Öğe Chylothorax with Down syndrome: Unusual case report(2009) Avci A.; Ülkü R.; Onat S.Trisomy 21 or Down syndrome (DS) is the most common chromosome aberration in the newborn. Its high incidence has led to better understanding of congeniatal anomalies and diseases in association with this syndrome. The presence of protein, triglycerides, and lymphocytes in the pleural effusion is diagnostic of chylothorax. Lymph vessel abnormalities or a leaky thoracic duct are thought to be origin of congenital chylothorax, which is repoted many times in the literature. But, there is no information in the english literature about 1-year-old baby with DS who is complicated with chylothorax. Aplasia of the twelfth rib is rare but found to be more common in DS than in normals. We report on a 1-year-old girl with DS refered to our clinic with signs and symptoms of respiratory distress. Diagnosis and treatment of chylothorax, pleural emphyema, and pericardial effusion were obtained in algorithm.Öğe Evaluation of patients diagnosed with spontaneous hemopneumothorax(Marmara University, 2017) Oruç M.; Monis S.; Şahin A.; Erbey A.; Meterolu F.; Onat S.Objective: Spontaneous hemopneumothorax (SHP) may threaten life unless timely diagnosed and appropriately treated. The aim of this study was to review our institutional experience in the management of patients with SHP. Patients and Methods: We retrospectively analyzed age, gender, the affected side, the amount of drainage, clinical findings, and treatment outcomes of 610 patients with diagnosis of spontaneous pneumothorax at emergency department. Later, they were admitted to the Department of Thoracic Surgery, Dicle University Hospital. Results: Posteroanterior chest radiographs, thoracic computed tomography (CT) scans, biochemistry and coagulation test results of all patients were evaluated. All patients underwent closed chest drainage. Seven (1.14%) of 610 patients were diagnosed with SHP. These patients underwent operation after thoracic drainage. Three of them were operated by video-assisted thoracoscopic surgery (VATS) and the others by thoracotomy. No postoperative complication was observed. Conclusion: SHP should be considered in patients presenting with sudden chest pain and dyspnea when there is air-fluid level in addition to pneumothorax on radiography. The first treatment approach should be the application of tube thoracostomy. Next step should include close clinical and hemodynamic control and be followed primarily by VATS or urgent thoracotomy in case of continued bleeding. An early diagnosis and appropriate surgical approach can prevent fatal complications. © Marmara University. Pulmonary pleomorphic carcinoma was introduced by the 1999 World Health Organization classification as a new peculiar type of lung carcinoma showing concurrent malignant epithelial and sarcomatoid spindle cell elements. Rarely seening, advange age, male sex dominance, upper lobe involvement, big sized (more than 7 cm.) at time of diagnosis , and poor prognosis are the characteristics of it. A 33-year-old female patient with a primary extragonadal giant yolk sac tumor presenting with thoracic wall mass, complaint of back pain.Öğe A rare cause of respiratory distress in a newborn: Mediastinal enteric cyst associated with costovertebral malformation: Case report(2010) Özbek M.N.; Taşkesen M.; Katar S.; Ecer S.; Onat S.Mediastinal enteric cyst is usually associated with segmental costovertebral malformations include Jarcho-Levin syndrome, spondylocostal dysostosis, and spondylothoracic dysostosis. We aimed to report a case of thoracal vertebral anomaly associated with a mediastinal enteric cyst. A 1 day-old girl patient was admitted to our clinic with symptoms of dyspnea, cyanosis, grunting and poor sucking. Low hairline, a short webbed neck, and flexion contracture in the left fourth phalanx were detected in her physical examination. Posteroanterior chest X-ray and thorax computerized tomography demonstrated a large cystic lesion at the right lung. Herein this newborn with respiratuar distress due to mediastinal enteric cyst associated with costovertebral malformation was reported. Copyright © 2009 by Türkiye Klinikleri.Öğe Resection of intercostal hemangioma with involved chest wall and ribs: In an 11-year-old girl(2010) Ulku R.; Onat S.; Avci A.; Ozmen C.A.We report a case of an 11-year-old girl who presented with a slowly enlarging mass in the right posterolateral chest wall. Computed tomography showed a soft-tissue mass 8.5 × 7.5 × 5.5 cm in size, arising from the right posterolateral 9th, 10th, and 11th intercostal spaces. Magnetic resonance imaging confirmed a vascular mass. The patient underwent complete resection of the tumor, together with the right 8th, 9th, 10th, 11th, and 12th ribs and their intercostal muscles. Reconstruction of the chest wall was performed with methyl methacrylate and Marlex mesh. Histopathologic examination of the tumor confirmed an intercostal cavernous hemangioma. At last examination, 6 months after the operation, the child was doing well, with no evidence of recurrence. © 2010 by the Texas Heart® Institute, Houston.Öğe Surgical treatment of pulmonary hydatid cysts: Report of 139 cases(2006) Ülkü R.; Yilmaz H.G.; Onat S.; Özçelik C.Hydatid disease is a parasitic infestation that is endemic in many sheep- and cattle-raising areas and is still an important health problem in the world. The aim of our study was to present our surgical experience and strategy in the management of pulmonary hydatid disease. One hundred thirty-nine patients with pulmonary hydatid cysts underwent surgical treatment in our clinic between January 1990 and April 2002. A total of 169 cysts were present in 139 patients. Fifteen patients had concomitant hydatid cysts in liver and lung. Our series consisted of 76 male and 63 female patients, and mean age was 28.6 ± 17 years (range, 5-65 years). Only pulmonary hydatid cysts were seen in 123 patients (88.5%), whereas there were 15 patients (10.8%) with combined pulmonary and hepatic disease. Hydatid cyst was found in a patient both in lung and spleen (0.7%). Lateral thoracotomy was performed in 115 patients (82.73%), bilateral thoracotomy in 9 patients (6.48%), thoracophrenotomy in 11 patients (7.91%), and median sternotomy and phrenotomy in 4 patients (2.88%). Resection techniques were used only in 10 patients. The most common complication was atelectasis (5.0%). The mortality rate was 1.8% (3 patients). Surgery is the treatment of choice for most patients with pulmonary hydatid cysts. Thoracaphrenotomy can be chosen as the surgical procedure in management of hepatic and pulmonary hydatid cysts.Öğe Tracheobronchial injuries(2003) Ulkü R.; Eren M.N.; Eren S.; Onat S.; Yavuz C.BACKGROUND: Tracheobronchial injuries are among the least common sort of thoracic traumas. This injury can be life threatening. However, successful diagnosis and treatment can prevent devastating acute or delayed complications. The aim of this study was to review the authors' surgical experience in tracheobronchial injuries. METHODS: We reviewed our records of 29 patients with tracheobronchial injuries from January 1979 to December 2000. The median age was 22.7. Twenty-seven patients were male and two were females. RESULTS: The causes of traumas were penetrating injury in seventeen, blunt injury in eleven patients and animal bite in one. In sixteen patients cervical trachea, in nine main stem bronchus, in 3 lobar bronchus and in one patient mediastinal trachea were found to be injured. In seven of twenty-nine patients there were combined tracheal and esophageal injuries. Surgical approach was made by oblique incision, collar incisions and thoracotomy. In general we preferred primary repair. The overall mortality rate was 24%. CONCLUSION: In our opinion, the number of complications and mortality can be diminished by early recognition of the injuries and treatment.Öğe Traumatic sternal fracture: Three years experience of thoracic surgery department(2008) Ülkü R.; Avci A.; Onat S.; Sayhan M.B.; Özçelik C.Traumatic sternal fractures are becoming common injuries in developed and developing countries. We would like to report our three years experience with sternal fracture. Emergency room management and clinical examinations are reviewed in this study.
