Öğe Concurrence of prenatally diagnosed Cantrell pentalogy and short and tight umbilical cord: A possible case of hexalogy(Yuzuncu Yil Universitesi Tip Fakultesi, 2016) Batur A.; Goya C.; Yavuz A.; Hamidi C.; Cetincakmak M.G.Cantrell pentalogy is a rather rare syndrome characterised by complete or partial ectopia cordis; together with omphalocele, congenital heart diseases, and defects in the pericardium, lower sternum and anterior diaphragm. In this article, our aim is to present the ultrasonographic evidence of the concurrence of a short umbilical cord and the Cantrell syndrome, which we believe will contribute to the literature. © 2016 Yuzuncu Yil Universitesi Tip Fakultesi. Methods The study included 27 pregnant patients with unilateral symptomatic persistent hydronephrosis (group 1) and 38 pregnant patients with physiological hydronephrosis of pregnancy (group 2). All pregnant patients underwent Doppler Ultrasonography to determine the Resistive Index (RI) and the difference between the RI of the corresponding and contralateral kidney (Delta Resistive Index = ?RI). Results There were no statistical differences between the two groups in terms of age, mean gestational period, or number of pregnancies. The mean renal RI of the hydronephrosis side was 0.68±0.05 in group 1 and 0.60±0.05 in group 2 (p<0.001). The mean ?RI of group 1 was significantly higher than the mean ?RI of group 2 (0.07±0.03 versus 0.02±0.01, respectively, p<0.001). The RI and ?RI were considered positive with values of ?0.70 and ?0.04, respectively, and the RI was sensitive in 44.4% and specific in 92.1% for intervention treatment. The corresponding values for ?RI were 88.9% and 89.5%. The positive predictive value and negative predictive value of ?RI for intervention were 85.7% and 91.9%, respectively. Conclusion It is much better to consider ?RI than RI when deciding on interventional treatment in hydronephrosis during pregnancy.Öğe Investigation of origin and trunc variations of coeliac trunc, superior mesenterik artery, and inferior mesenterik artery by multidetector computed tomography(Turkiye Klinikleri, 2020) Ekingen A.; Hatipoğlu E.S.; Hamidi C.Objective: In our study, it was aimed to determine origin and trunc variations of the celiac trunc (CT), the superior mesen-terik artery (SMA), and the inferior mesenterik artery (IMA) by computed tomography, these arteries are the most important unpaired visceral branches of the abdominal aorta (AA) and provide blood supply to most of the abdominal structures and organs. Material and Methods: Screening the datas of 261 patients (115 female, 146 male) who were obtained by multi detector computerized tomography angiography examination at the Radiology Department of Dicle University Medical Faculty was retrospectively examined. 16-83 age range were included in the study. The quantitative data of origin and truncus types obtained in the study were evaluated statistically in the SPSS program. The origin and truncus variations detected in the CT, SMA, and IMA were classified. Results: It was found that the branch shown the most variation is the CT (31.03%), the branch shown the least variation is the IMA (7.84%) among the CT, SMA and IMA. The origin and truncus variations were identified that seven different related to the CT, six different related to the SMA, and three different related to the IMA. The celiacomesenteric trunk in 1.15%, the bime-senteric trunk in 1.90%, the pancreaticoduodenomesenterik trunk in 0.39% rate were found. Conclusion: There can be quite different variation types in humans than the types described in the classical anatomy books on the origin and truncus structure of CT, SMA and IMA. More detailed research is needed on this subject. © 2020 by Türkiye Klinikleri. Material and methods: Forty six postmenopausal women diagnosed with RA according to the criteria of American College of Rheumatology and 42 postmenopausal healthy women who have similar age range with the patient group were involved into the study. Disease activity was measured with Disease Activity Score (DAS 28). Furthermore, Larsen score was used to evaluate radiological damage. Results: OC and BALP levels were not different between postmenopausal patients with RA and control group (p >0.05). However, levels of cathepsin K were elevated in serum of the patients with postmenopausal RA when compared with that of the healthy control group (p<0.05). In addition, the elevated serum levels of cathepsin K were positively correlated with disease activity and joint destruction (p<0.05). No significant relationship was observed between OC and BALP levels and disease activity and joint destruction (p >0.05). Conclusions: Cathepsin K may be a valuable parameter to assess disease prognosis in postmenopausal RA.
