    Diyarbakır'daki taziye evlerinde akut dönem yas sürecinin yaşanması ve yasın ağırlık derecesine etki eden faktörlerin belirlenmesi

    Amaç: Bu çalışmada, yakını öldükten sonraki ilk günlerde yas sürecinde bulunan kişilerin durumluk kaygı düzeyleri ve diğer yas tepkilerinin belirlenmesi, yakınlık derecesinin ve cinsiyet farklılığının yas tepkilerine etkisinin saptanması amaçlandı. Metod: Çalışma Diyarbakır ve çevresindeki yerleşim merkezlerinde kurumsal hale gelmiş olan taziye evlerinde yas yaşayan ve ölenin akrabası olan 122 birey (49 kadın, 73 erkek) ile yapıldı. Kontrol grubu olarak taziye dilekleri için gelen ve ölen kişi ile akrabalık bağı olmayan 54 birey (27 kadın, 27 erkek) alındı. Tüm katılımcılara çalışma hakkında bilgi verildi ve izinleri alındı. Katılımcılara sosyodemografik veri formu, durumluk kaygı envanteri ve yasta ortaya çıkabilecek olan duygusal-davranışsal tepkilerin bir listesi sunuldu. Bulgular: Birinci derece akrabalarda akut yas daha şiddetli yaşanmaktadır (F=13.976, p<0,001) ve durumsal kaygı puanları daha yüksektir (DKE ortalaması birinci derece akrabalarda 53.3±8.9; ikinci derece akrabalarda 44.1±9.6; üçüncü derece akrabalarda 43.9±11.6 idi). Ölümün ani olması yas şiddetini artırmaktadır; ani ölenlerin yakınlarındaki DKE puanı 51.8±8.9; ölümü beklenenlerin yakınlarındaki DKE puanı 45.8±11.4 bulundu (t=3.257, p<0.001). Çalışma grubundaki kadınlarda yas semptomları daha şiddetlidir; kadınlarda DKE puanı 53.7±8.2; erkeklerde 45.4±10.8 bulundu (t=4.571, p<0.001). Kontrol gurubu ile kıyaslandığında akrabalardaki psikofizyolojik tepkilerinin daha yoğun olduğu saptandı (Örneğin; ölenle ilgili yoğun düşünceler ve hayaller içinde bulunma belirtisi için z=5.009, p<0.001). Sonuç: Yas kültürden kültüre ve bireyden birey çeşitlilik gösteren bir süreçtir. Yakınlık derecesi artıkça ve ölüm beklenmedik ve ani olunca yaslı kişiler daha çok etkilenmekte, kaygı düzeyleri daha yüksek çıkmaktadır. Kadınlar yas sürecinde erkeklerden daha çok etkilenmektedir. Anahtar kelimeler: Akut yas, akrabalık, cinsiyet, durumluk kaygı envanteri Aim: In this study, determination of state anxiety levels and other mourning reactions of individuals who lost their close relatives in first few days of the lost and the effects of closeness degree and gender on mourning reactions were aimed. Method: The study was carried out on 122 subjects (49 female, 73 male) who were close relatives of a died person and mourning in the condolence houses which became an institution for mourning and were located in Diyarbakir and its settlement areas. Fifty-four individuals (27 female, 27 male), who were present at condolence house for condolence but not close relatives of death person, have been included into the study as controls. All the participants were informed about the study and their informed consents were taken. They were asked for filling a sociodemographic data from, state anxiety inventory and a list of emotional-behavioral reactions that may arise during mourning. Result: Acute mourning was observed to be more severely experienced in the first degree relatives (F=13. 976, p<0.001), and their state anxiety level was also higher (Mean SAI level in first degree relatives was 53.3 ±8,9; in second degree relatives was 44.1±9,6; in third degree relatives was 43.9±11,6). Sudden death increased the severity of mourning; mean SAI score in relatives of suddenly died person was 51.8±8.9, whereas, it was 45.8±11.4 in the relatives of individuals whose death was expected (t=3.257, p<0.001). Mourning symptoms were more severe among women in the study group. SAI score was 53.7±8.2 in women and 45.4±10.8 in males (t=4.571, p<0.001). Psycho-physiological reactions were observed to be more severe in close relatives of when compared to controls (e.g. having symptoms of intense thoughts and daydreams about died person z=5.009, p<0.001). Conclusion: Mourning is a process which varies from culture to culture and from individual to individual. Individuals in mourning are affected more and their anxiety levels become higher as their closeness degree increases to the lost person, and when it occurs unexpectedly and suddenly. In mourning process women are affected more negatively than men. Key words: Acute mourning, kinship, gender, state anxiety inventory

    Venlafaxine Addiction without a History of Alcohol and Substance Abuse: A Case Report

    Venlafaxine addiction without a history of alcohol and substance abuse: a case report In this paper, the aim was to present a patient who had no history of drug or alcohol abuse but was prescribed venlafaxine because of her complaints of headache and increased its dosage by herself on the pretext of a decrease in efficiency. A 35 year-old female patient complaining of headache was prescribed 75 mg/day venlafaxine by a psychiatrist, and the dosage was increased to 225 mg/day by the same doctor, and then the drug dosage was increased to 2100 mg/day by the patient herself. The patient was hospitalized and venlafaxine dosage was gradually decreased and stopped in 10 weeks. This case shows that antidepressant dependence may develop in patients without a history of drug or alcohol abuse.

    Venlafaxine dependency: A case report

    In this case report, the aim was to present a patient who is complaining of headache and started to venlafaxine and increased the dosage of this drug with a reason of decreasing its efficiency. A 35 year-old woman patient who applied to the doctor complaining of headache is started to 75 mg/day venlafaxine by psychiatrist, the dosage was increased to 225 mg/day by the same doctor and then the drug dosage was increased to 2100 mg/day by client self-decision. The venlafaxine dosage of patient, who was investigated by hospitalization with the diagnosis of venlafaxine addiction along 10 weeks, was gradually decreased and stopped. However, this detoxification period was hardly carried out due to limited mental capacity of the patient. This case shows that the antidepressant addiction is able to develop in patients who have not drug and alcohol usage history.
