Öğe Assessment of the neutrophil/lymphocyte ratio in patients with supraventricular tachycardia(Turkish Soc Cardiology, 2016) Aydin, Mesut; Yildiz, Abdulkadir; Yuksel, Murat; Polat, Nihat; Aktan, Adem; Islamoglu, YahyaObjective: The neutrophil/lymphocyte ratio (NLR) has been evaluated as a new predictor of cardiovascular risk. Inflammation has been shown to be associated with various arrhythmias including supraventricular tachycardias (SVTs). In this study, we aimed to investigate the relation between NLR and SVT in patients with a documented atrial tachyarrhythmia. Methods: The study used a retrospective cross-sectional design. Patients who had SVT but were otherwise healthy were included. The exclusion criteria included drug use (except antiarrhythmic agents), morbid obesity, acute or chronic infection, inflammatory diseases, systemic diseases, and cancer. Total and differential leukocyte counts and routine biochemical tests were performed before the ablation procedure. Results: The study included 150 patients with SVT and 98 healthy controls. The biochemical and hematological parameters were comparable between the groups, except neutrophil and lymphocyte counts. The neutrophil count was significantly higher (4.7 +/- 1.5x10(3)/mu L versus 4.1 +/- 1.0x10(3)/mu L; p<0.001) and lymphocyte count was significantly lower (2.2 +/- 0.6x10(3)/mu L versus 2.5 +/- 0.6x10(3)/mu L; p=0.001) in the SVT group than in the control group. As a result, the SVT group had significantly higher NLR values than the control group (2.2 +/- 0.9 versus 1.7 +/- 0.5; p<0.001). In addition, NLR values were higher in patients in whom tachycardia was induced during an electrophysiological study (EPS) (2.3 +/- 0.9 versus 2.0 +/- 0.8; p=0.02). The association between NLR and SVT remained significant after multivariate analysis (odds ratio: 1.5, 95% confidence interval: 1.001-2.263, p=0.049). Conclusion: Our study indicated that NLR values were significantly higher in patients with documented SVT than in control subjects. Inducibility of SVT during EPS was associated with higher NLR values.Öğe Association Between Neutrophil to Lymphocyte Ratio and Severity of Coronary Artery Disease(Sage Publications Inc, 2014) Kaya, Hasan; Ertas, Faruk; Islamoglu, Yahya; Kaya, Zekeriya; Atilgan, Zuhal Ariturk; Cil, Habib; Caliskan, AhmetThe aim of this study is to investigate the association between neutrophil to lymphocyte ratio (NLR) and severity of coronary atherosclerosis. A total of 172 patients undergoing coronary angiography were included in the study. Control group consisted of patients with normal coronary arteries. Patients with coronary stenosis were divided into 2 groups by use of Gensini scores. The NLR was higher in severe atherosclerosis group compared with mild atherosclerosis and control groups (P < .001). In correlation analysis, NLR showed significant correlation with Gensini score. A cutoff value of 2.5 for NLR predicted severe atherosclerosis with a sensitivity and specificity of 62% and 69%, respectively. After multivariate analysis, high levels of NLR were independent predictors of severe atherosclerosis together with glucose and high-density lipoprotein. Our study suggests that the NLR is a predictor of severe atherosclerosis that may be useful for cardiac risk stratification in patients with coronary artery disease.Öğe Coexistence of permanent junctional reciprocating tachycardia with rheumatic valvular disease; a case sucessfully treated with radiofrequency ablation(Aves Press Ltd, 2015) Aydin, Mesut; Yildiz, Abdulkadir; Yuksel, Murat; Islamoglu, YahyaA seventeen years old female had presented with palpitation. On the electrocardiography (ECG), long RP, narrow QRS tachycardia, inverted P waves in leads D2, D3 and aVF were noticed. On transthoracic echocardiography, rheumatic mitral valvular disease (RMVD) with normal ventricular systolic function was observed. Eccentric mitral regurgitation was also present. Electrophysiology study (EPS) was performed at our institution. According to EPS and surface ECG findings our diagnosis was permanent junctional reciprocating tachycardia (PJRT). Atrial activation appeared earliest and nearly ostium of the coronary sinus. The accessory pathway was detected near the coronary sinus ostium. RF ablation was performed, successfully. Tachycardia was terminated in three seconds. Permanent junctional reciprocating tachycardia can rarely coexist with RMVD. Here, we are reporting a case of PJRT and RMVD coexistence that underwent successful radiofrequency (RF) catheter ablation. This case report confirms that RF catheter ablation should be considered as the treatment of choice in adult patients with PJRT for the following reasons: catheter ablation is highly successful, the complication rate is low, and PJRT may lead to tachycardia induced cardiomyopathy, which is reversible.Öğe Effects of acute exercise on the diameter of the spermatic vein, and duration of reflux in patients with varicocele(Informa Healthcare, 2013) Atar, Murat; Soylemez, Haluk; Oguz, Fatih; Beytur, Alt; Altunoluk, Bulent; Kahraman, Bayram; Islamoglu, YahyaObjective. The aim of this study was to investigate the effects of acute exercise on the diameter of the spermatic vein, and on the duration of reflux in patients with varicocele. Material and methods. The study included 38 patients with complaints of infertility and scrotal pain between 2009 and 2010. The diagnoses were made by physical examination and colour Doppler ultrasound, with both performed before and after exercise tests. Results. The mean age of the participants was 25.7 +/- 4.9 years. During the first examination, the grades of the varicoceles detected were as follows: grade I, n = 7; II, n = 10; and III, n = 21. The diameters of veins in patients with grades I, II and III varicocele were 2.1 mm, 2.9 mm and 4.2 mm, respectively, before exercise, whereas they were 2.6 mm, 3.2 mm and 4.3 nun, respectively, after exercise. In patients with grade I varicoceles, compared with pre-exercise values, the diameter of the left spermatic vein and duration of reflux measured during Valsalva manoeuvres were increased significantly after exercise (p = 0.042 and p = 0.034, respectively); similar results were obtained for the patients with grade II varicoceles (p = 0.007 and p = 0.008, respectively). However, the minimal relative increase in cases with grade III varicoceles was not statistically significant (p > 0.05). Conclusions. This study demonstrates that acute exercise increases the spermatic vein diameter and reflux time in patients with varicoceles. These outcomes demonstrate that acute exercise may be an aggravating factor for varicocele, as seen in chronic exercise.Öğe Factors affecting mortality in penetrating cardiac injuries: our 10-year results(Baycinar Medical Publ-Baycinar Tibbi Yayincilik, 2011) Yavuz, Celal; Cil, Habib; Basyigit, Ismail; Demirtas, Sinan; Islamoglu, Yahya; Tekbas, Guven; Elbey, Mehmet AliBackground: In this study, we investigated the factors affecting survival in patients who were admitted to our center because of a penetrating heart injury and given treatment. Methods: Ninety-four patients with penetrating heart injuries (82 males, 12 females, mean age 25.9 +/- 12.7 years; range 3 to 67 years) admitted to our clinic between November 2000 and June 2010 were retrospectively evaluated. We recorded the demographic features, transfer time to hospital, way of transfer, clinical condition at admission, and reasons for the patient injuries. Additionally, the presence of hemothorax, pericardial hematoma, tamponade, coronary artery injury, the method of decision for surgery (operation following resuscitation thoracotomy, clinical follow-up or some diagnostic methods), the injured chamber of the heart, presence of accompanying injury in another organ, heart rhythm at the beginning of the surgery, and the surgical approach used were recorded. Results: The mortality rate was 13.8% in our study. Survival was not found to be related to age, gender, reason of injury (gunshot, stabbing, iatrogenic injuries), and presence of tamponade, and coronary artery injury. However, a significant relationship was found between survival and the way of transfer (ambulance or other vehicles), transfer time to hospital, clinical condition at admission, the method of decision for surgery, heart rhythm before the surgery, presence of pericardial hematoma or hemothorax, injured heart chamber, and presence of accompanying injury in another organ. Conclusion: The transfer time to hospital is the most important modifiable factor that may affect the outcomes in penetrating heart injuries. The study included 44 patients (23 women, 21 men; mean age 44 +/- 19 years; range 15 to 85 years) who were diagnosed as having definite IE, according to the modified Duke criteria, between June 2007 and June 2011. Data were reviewed on age, sex, underlying heart disease, echocardiographic and microbiological findings, treatment, complications, and mortality. Infective endocarditis developed on a native valve in 30 (72.7%), a mechanical prosthetic valve in 12 (27.3%). Prosthetic valves 12 (27.3%) were the most common preexisting valvular abnormality. The mitral valve was the most commonly affected valve in both native valves (50%) and prosthetic valves (66.6%). The most frequent symptom was fever (n=27, 61.4%). Electrocardiography showed abnormal findings in 22 cases (50%). Transthoracic and/or transesophageal echocardiography showed a vegetation in 41 cases (93.2%), and absce 3 cases (6.8%). Staphylococci (29.6%) and streptococci (25%) were the most common causative agents,and Brucella were 15.9%. Cultures were negative in 7 cases (15.9%). Nine patients (20.4%) underwent surgical treatment. Embolic events (n=6, 35.3%) were the major complications. In-hospital mortality occurred in 6 cases (13.6%). The data reflect epidemiological, clinical, and microbiological profile of IE in multiple tertiary hospital located in the South-eastern and Eastern Anatolia.Öğe Increased echocardiographic epicardial fat thickness is related to impaired diurnal blood pressure profiles(Taylor & Francis Ltd, 2012) Ertas, Faruk; Kaya, Hasan; Acet, Halit; Cil, Habib; Akyuz, Abdurrahman; Islamoglu, Yahya; Tekbas, EbruObjective. Epicardial fat has been proposed as a new cardiometabolic risk factor. Although epicardial fat thickness (EFT) is associated with hypertension, the relationship between diurnal blood pressure profiles and EFT is still unknown. The purpose of this study is to investigate the association between the echocardiographic EFT and diurnal blood pressure profiles in hypertensive patients. Methods. After the ambulatory blood pressures of 123 patients were monitored, they were divided into three groups according to the clinical diagnoses: 41 patients (33.3%) were in the normotensive group, 40 patients (32.5%) were in the dipper hypertensive group and 42 patients (34.1%) were in the non-dipper hypertensive group. All participants underwent transthoracic echocardiography and ambulatory blood pressure monitoring to measure the EFT and blood pressure responses. Results. The mean EFT measurements of the dipper group were significantly higher than the normotensive group (6.5 +/- 0.6 vs 5.8 +/- 0.6; p < 0.0001). On the other hand, the mean EFTs of the non-dipper group were also significantly higher than the dipper group (7.4 +/- 0.7 vs 6.5 +/- 0.6, p < 0.0001). An EFT of >= 7 mm predicted the non-dipper profile in hypertensive patients with 74% sensitivity and 71% specificity (receiving operator characteristic area under the curve: 0.826, 95% CI 0.738-0.913; p < 0.0001). EFT was associated with both dipper (OR 8.9, 95% CI 3.03-26.3; p < 0.0001) and non-dipper blood pressure profiles (OR 12.3, 95% CI 1.75-86.31; p < 0.0001), and this relationship was also independent from all the risk factors. Conclusion. Echocardiographic EFT assessment is independently associated with impaired diurnal blood pressure profiles in the hypertensive individuals. Thus, the echocardiographic assessment of the EFT may be helpful in cardiometabolic risk stratification and therapeutic interventions.Öğe Investigation of the Effects of Leptin on the Cardiovascular System in Patients With Systemic Sclerosis(Baycinar Medical Publ-Baycinar Tibbi Yayincilik, 2013) Atilgan, Zuhal Ariturk; Islamoglu, Yahya; Tekbas, Ebru; Budulgan, Mahmut; Batmaz, Ibrahim; Tahtasiz, Mehmet; Yuksel, HaticeObjective: Cardiac involvement in systemic sclerosis can be seen, and often remains silent. Leptin, which is often associated with hypertension and regulation of sympathetic tone, has been reported to contribute to the development of atherosclerosis, endothelial dysfunction and thrombosis by acting directly. The aim of this study was to investigate the effects of leptin on the cardiovascular system in patients with systemic sclerosis. Materials and Methods: Twenty-seven patients with systemic sclerosis and 28 healthy subjects as a control group were included in the study. Clinical and laboratory parameters, 24-hour Holter ECG and ambulatory blood pressure monitorings were recorded and serum leptin levels were measured in all subjects. Results: Mean leptin levels were lower in patients with systemic sclerosis than in controls. In 24-hour ambulatory blood pressure Holter recordings, the mean systolic and diastolic blood pressure, daytime systolic and diastolic blood pressure, systolic and diastolic blood pressure values at night were lower in patients when compared with controls. There was no relationship between leptin and heart rate variability parameters. A positive correlation was found between leptin levels and 24-hour ambulatory blood pressure parameters. Conclusion: Decreased serum leptin levels and increased mean heart rate in patients with systemic sclerosis may be the result of increased sympathetic tone.Öğe Left ventricle hydatid cyst mimicking acute coronary syndrome(J Infection Developing Countries, 2012) Tekbas, Ebru Onturk; Tekbas, Guven; Atilgan, Zuhal Ariturk; Islamoglu, Yahya; Cil, Habib; Yazici, MehmetCardiac echinococcosis rarely mimics acute coronary syndrome. The diagnosis of cardiac hydatid cyst might be difficult on account of varying clinical presentations and nonspesific symptoms. A 75-year-old female was admitted to our hospital with typical chest pain. The patient had no history of previous cardiac symptoms or any illness leading to heart disease. Her ECG revealed ischemic changes. However, her coronary angiography revealed noncritical plaques in the left anterior descending artery. The diagnosis of cardiac echinococcosis was identified using echocardiography, computed tomography and magnetic resonance imaging. The patient was referred to cardiac surgery for resection of the cyst; however, she refused surgery. An increased MPV is associated with acute myocardial infarction (AMI) and long-term mortality. The aim of this study was to compare MPV in patients with ST-segment elevation myocardial infarction (STEMI) and non-ST-segment elevation myocardial infarction (NSTEMI). Also, we investigated the value of MPV on in-hospital mortality and long-term prognosis of patients with STEMI and NSTEMI. We studied 429 patients with AMI (70.4% male, 61.9 +/- 12.4 years; 279 patients with STEMI, 150 patients with NSTEMI). MPV and platelet count were similar in both groups. Elevated MPV increased the risk of death by 3.1-fold (p < 0.001) in STEMI group during the hospitalization. However, increased MPV was not associated with in-hospital mortality in NSTEMI group. The area under the receiver operating characteristic curve of MPV was 0.868 (95% CI, 0.830-0.907) for predicting two-year mortality. A cut-off point of 11.1 fL showed a sensitivity of 81% and a specifity of 77% for prediction of two-year mortality. Kaplan-Meier survival curve showed two-year mortality rate of 12.5% in patients with MPV > 11.1 fL versus 9.9% in patients with MPV < 11.1 fL (p < 0.001). Cox regression analysis showed MPV to be an independent predictor of two-year mortality (Hazard ratio 1.7; 95% CI 1.5-1.9; p < 0.001). An increased MPV is an independent predictor of in-hospital mortality in patients with STEMI. However, elevated levels of MPV did not predict in hospital mortality in NSTEMI group. A clear and odorless liquid, hydrogen peroxide is an oxidizing agent found in most households and many industrial environments. Cardiovascular manifestations of hydrogen peroxide ingestion are extremely rare. Here we report a 60 year-old woman with acute inferolateral myocardial infarction (ML) after hydrogen peroxide ingestion, who had no history of coronary artery disease. Physicians dealing with hydrogen peroxide ingestion in the emergency department should be aware of the probability of MI and obtain an electrocardiogram, even if the patient has no cardiac complaint. The most common cause of death in these cases is cardiac arrest. In addition, deaths can occur due to injury of the respiratory center. Material and Methods: In the literature, there are many case reports regarding lightning injuries. Howewer, there are only a few studies on the effects of lightning strike on the heart. In this paper, we specifically investigate the noninvasive electrophysiological effects of lightning strike on the heart. Results: Ventricular fibrillation, asystole, supraventricular tachycardia, QT prolongation and nonspecific ST-T changes may occur in patients with lightning injury. However, there is no typical electrocardiogram (ECG) sign/signs of lightning injury. Conclusion: Electrocardiographic changes due to lightning strike depend on the severity and duration of exposure of the body.Öğe Platelet Count and Mean Platelet Volume in Patients With In-Hospital Deep Venous Thrombosis(Sage Publications Inc, 2012) Cil, Habib; Yavuz, Celal; Islamoglu, Yahya; Tekbas, Ebru Onturk; Demirtas, Sinan; Atilgan, Zuhal Ariturk; Gunduz, ErcanAim: To investigate the relationship between mean platelet volume (MPV) and in-hospital deep venous thrombosis (DVT). Material and methods: 147 patients with the diagnosis of DVT and 149 control participants were included in the study. For all participants, clinical risk factors, smoking status, and other demographic data were recorded from hospital registries. The data of patients with DVT were compared with the control participants. Results: Mean MPV was significantly higher in patients with DVT than the control group (8.91 +/- 1.86 vs 7.86 +/- 0.9; P < .001). Body mass index, smoking frequency, hematocrit, and platelet count were significantly correlated with MPV. Independent predictors of in-hospital DVT were MPV (odds ratio [OR] = 1.5; 95% confidence interval [CI] = 1.2-1.87; P <= .001), body mass index (OR = 1.17; 95% CI = 1.04-1.34; P = .012), and smoking (OR = 1.83; 95% CI = 1.09-3.08; P = .023). Palmar collateral circulation locates between ulnar and radial arteries, and its adequacy can be evaluated by noninvasive tests. We aimed in this study to investigate relationship between coronary collateral circulation and palmar collateral circulation. 96 patient, detected in coronary arteries at least 70% stenosis, were included the study. Coronary collateral circulation was evaluated according to rentrop classification as adequate and inadequate collateral circulation. Hand circulations of the patients were evaluated with Allen Test and Modified Allen Test. Incomplete palmar arch were detected in patients with adequate and inadequate coronary collateral circulation 22% and 29%, respectively. There was no significant different between two groups according to presence of incomplete or complete palmar circulation. In our study, we demonstrated that there was no relationship between coronary collateral circulation and palmar collateral circulation.Öğe Relationship Between Mean Platelet Volume and Varicocele: A Preliminary Study(Elsevier Science Inc, 2012) Bozkurt, Yasar; Soylemez, Haluk; Sancaktutar, Ahmet Ali; Islamoglu, Yahya; Kar, Aytan; Penbegül, Necmettin; Atar, MuratOBJECTIVE To examine the relationship between the mean platelet volume (MPV) and varicocele to clarify the etiopathogenesis of varicocele. METHODS MPV levels were measured in 60 varicocele subjects (group I) and 57 healthy controls (group II) from 2009-2010. Ages were similar between the 2 groups. The diagnosis of varicocele was based on the findings from both physical examination and color Doppler ultrasound. RESULTS The average patient age at the time of examination was 28.12 +/- 6.4 years in group I and 28.10 +/- 5.8 in group II. MPV levels were statistically significantly higher in group I than in group II (P < .001). Positive correlations were found between MPV and varicocele grade as between MPV and diameter of left spermatic vein (P < .05). CONCLUSION The increase in MPV is independent of the disease, and the increase in varicocele grade is associated with higher MPV in varicocele patients. On the basis of the importance of the vascular component in the pathophysiology of varicocele, we would suggest conducting more studies with platelets in varicocele patients. Several research groups have demonstrated that they also play important roles in states of cardiovascular diseases. Our aim is to investigate whether there is a relationship between trace elements (Zn and Cu) and the degree of atherosclerosis. The sample consisted of 67 patients with coronary artery disease and 26 clinically healthy individuals. Ninety-three subjects were separated into four groups according to their Gensini scores, the number of diseased vessels, the presence of acute coronary syndrome, and ejection fraction. Each group was divided into three subgroups, and serum zinc and copper levels were measured for each individual. The serum levels of zinc and copper were found to be significantly lower in patients with atherosclerosis than in the control group, but there were no significant differences in the serum levels of Cu and Zn between severe atherosclerosis and mild atherosclerosis. In Spearman's rank correlation, the zinc and copper levels were correlated with the Gensini score and the number of diseased vessels. The present study revealed a relationship between the serum levels of zinc and copper and atherosclerosis, but not between these levels and the severity of the disease.Öğe Resolution of Intracoronary Thrombus with Tirofiban Infusion: A Case Report(Aves Press Ltd, 2010) Ariturk, Zuhal; Tekbas, Ebru; Cil, Habib; Islamoglu, Yahya; Elbey, M. Coronary angiography revealed total occlusion of the left anterior descending coronary artery. Tirofiban infusion was administered for 48 hours. Then, coronary angiography showed intraluminal filling defects due to a massive thrombus in the proximal segment of the left anterior descending coronary artery.
