laparoskopik splenektomi yapılan 38 hastanın kayıtları retrospektif olarak incelendi. Bulgular: Laparoskopik splenektomi uygulanan hastaların 15’i erkek, 23’ü kadın olup, ortalama yaş 33,9 ± 12,9 idi. Splenektomi endikasyonları; 34 hasta idiyopatik trombositopenik purpura (ITP), 4 hasta herediter sferositoz (HS) idi. Cerrahi eksplorasyonda 7 hastada aksesuar dalak tespit edilerek çıkarıldı. Safra kesesinde taş olan 2 HS’li hastaya aynı seansta laparoskopik kolesistektomi uygulandı. Bir hastada diseksiyon esnasında görüşü engelleyen hemoraji nedeniyle açığa geçildi. Postoperatif dönemde bir hastada atelektazi ve bir hastada yara yerinde yağ nekrozu gelişti. Ayrıca bir hastada trombositoz gözlendi. ITP’li 4 hastada geçici, iki hastada kalıcı refrakter trombositopeni nedeniyle hematolojik tedaviye devam edildi. Bu hastalara operasyon sonrası yapılan dalak sintigrafisinde bir hastada aksesuar dalak tespit edildi. Ortalama hastanede yatış süresi 2,6 ± 0,7 gün olarak bulundu. Sonuç: Hematolojik hastalık nedeniyle uygulanacak splenektomilerde laparoskopik splenektomi; daha az hastanede kalış süresi ve morbidite ile ilk seçenek tedavi modalitesi olarak düşünülebilir.Öğe Low bone density and high sclerostin in non-functioning pituitary adenoma(Elsevier Inc., 2023) Pekkolay, Zafer; Yıldırım, Devran Varhan; Tuzcu, Şadiye Altun; Taşdemir, Bekir; Tuzcu, Alpaslan KemalPurpose: Sclerostin reduces bone formation by inhibiting the Wnt signaling pathway in bone tissue. This study evaluated the serum sclerostin level in non-functioning pituitary adenoma (NFPA) patients and analyzed its relationship with bone metabolism. Method: The data of the patients who applied to the Dicle University Endocrinology, diagnosed with non-functioning pituitary adenoma, and the control group consisting of healthy individuals were included in the study. Serum sclerostin levels and DXA analysis parameters were evaluated and compared with healthy control groups. Results: The study consisted of 39 patients (F / M: 27/12) with NFPA (patient group) and 43 control groups (F / M: 26/17). There was no difference in terms of gender, age, height, weight and serum calcium, phosphorus, creatinine, 25-OH vitamin D, parathyroid hormone levels. Serum sclerostin levels (32.31 ± 1.53 ng / ml) in the patient group was found to be significantly higher than the control group (22.45 ± 8.9 ng / ml) (p < 0.001). BMD (Patients groups vs control group); total lumbar BMD (0.951-1.56 gr / cm2) (p < 0.001), femoral neck BMD (0.752-1.15 g / cm2) (p < 0.001), femoral total BMD (0.995- 1.63 gr / cm2) (p < 0.001), were found to be statistically significantly lower. Conclusion: This study provides the first evidence that serum sclerostin levels were increased in non-functioning pituitary adenomas, which showed that bone parameters were negatively affected.Öğe Myocardial perfusion scintigraphy fi ndings in patients with mild coronary atherosclerotic lesions on coronary angiography(Dicle Üniversitesi Tıp Fakültesi, 2010) Dostbil, Zeki; Çil, Habib; Tekbaş, Ebru Öntürk; Atılgan, Zuhal Arıtürk; İslamoğlu, Yahya; Taşdemir, Bekir; Dağ, YusufObjectives: Myocardial perfusion scintigraphy (MPS) is widely used in functional assessment of myocardial perfusion. But, some study results are in contradiction with severity of coronary artery disease detected by coronary angiography (CA). It is frequently encountered case that CA is completely normal whereas MPS describes ischemia. In this study, we aimed to investigate whether mild atherosclerotic lesions cause ischemia. Materials and methods: MPS with 99mTc-MIBI was performed in 52 patients who applied to cardiology clinics for history of chest pain and underwent diagnostic CA within 3 months. Results: In 22 of 52 patients with mild atherosclerotic lesions, ischemia in various degrees was detected on MPS. In statistical analysis, any signifi cant relationship was not found between ischemia and gender, hypertension, DM, dyslipidemia, smoking, mitral valve insuffi ciency, left ventricular hypertrophy, exercise testing result and affected coronary artery. Inflammation is known to be related to the development, spread, prognosis, and treatment response in cancer patients. Our study aimed to evaluate the correlation between inflammation indices and positron emission tomography-computed tomography (PET/CT) parameters and investigate their relationship with progression-free survival (PFS) and overall survival (OS) in patients diagnosed with stage-IV non -small cell lung cancer (NSCLC). Material and methods. Demographic, clinicopathological, laboratory, and PET/CT data of 179 patients diagnosed with stage-IV NSCLC who presented to the Oncology Department of Dicle University, Faculty of Medicine between 2010-2020 were retrieved from patient files and the hospital database system. Results. The median age at diagnosis was 64 (27-87) years. All patients included in the study had NSCLC: 72.6% had adenocarcinoma, 21.2% had squamous cell carcinoma, and 6.1% had other histological types. Of the 78 patients who were subjected to molecular analysis, 26 (33.3%) were EGFR-mutation positive. During the 10-month median follow-up, median first -line PFS was 6 months (95% CI 5.00-6.99), and median OS was 10 months (95% CI 7.8-12.1). The multivariate analysis performed for first -line PFS determined hemoglobin (HR = 1.01; 95% CI 1.003-1.02; p = 0.005) and PET total lesion glycolysis (TLG) (HR = 1.002; 95% CI 1.001-1.003; p = 0.003) values as independent prognostic factors. The multivariate analysis for OS determined positive EGFR mutation status (HR = 0.385; 95% CI 0.213-0.696; p = 0,014) and performance status (HR = 1.88; 95% CI 1.092-3.238; p = 0,008) as independent prognostic factors. Conclusions. Our study determined the hemoglobin level and PET TLG from PET/CT parameters to be independent prognostic factors for PFS, and performance status and EGFR mutation positivity to be independent prognostic factors for OS.Öğe Tc-99m MIBI SPECT miyokard perfüzyon sintigrafisindeki revers redistribüsyon paterninin sol ventrikül ejeksiyon fraksiyonu üzerindeki etkisi(2015) Yüksel, Murat; Taşdemir, Bekir; Dostbil, Zeki; Sezgin, İlhanAmaç: Revers redistribüsyon paterni (RRP)'nin nedenleri ve prognostik önemi henüz netlik kazanmamıştır. Bu çalışmada amacımız; RRP'nin sol ventrikül ejeksiyon fraksiyonu (EF) üzerindeki etkisini değerlendirmektir.Yöntemler: Teknesyum-99m sestamibi miyokard perfüzyon sintigrafisi yapılmış ve normal olarak değerlendirilmiş hastaların görüntüleri retrospektif olarak incelendi. Tek gün protokolü ve/veya farmakolojik stres testi uygulanan hastalar çalışma dışı bırakıldı. RRP saptanan toplam 21 hasta çalışma kriterlerini sağladı. Bu hastaların tamamı RRP grubu olarak çalışmaya dahil edildi. Randomize seçilen 21 RRP saptanmayan hasta ise kontrol grubu olarak çalışmaya alındı. Transiyel iskemik dilatasyon (TID) skorları ve EF değerleri Quantitative Gated SPECT (QGS) programı kullanılarak otomatik olarak hesaplandı. Bulgular: Yaş, cinsiyet, hipertansiyon, aile hikayesi, hiperlipidemi ve diyabet yönünden RRP ve kontrol grubu arasında istatistiksel olarak anlamlı bir fark saptanmazken (p>0,05), sigara kullanımı yönünden anlamlı fark tespit edildi (p=0,019). RRP grubundaki TID skorları, kontrol grubuna kıyasla anlamlı bir şekilde daha düşüktü (p<0,001). Ayrıca RRP grubunda, istirahat görüntülerindeki EF değerlerinde, stres görüntülerindekilere kıyasla anlamlı bir şekilde düşüş tespit edildi (p=0,002). Dahası bu düşüş, kontrol grubundakine kıyasla anlamlı bir şekilde daha yüksekti (p=0,034). Sonuç: TID skoru ve EF değerleri ile ilgili bulgular, RRP zemininde iskemik bir nedenin varlığını düşündürmektedir. Current information on Ga-68 PSMA PET/CT shows that primary staging with PET/CT is important in patients with high-risk PC. Ga-68 PSMA PET/CT may also have a place in patients with intermediate risk PC, but only a few data are available at present. In this study, we aimed to elucidate the relationship between PSMA expression value in the prostate gland, total PSA levels and GS in patients diagnosed with prostate cancer. Material and Method: A total of 98 patients who were pathologically diagnosed with prostate cancer that did not receive any treatment and underwent Ga-68 PSMA PET/CT imaging for staging were included in the study. Findings detected in Ga-68 PSMA PET/CT imaging were categorized as prostate, lymph node, bone, and visceral organ. The focal increased PSMA expression values ??observed in the prostate gland were recorded as SUVmax. The patients were divided into two separate groups according to their GS score (GS>7 and GS?7). Correlations between prostate PSMA SUVmax, GS score and total PSA scores were investigated. Results: PSMA SUVmax levels of the group with a Gleason score of >7 were found to be significantly higher than the group with a Gleason score of ?7 (p=0.03). The rates of lymph node metastasis, bone metastasis and visceral organ metastases were found to be significantly higher in the group with a Gleason score >7 compared to the group with a Gleason score ?7. A positive correlation was observed between PSMA SUVmax and Total PSA (r=0.260, p=0.01). A positive correlation was observed between PSMA SUVmax and Gleason score (r=0.260, p=0.01). A positive correlation was observed between total PSA and Gleason score (r=0.320, p=0.001). Conclusion: In conclusion, PSMA SUVmax and Total PSA levels were higher in the group with high Gleason score. There is a positive correlation between PSMA SUVmax and total PSA. Clinicians should be careful in this regard, as the possibility of metastasis will be high in groups with high Gleason scores.Öğe Tükürük bezi sintigrafisinde majör tükürük bezi fonksiyonları hesaplanmasında geri plan düzeltmesinin önemi(Dicle Üniversitesi Tıp Fakültesi, 2015) Taşdemir, Bekir; Dostbil, ZekiAmaç: Bu çalışmanın amacı, parotis (P) ve submandibuler (SM) bezlerin 5 farklı bölgeye çizilen geri planlar kullanılarak hesaplanan 99mTc-perteknetatı konsantre ve ekskrete etme oranlarını karşılaştırmaktır. Yöntemler: Toplam 49 hastanın 98 P ve 98 SM bezi çalışmaya dahil edildi. Tüm hastalara 99mTc-perteknetat ile tükürük bezi sintigrafisi yapıldı. Görüntülemenin 15. dakikasında limon suyu ile oral stimülasyon gerçekleştirildi. Geri plan düzeltmesi yapılmaksızın ve geniş frontal, dar frontal, temporal, servikal ve omuz bölgesine çizilen geri planlar kullanılarak hesaplanan P ve SM bezlerin dolaşımdan 99mTc-perteknetatı tutma ve ekskrete etme oranları ayrı ayrı hesaplandı. Bu değerler ikili olarak istatistiksel olarak birbirleri ile karşılaştırıldı. Bulgular: P ve SM bezlerin geri plan düzeltmesi yapılmaksızın hesaplanan radyoaktivite tutulum ve ekskresyon oranları, geri plan düzeltmesi yapılarak hesaplanan oranlardan istatistiksel olarak anlamlı bir şekilde farklı bulundu. Ayrıca beş farklı bölgeye çizilen geri planlara göre hesaplanan P ve SM bezlerin radyoaktiviteyi tutma ve ekskrete etme oranları çoğunlukla istatistiksel olarak anlamlı bir şekilde farklı bulundu (p<0,05). Sonuç: P ve SM bezlerin perteknetatı tutma ve ekskrete etme oranlarını doğru hesaplamada geri plan düzeltmesi yapmak önemlidir. Bu çalışma P ve SM bezlerin perteknetatı tutma ve ekskrete etme oranlarının kullanılan geri plana göre değişebileceğini ortaya koymuştur.
