Öğe Comparison of Chest Computed Tomography Findings in Patients with H1N1 and Coronavirus Disease-2019 Pneumonia(Galenos Publ House, 2024) Simsek, Sadullah; Ozmen, Cihan AkgulObjective: In this study, we compared the differences between highresolution computed tomography (CT) features of two types of viral pneumonia associated with H1N1 virus and coronavirus disease -2019 (COVID-19). Methods : A total of 25 patients with H1N1 pneumonia were compared with 150 patients with COVID-19 pneumonia. The findings were analyzed by IBM SPSS Statistics for Windows, version 22 (IBM Corp., Armonk, N.Y., USA). CT findings were compared between groups by chi-square test, and scale variables were compared using the t -test. Finally, significant findings for the H1N1 or COVID-19 groups were evaluated by logistic regression analysis. Results: The median age of COVID-19 pneumonia patients was 54.6 +/- 15.9 years, and the median age of H1N1 pneumonia patients was 46.2 +/- 12.4 years (p<0.01). H1N1 pneumonia patients were younger than COVID-19 pneumonia patients. Based on the distribution pattern, diffuse pattern, peripheral pattern, and bronchopulmonary infiltration were observed more frequently in COVID-19 pneumonia (46.0%, 80.7%, 49.3%, respectively, p<0.05). Mediastinal lymphadenopathy and pleural effusion were observed more frequently in patients with H1N1 pneumonia (24% and 36%, respectively, p<0.05). Logistic regression analysis demonstrated that advanced age and a diffuseperipheral pattern indicated COVID-19 (p=0.031, p=0.029). Conclusion: Most of the lesions caused by COVID-19 pneumonia are located in the peripheral region and near the pleura, whereas those caused by influenza virus pneumonia are far from the peripheral region.Öğe Correlation of the Neutrophil-to-Lymphocyte Ratio with the Middle Ear Risk Index in Patients with Chronic Otitis Media(Springer India, 2022) Islek, Akif; Balci, Mustafa Koray; Simsek, SadullahThis study aimed to investigate the neutrophil-to-lymphocyte ratio (NLR) and severity of the chronic otitis media according to middle ear risk index (MERI) scores. The NLR and middle ear risk index were calculated for 210 chronic otitis media (COM) patients retrospectively. NLR compared between COM patients and 159 participants in control group. Pearson correlation analysis was performed for NLR and MERI. In addition, The cut-off value, sensitivity, and specificity for NLR were determined in COM according to the severity of the disease with ROC analysis. The average of NLR in all patients was 1.94 +/- 0.89. NLR was calculated as 2.05 +/- 1.03 in the patients with COM and 1.79 +/- 0.64 in the control group (p = 0.006, 95% CI 0.07-0.44, t test). According to the ROC analysis, the cut-off point was determined as 1.95 for moderate and severe disease in MERI. For the cut of a NLR = 1.95, sensitivity was calculated as 48.5% and the specificity 57.1% for moderate and severe COM. MERI score was found significantly higher in patients with postoperative graft perforation (p < 0.001, 95% CI 0.7-1.8). NLR has not related significantly with the MERI score according to linear regression analysis (p = 0.927). NLR was found to be significantly higher in chronic otitis media patients compared to the control group. But NLR was not related to the severity of the disease classified with MERI. The cut-off value, sensitivity and sensitivity rates of NLR obtained was not available for clinically use.Öğe Diagnostic and predictive value of resistive / pulsatility indices of ophthalmic artery and common carotid artery for the development of diabetic retinopathy(Sage Publications Ltd, 2023) Simsek, Sadullah; Islek, AkifBackground Ophthalmic artery resistive index (OA RI) was a controversial parameter to show hemodynamic changes due to diabetic retinopathy (DRP). Purpose To investigate the diagnostic and predictive value of resistive and pulsatility index (RI and PI, respectively) of OA and common carotid artery (CCA) for the development of DRP. Material and Methods A total of 60 patients with diabetes mellitus (DM) type 2 (study group) and 30 healthy participants (control group) were evaluated between January and June 2021 by Doppler ultrasonography (DUS). RI and PI values were compared between groups with a Student's t-test. Cutoff value, sensitivity, and specificity were calculated for the significant variables with receiver operating characteristic (ROC) analysis. Results In total, there were 20 (22.2%) patients with DM without DRP (DMwoRP), 20 (22.2%) patients in the non-proliferative diabetic retinopathy group (NPDRP), and 20 (22.2%) patients in the proliferative diabetic retinopathy group (PDRP). The mean of CCA RI and OA RI in the PDRP group was significantly higher than in the other three groups (P < 0.001). The mean of CCA RI and OA RI was significantly higher in the PDRP group than in the NPDRP group, and in the NPDRP group compared to the DMwoRP group. CCA RI and OA RI showed a significantly high correlation (r = 0.849; P < 0.001). Sensitivity was 95% and specificity was 100% for the diagnosis of PDRP for the 0.82 cutoff value of OA RI (AUR = 0.999, 95% confidence interval for AUC = 0.997-0.1000; P < 0.001). Conclusion The OA RI accurately reflects DRP-induced orbital blood flow changes and is a predictive index for DRP prognosis.
