Assessment of sexual function and quality of life in patients with lower limb amputations
(Ios Press, 2019) Em, Serda; Karakoc, Mehmet; Sariyildiz, Mustafa Akif; Bozkurt, Mehtap; Aydin, Abdulkadir; Cevik, Remzi; Nas, Kemal
BACKGROUND: Diseases causing disabilities lead to sexual dysfunction. However, studies that evaluate sexual functions in patients with traumatic lower limb amputations (LLA) are limited and controversial. OBJECTIVE: The objective is to evaluate sexual functions in male patients with LLA and to identify factors that might contribute to sexual dysfunction. The hypothesis is that male patients with LLA would experience worse sexual and emotional functioning and consequently emotional impairment that consequently influenced the sexual function. METHODS: Sixty men with traumatic LLA and 60 healthy men were assessed according to the International Index of Erectile Function (IIEF) scoring system. Emotional state was assessed by the Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI), and the quality of life by the Short Form-36 Health Survey (SF-36). RESULTS: Patients had significantly lower scores in the IIEF total score and subscores in comparison with the controls. Intercourse satisfaction scores were significantly higher (p < 0.05) in patients with trans-tibial amputation than in those with trans-femoral amputation. BDI and BAI scores negatively correlated with erectile function, sexual desire, intercourse satisfaction, overall satisfaction and total IIEF scores. The mental and physical scores of SF-36 significantly correlated with some of the sub scores and IIEF total scores. The power of statistical analysis for study population was 100% according to the given effect size (alpha = 0.01). CONCLUSION: The present study suggests that the LLA leads to impairments in the sexual function and quality of life in male patients. Also, sexual dysfunction in patients is strongly associated with emotional state, pain, level of amputation and quality of life.

(Georg Thieme Verlag Kg, 2023) Demir, Mesut; Batmaz, Ibrahim; Sariyildiz, Mustafa Akif; Ekinci, Aysun; Akar, Zeynel Abidin; Batmaz, Oznur; Karakoc, Mehmet
Introduction This study sought to determine the serum levels of follistatin-like protein 1 (FSTL-1), a glycoprotein of mesenchymal origin shown to be involved in the pathogenesis of many inflammatory diseases, for example Behcet's disease (BD), and to investigate its association with disease activity.Material and Methods The study included 45 patients followed for newly diagnosed Behcet's disease who met the 1990 International Study Group criteria for BD and a control group of 36 healthy volunteers who were age- and gender-matched. Peripheral blood samples were collected from the patients and control groups. Serum FSTL-1 levels were measured using an enzyme-linked immunosorbent assay. The disease activity was assessed using the Behcet's Disease Current Activity Form (BDCAF).Results A comparison of serum FSTL-1 levels in the patient and control groups found significantly higher levels of FSTL-1 in the patient group. Serum FSTL-1 levels were significantly higher in female patients compared with male patients. Female patients exhibited significantly higher BDCAF scores compared with male patients (p=0.008). Erythrocyte sedimentation rate and serum FSTL-1 levels were positively correlated. Conclusion Serum FSTL-1 levels may play a role in the aetiopathogenesis of BD and can also be a useful biomarker for determining disease activity.

(Taylor & Francis Ltd, 2014) Batmaz, Ibrahim; Dilek, Banu; Sariyildiz, Mustafa Akif; Erbatur, Serkan; Bez, Yasin; Yazmalar, Levent; Bozkurt, Mehtap
Objective: To determine the affective temperaments and somatoform amplification of the symptoms and its relation with the complex regional pain syndrome [CRPS] in patients who had tendon injuries of the forearm and hand. Methods: Sixty-seven patients [60 males, 7 females] with a forearm or hand tendon injury were enrolled in the present study. The patients were divided into two groups: CRPS and non-CRPS. The patients' pain levels within the last 48 hours were assessed using the visual analog scale [VAS]. The temperament of the patients was evaluated using the Temperament Evaluation of Memphis, Pisa, Paris and San Diego Auto questionnaire [TEMPS-A]. Their anxiety levels were evaluated with the help of the State-Trait Anxiety Inventory [STAI]. The depression assessment was performed using the Beck Depression Inventory [BDI]. The Somato sensory Amplification Scale [SSAS] was employed for the evaluation of the somatoform disorders. Results: The mean age of the patients was 26.21 +/- 8.01 years [age range: 18 to 57 years]. Among the patients 24 [35.8%] had developed CRPS. When the patients with and without CRPS were compared in terms of their psychological scores, the STAI-II and BDI scores were observed to be significantly deteriorated in the CRPS group in comparison to the non-CRPS group [p<0.05]. Among the TEMPS-A subtypes, anxious [16.7%] and depressive [41.7%] temperaments were more dominant in the CRPS group and the ratio was significantly more deteriorated than the non-CRPS group [p<0.05]. No significant difference was observed between the groups regarding the SSAS scores [p>0.05]. Conclusion: The present study is the first one evaluating the affective temperaments of CRPS patients with tendon injury, and the main findings point out that depressive and anxious temperaments are the most common dominant affective temperaments among our patients.

(Ios Press, 2017) Sariyildiz, Mustafa Akif; Batmaz, Ibrahim; Yazmalar, Levent; Gunes, Mehmet; Turan, Yahya
BACKGROUND: The significance of fluoroscopy-guided transforaminal epidural steroid injections (TFESI) in the treatment of lumbar disc herniation (LDH) is well known. The aim of our study is to investigate the effectiveness of TFESI on radicular pain, functionality, psychological status, and sleep quality in patients with LDH. METHODS: Seventy-five LDH patients (36 males, 39 females) were enrolled in the study. All patients received a fluoroscopically guided TFESI (betamethasone 40 mg, lidocaine 2%). Also all patients were evaluated according to (with the visual analogue scale) radicular pain, Oswestry disability index (ODI), hospital axiety and depression scale, and Pittsburgh sleep quality index (PSQI) at baseline, at two weeks, and 12 months post injections. RESULTS: Mean age was 46.4 +/- 12.5. When compared to baseline measurements there were significant improvements in radicular pain, ODI, modified schober, Laseque angle, finger to floor distance, depressive symptoms and PSQI scores at two weeks and 12 months after injection. Improvement of at least 50% in radicular pain relief, ODI score and sleep quality index was detected at two weeks 83%, 71%, 69% respectively. This rate showed regression at 12 months of 73%, 65% and 62% respectively. Duration of symptoms was significantly negatively correlated with changes in scores of radicular pain, ODI, depressive symptoms, and PSQI. There were no significant correlations with symptom duration and anxious symptoms. CONCLUSION: Fluoroscopy guided TFESI had positive effects on radicular pain, functionality, depressive symptoms and sleep quality in management of LDH.

(Ios Press, 2016) Dilek, Banu; Batmaz, Ibrahim; Sariyildiz, Mustafa Akif; Sahin, Ebru; Ilter, Leman; Gulbahar, Selmin; Cevik, Remzi
BACKGROUND: Lateral epicondylitis (LE) is a painful condition that affects the tendinous tissue of the lateral epicondyle of the humerus and leads to loss of function of the affected limb. Therefore it can have a major impact on the patient's social and personal life. Many treatments are recommended for lateral epicondylitis; unfortunately the evidence is limited. OBJECTIVES: The aim of study was to investigate the effect of kinesio taping (KT) on pain, grip strength and function in patients LE. METHODS: Thirty-one (23 females, 8 males) patients with LE were included. KT was applied twice a week for 2 weeks. Pain at rest, activity of daily living (ADL), night and palpation on lateral epicondyle was evaluated with the visual analog scale (VAS 0-10 cm), and the grip strength was measured with a hand held dynamometer. The stage of the disease was evaluated by the Nirschl score and the functional status was assessed with Patient-Rated Forearm Evaluation Questionnaire (PRTEQ) score. These parameters were evaluated before, at 2 weeks and 6 weeks after treatment. Patients' satisfaction was also recorded on a Likert scale after treatment at 2 weeks and 6 weeks. RESULTS: The average age of the patients was 43.58 +/- 9.02. The dominant limb was affected in 64.5% (20) of the patients. After the application of KT on lateral epicondyle, there was a significant improvement in all parameters in terms of pain, Nirschl score, hand grip strength, patient satisfaction, and PRTEQ scores at 2 and 6 weeks. CONCLUSIONS: Kinesio taping can be an effective treatment method in LE. This application improves pain, grip strength and functional status of the patients with LE.

RADIOLOGICAL DAMAGE IN RHEUMATOID ARTHRITIS: RELATIONSHIP BETWEEN THE DURATION OF MORNING STIFFNESS, DEPRESSIVE SYMPTOMS, AND DISEASE DURATION
(Carbone Editore, 2015) Celepkolu, Tahsin; Sariyildiz, Mustafa Akif; Yazmalar, Levent; Bucaktepe, P. Gamze Erten; Cetincakmak, Mehmet Guli; Satici, Omer
Aims: The aim of this study was to evaluate radiological damage and disease-related variables in patients with rheumatoid arthritis (RA). Material and methods: Ninety patients diagnosed with RA were enrolled in this cross-sectional and descriptive study. All of the patients were assessed using the Rheumatoid Arthritis Quality of Life and Health Assessment Questionnaire scales and the Beck Depression Inventory. The Disease Activity Score 28 scale was used to assess disease activity, and antibodies against mutated citrullinated vimentin (Anti MCV) were identified with an Enzyme-Linked Immuno Sorbent Assay. Radiological damage was calculated with the modified Larsen method. Results: According to the suggested Cut-off point for the Larsen score (scores >= 28), 35.5% (n=32) of the patients with RA had an erosive score. The patients with erosive RA had significantly higher Anti MCV, disease duration and Beck depression scores than did the non-erosive RA group. Multivariate regression analyses indicated that the Beck depression score, disease duration and duration of morning stiffness were independently associated with the radiological score. Conclusion: Radiological damage in RA patients is a result of multifactorial origins and is associated with disease-related variables such as disease duration, duration of morning stiffness, Anti- Cyclic citrullinated peptide levels and depressive symptoms.

Relationship between serum leptin level and disease activity in patients with systemic sclerosis
(Springer London Ltd, 2014) Budulgan, Mahmut; Dilek, Banu; Dag, Sevin Buluttekin; Batmaz, Ibrahim; Yildiz, Ismail; Sariyildiz, Mustafa Akif; Cevik, Remzi
To determine the relationship between serum leptin levels and disease activity in systemic sclerosis (SSc). A total of 60 subjects (30 controls and 30 patients) were included. The inflammatory markers and leptin levels were evaluated and body mass index (BMI) was measured for both groups. The assessment of the skin involvement was performed based on the modified Rodnan skin score (mRSS). Disease activity was evaluated according to the Valentini scleroderma disease activity index. There was a significant difference between the patient and control groups in terms of BMI (p < 0.05); however there was no difference with regards to age and gender (p > 0.05). Valentini scores and mRSS were determined to be significantly higher in active patients (n = 14) than in inactive patients (n = 16) (p < 0.05). No significant difference was determined between groups in terms of leptin levels (p > 0.05). However, leptin levels were significantly lower in active patients than in inactive patients (p < 0.05). We found a significant positive correlation between serum leptin and BMI (p < 0.05), and leptin and serum C3 levels (p < 0.05); no relationship was detected between leptin and other parameters. Leptin can be used as an activity marker in SSc. Further studies, including larger series, should be carried out to clarify this relationship.

Relationship of the HLA-DRB1 alleles and seropositivity, anti-MCV, functional status and radiological damage in Turkish patients with rheumatoid arthritis
(Ios Press, 2013) Sariyildiz, Mustafa Akif; Batmaz, Ibrahim; Cetincakmak, Mehmet Guli; Yildiz, Ismail; Nas, Kemal; Cevik, Remzi
OBJECTIVES: The HLA-DRB1 gene plays an important role in the genetic predisposition to rheumatoid arthritis. The anti-citrullinated protein antibody (ACPA) is known to be associated with the susceptibility to rheumatoid arthritis (RA), as well as the severity of the disease. The aim of this study is to determine the HLA genes that are associated with the severity of the RA measured by seropositivity, functional status, radiological damage and the anti-modified citrullinated vimentin (MCV) levels. MATERIAL AND METHOD: Ninety-six patients diagnosed with rheumatoid arthritis and a control group consisting of 84 healthy individuals were enrolled in the study. The HLA-DRB1 type and subtypes were specified using the polymerase chain reaction with sequence-specific primers (PCR-SSP). The association of the HLA-DRB1 type and subtypes with seropositivity, anti-MCV levels, functional status and anatomical joint damage were explored using the modified Larsen scoring method. RESULTS: The DRB1*01, DRB1*04, DRB1*07, DRB1*10, DRB1*11 and DRB1*15 alleles were found to be related with higher anti-MCV levels (p < 0.05). The DRB1*11 type and the DRB1*1001 subtype were observed to be associated with poor functional status (Stages 3-4). The DRB1*0801 subtype was associated with lower anti MCV levels (OR = 8.35, p = 0.02). DRB1*04 type and DRB1*0405 subtypes were related with radiological damage (OR = 0.52 and 0.25; p = 0.04 and 0.03, respectively). No significant relationship was observed between the RF seropositivity and the HLA-DRB1 alleles. CONCLUSION: This study revealed the association between the HLA alleles and seropositivity, functional status, anti-MCV levels and radiological damage in patients in the Southeast Anatolian region of Turkey.

Relationship of the sexual functions with the clinical parameters, radiological scores and the quality of life in male patients with ankylosing spondylitis
(Springer Heidelberg, 2013) Sariyildiz, Mustafa Akif; Batmaz, Ibrahim; Dilek, Banu; Inanir, Ahmet; Bez, Yasin; Tahtasiz, Mehmet; Em, Serda
The aim of this study was to explore the impact of ankylosing spondylitis (AS) and the disease-related variables on the patients' sexual function according to the International Index of Erectile Function (IIEF) scoring system. A total of 70 sexually active male AS patients and 60 healthy controls were enrolled in this study. Their demographic data were evaluated, and the pain was assessed according to the visual analogue scale (VAS). Laboratory tests were conducted in order to measure the C-reactive protein (CRP) and erythrocyte sedimentation rates (ESR) of the patients. The disease activity was evaluated using the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI). Functional statement was evaluated with the help of the Bath Ankylosing Spondylitis Functional Index (BASFI) and with the scores obtained from the spinal measurements with the Bath Ankylosing Spondylitis Metrology Index (BASMI). The Bath Ankylosing Spondylitis Radiology Index (BASRI) was used to evaluate the radiological damage. The disease-related quality of life was measured with the Ankylosing Spondylitis Quality of Life Questionnaire (ASQoL). The anxiety and depression level of the patients was revealed through the Hospital Anxiety and Depression Scale (HADS). In comparison with the healthy control group, patients with AS had significantly lower scores in each of the 5 domains of the IIEF (p < 0.0001). The BASDAI, BASFI, BASMI, BASRI, ASQoL, HADS scores and CRP levels were negatively correlated with IIEF (p < 0.05). Orgasmic function and sexual desire scores were significantly lower in patients with peripheral arthritis (p < 0.05). No significant correlation was observed with the disease duration, smoking status, pain (VAS), and ESR levels when the total scores and the scores from the domains of IIEF were compared. The multivariate regression analyses indicated that BASFI and BASMI were independently associated with the sexual function. The sexual function is impaired in male patients with AS. This impairment in the sexual function is especially correlated with the BASFI and BASMI among the clinical and laboratory parameters.

Serum GDF-15 level in Behcet's disease: relationships between disease activity and clinical parameters
(Wiley, 2016) Sariyildiz, Mustafa Akif; Yazmalar, Levent; Batmaz, Ibrahim; Alpayci, Mahmut; Burkan, Yahya Kemal; Sula, Bilal; Kaplan, Ibrahim
Growth differentiation factor-15 (GDF-15), a member of the transforming growth factor-b superfamily of cytokines, plays an important role in cell growth, signal transduction, and apoptosis regulation. The aim of this study was to evaluate serum GDF-15 levels and their relationships with disease-related variables in patients with Behcet's disease (BD). Fortysix patients diagnosed with BD and 30 demographically matched healthy control subjects participated in the study. GDF-15 levels were measured in blood samples from patients and controls. The Behcet's Disease Current Activity Form (BDCAF) was used to evaluate the disease activity of BD. There were no significant differences between the two groups in C-reactive protein (CRP) level, mean erythrocyte sedimentation rate (ESR), age, body mass index, and mean GDF-15 levels (P > 0.05). Serum GDF-15 levels were positively correlated with findings for peripheral arthritis and CRP, and with BDCAF erythema nodosum, BDCAF arthralgia, and BDCAF arthritis scores. Patients with BD were divided into two groups according to the presence of peripheral arthritis; nine subjects (20%) were positive for peripheral arthritis. Serum ESR, CRP, white blood cell counts, and GDF-15 levels were significantly higher in the group that was positive for peripheral arthritis (P < 0.05). GDF-15 may play a role in the progression and pathway of Behcet's joint involvement and erythema nodosum that is independent of classic inflammatory response measures.

Results: On average, FM patients were diagnosed within 3.2 years of symptom onset, and patients had a mean of 14 tender points. There were no significant differences between patients and controls in age, body mass index, serum TAS, or PON-1 levels. However, patients with FM demonstrated higher serum prolidase activity, TOS, and OSI than the control group. Serum prolidase activity was positively correlated with serum TOS, OSI, and visual analog scale pain and fatigue scores. No correlation was found between serum prolidase activity and FM duration or the average number of tender points. Discussion: Our results demonstrate a previously unreported association between serum prolidase enzyme activity and FM. Increased prolidase activity may contribute to the pathogenesis of FM, and measuring serum prolidase enzyme activity may be a useful FM biomarker.

SERUM PROLIDASE ENZYME ACTIVITY AND OXIDATIVE STATUS IN PATIENTS WITH SCLERODERMA
(Carbone Editore, 2014) Bozkurt, Mehtap; Dag, Evin; Oktayoglu, Pelin; Em, Serda; Yuksel, Hatice; Caglayan, Mehmet; Sariyildiz, Mustafa Akif
Objective: To assess serum prolidase enzyme activity and oxidative stress in patients with scleroderma (also known as systemic sclerosis, or SSc) and determine its relationship with serum oxidative status. Methods: The study population consisted of SSc patients (n = 21) and healthy participants (n =29). Serum prolidase enzyme activity, total antioxidant status (TAS), total oxidative status (TOS), oxidative stress index (051), and paraoxonase-1 (PON-1) levels were compared betweeen the two groups. Results: The mean duration of SSc involvement was 5 years; and the mean modified Rodnan skin score was 16. No statistically significant differences existed between the SSc groups and the control participants in terms of age, gender, TAS, PON-I, and haematocrit levels. Serum prolidase activity, 051, and TOS levels were statistically significantly higher in SSc patients in comparison with controls participants. Also serum prolidase activity was statistically higher in Raynaud's phenomenon positive patients than Raynaud's phenomenon negative patients. But no correlation emerged between the serum prolidase activity and modified Rodnan Skin Score (P=0.235, r=0.304). Conclusion: High prolidase activity may indicate critical biological activities relevant to pathological events in SSc, and this activity may be a biological indicator of disease. Further studies are needed to verify these findings.
