Öğe Abdominoplastide gerginlik azaltıcı dikişlerin kullanımındaki deneyimlerimiz(2012) Güler, İlgi; Eryılmaz, Tolga; İnözü, Emre; Özakpınar, Hülda Rıfat; Durgun, Mustafa; Tellioğlu, Ali TeomanAmaç: Abdominoplasti sonrası en sık görülen lokal komplikasyonlar, flep nekrozu, hipertrofik skar gelişimi, seroma ve hematom oluşumudur. Gerginlik azaltıcı dikişler, abdominoplasti sonrası lokal komplikasyonların azaltılması için tanımlanmıştır. Bu çalışmada, abdominoplasti uygulamalarında gerginlik azaltıcı dikişlerin kullanımı ile ilgili deneyimlerimizi aktarmayı amaçladık. Yöntemler: Çalışmaya abdominoplasti ameliyatı uygulanan 19 kadın hasta dahil edildi. Hastaların ortalama yaşı 42 (35-59) idi. Tüm olgularda, abdominoplasti flebi ilerletilirken derin ve yüzeysel fasya arasına gerginlik azaltıcı dikişler uygulandı. Bulgular: Ortalama takip suresi 11 (6-26) aydı. Hastaların hiçbirinde hipertrofik skar, flep nekrozu, enfeksiyon, seroma veya hematom gelişmedi. Bir hastada (%5) orta hatta lokalize minimal yağ nekrozu, bir başka hastada (%5) ise pulmoner emboli meydana geldi. Her iki hasta da sorunsuz olarak iyileşti. Sonuç: Gerginlik azaltıcı dikişler, flep distalinde oluşabilecek iskemiyi azaltarak flep nekrozu ve hipertrofik skar oluşumunu azaltmaktadır. Ayrıca olu boşluk miktarını azaltarak, seroma veya hematom oluşma riskini azaltmaktadır. Sonuç olarak, ameliyatın normal suresinde bir miktar uzamaya neden olsa da, abdominoplasti olgularında lokal komplikasyonları azaltmak için kullanılabilecek bir yöntemdir. In this report, we present the surgery of delayed anterosuperior dislocation of intact mandibular condyle into the temporal fossa. A 17-year-old adolescent girl with anterosuperior dislocation of left intact condyle and associated mandible and zygomatic body fracture underwent surgery 33 days after trauma because of priority accompanied life-threatening intracranial and intra-abdominal injuries. Temporalis muscle shortening with soft tissue attachment prevented closed reduction of the temporomandibular joint; therefore, the temporalis muscle was divided from its insertion on coronoid in a process that, to our knowledge, has never before been described. Although good occlusion was achieved at operation, open bite deformity and laterognathism were observed at the sixth month of follow-up.Öğe Bazal hücreli karsinom: 5 yıllık deneyim(2012) Durgun, Mustafa; Horoz, Uğur; İnözü, Emre; Tellioğlu, Ali Teoman; Eryılmaz, Avni Tolga; Arpacı, EnverAmaç: Bazal hücreli karsinom en sık görülen kanser türüdür. En sık pilosebase bezlerin yoğun olduğu ve güneşe maruziyetin belirgin olduğu baş-boyun bölgesinde görülür. Yavaş büyür, lokal invazyon gösterir ve uzak metastazı nadirdir. Cerrahi olarak tümörün çıkartılması ve histolojik olarak cerrahi sınırların temiz olduğunun gösterilmesi ile tedavi sağlanır. Yöntem ve Gereçler: Kliniğimizde 2006 - 2011 yılları arasında bazal hücreli karsinom nedeniyle cerrahi tedavi uygulanan 436 vaka retrospektif olarak değerlendirilmiştir. Hastaların yaşı, cinsiyeti ile tümörün boyutu, anatomik lokalizasyonu, histolojik alt tipi kaydedildi. Cerrahi sınır pozitifliği ve nüks oranları belirlendi. Bulgular: Hastaların ortalama yaşı 58,4 idi. Lezyonların büyük kısmı yüz bölgesinde (%82,6) lokalizeydi. En sık nodüler tip görülürken, en fazla nüks infiltratif tipte izlendi. Histopatolojik inceleme sonucunda 29 hastada (%6,7) cerrahi sınır pozitifliği görüldü. Ortalama 30,6 ay takip edilen hastaların 13’ünde (%3,4) nüks, 6’sında (%1,6) yeni oluşan bir bazal hücreli karsinom izlendi. Sonuç: Bazal hücreli karsinomların cerrahi tedavisinin başarısında eksizyon sınırlarının doğru olarak belirlenmesi önemlidir. Bunun için risk faktörleri iyi değerlendirilmelidir. Histolojik tip cerrahın tedavi yaklaşımını, dolayısıyla olası nüks oranını etkilemektedir.Öğe Cartilage-Supported Paramedian Forehead Flaps for Reconstruction of Full-Thickness Nasal Defects(Lippincott Williams & Wilkins, 2013) Selcuk, Caferi Tayyar; Durgun, Mustafa; Ozalp, Burhan; Bozkurt, MehmetThe nose is an important esthetic structure of the human face, and its defects may lead to serious esthetic and functional losses. In the current study, our aim was to present the results of the repairs that we performed using cartilage-supported forehead flaps in patients with full-thickness nasal defects in various locations. Between March 2009 and March 2012, a total of 6 patients with full-thickness nasal defects underwent repairs using cartilage-supported forehead flaps. Among the patients, 4 were women, whereas 2 were male. The median age of the patients was 60.3 (38-67) years. The defects were caused by either tumor excision or burn wounds. Defect areas were at the dorsum of the nose, the medial canthal area, and the distal half of the nose. The repairs in all patients were performed using cartilage-supported median forehead flaps. In the method we applied, to provide framework support, a cartilage graft was placed into the pouch formed between the frontal muscle and the subcutaneous tissue within the flaps to be adapted to the defect area. The patients were followed up for a median period of 11 (4-27) months. In the long-term follow-up of all patients, satisfactory results in terms of the functional and cosmetic outcomes were obtained. Through the technique we applied, a simple and safe repair with adequate framework support and full color and texture harmony is achieved.Öğe The chiral boronate-catalyzed asymmetric transfer hydrogenation of various aromatic ketones to high-value alcohols: Preparation and spectroscopic studies(Elsevier Science Sa, 2019) Kilic, Ahmet; Durgun, Mustafa; Durap, Feyyaz; Aydemir, MuratThis work deals with the synthesis, spectroscopic studies and catalytic evaluation of the novel chiral salen (L1H2) and (L2H2) ligands and their chiral boronate [L-1(B1-4)] and [L-2(B1-4)] complexes. Initially, the reaction of 5-azidomethyl salicylaldehyde and (R)-(-)-2-amino-1-butanol in absolute ethanol afforded a new chiral salen ligand (L1H2). Then, a novel chiral salen ligand (L2H2) have been prepared from chiral salen ligand (L1H2) for the synthesis of boronate [L-2(B1-4)] complexes through click reaction approach under ambient conditions. The reaction of chiral salen (L1H2) and (L2H2) ligands with various boronic acids afforded a new tetra-coordinated mononuclear chiral boronate [L-1(B1-4)] and [L-2(B1-4)] complexes. All the compounds are remarkably stable crystalline solids and were obtained in good yields. For the full characterization of newly synthesized chiral salen ligands and their boronate complexes, the FT-IR, UV-Vis, NMR (H-1, C-13, and B-11), LC-MS, and elemental analysis techniques have been used. The well-shaped chiral boronate compounds were investigated as catalyst for the asymmetric transfer hydrogenation (ATH) of aromatic ketones under appropriate settings. Particularly, it was proved that the ferrocene-based boronate compounds can afford an efficient catalytic conversion compared to the other boronate complexes in the asymmetric transfer hydrogenation catalytic studies. All rights reserved.Öğe Comparison of the antibacterial effect of silver sulfadiazine 1%, mupirocin 2%, Acticoat and octenidine dihydrochloride in a full-thickness rat burn model contaminated with multi drug resistant Acinetobacter baumannii(Elsevier Sci Ltd, 2012) Selcuk, Caferi Tayyar; Durgun, Mustafa; Ozalp, Burhan; Tekin, Alicem; Tekin, Recep; Akcay, Cemal; Alabalik, UlaIn this study, our aim is to compare the efficacy of different topical antibacterial agents in a rat model contaminated with a multi drug resistant (MDR) standard Acinetobacter baumannii strain. The study was carried out on 40 Sprague-Dawley rats of 250-300 g each. For the purposes of this study, the rats were divided into 5 groups, with 8 rats in each group: Group 1 control; Group 2 silver sulfadiazine; Group 3 mupirocin; Group 4 Acticoat group; and Group 5 octenidine dihydrochloride group. Following to the formation of the full-thickness burn areas in rats, the MDR A. baumannii standard strain was inoculated into the burned area. The rats in all the groups were sacrificed at the end of the 10th day and subjected to histopathological and microbiological evaluation. In the histopathological evaluation, the lowest inflammatory cell response and bacterial density in the eschar and muscle tissues were observed in the Acticoat group. While these results were found to be statistically significant compared to the silver sulfadiazine group, only the bacterial density in the muscle tissue was found as significant in comparison to the mupirocin and octenidine groups. In the microbiological evaluation, the lowest growth in the muscle tissue culture among all the groups was observed in the Acticoat group. The growth in the eschar tissue culture was significantly lower in the Acticoat and octenidine groups in comparison to the silver sulfadiazine group. At the end of the study, it has been observed that Acticoat was effective both in eschar and muscle, while octenidine was effective in eschar tissues in a rat burn model contaminated with MDR A. baumannii. In this study 14 patients operated because of dermatofibrosarcoma protuberans has been evaluated for sufficient surgical border and reccurence. Material and Methods: Patiens included in this study were operated between January 2007-October 2012 in Diskapi Yildirim Beyazit Training and Research Hospital Department of Plastic Reconstructive and Aesthetic Surgery and Dicle University Plastic Reconstrucitve and Aesthetic Surgery diagnosed with dermatofibrosarcoma protuberans. Patients evaluated for age, sex, reconstructive method and postoperative follow duration. Lesions excised widely including 4 cm of intact skin border and fasia below. Results: Among 14 patients who were included 6 were male 8 were female. Age of patients varied between 18-89 with an average of 45 years. Lesions were on trunk in 9 patients (%64), on upper extremity in 3 patients (%21), and on lower extremity in 2 patients (%15). Average follow up duration were 21 months. A patient with a lesion on wrist recieved radiotherapy additional to surgery. No reccurence observed in any patient. Conclusions: In our study, a wide excision of skin with 4 cm can provide histopathologically negative border.Öğe Different Hemi-Salen/Salan Ligand Containing Binuclear Boron-Fluoride Complexes: Synthesis, Spectroscopy, Fluorescence Properties, and Catalysis(Taylor & Francis Ltd, 2019) Kilic, Ahmet; Ozbahceci, Orcun; Durgun, Mustafa; Aydemir, MuratA family of hemi-salen (L1H-L6H) and hemi-salan (L1aH-L2aH) ligands-based N,O-chelated binuclear boron-fluoride [L-n(BF2)(2)] (n = L-1-L-6 or L-1a-L-2a) complexes have been prepared and characterized by a variety of spectroscopic techniques (H-1, C-13 and F-19 NMR, FT-IR, UV-Vis, LC-MS, and fluorescence spectra) and elemental analysis. All of the binuclear boron-fluoride complexes exhibit strong absorption bands due to S-0 -> S-1 transitions and strong fluorescence properties were observed at room temperature in the solution. The binuclear boron complexes containing two naphthyl groups are significantly red-shifted in comparison with the other binuclear boron-fluoride complexes. After the structures are characterized, these hemi-salen and salan ligand-based N, O-chelated binuclear boron-fluoride complexes were utilized to the transfer hydrogenation of the different acetophenone derivatives conversion to 1-phenylethanol derivatives as catalysts.Öğe Effect of 2100 MHz mobile phone radiation on healing of mandibular fractures: an experimental study in rabbits(Taylor & Francis Ltd, 2016) Durgun, Mustafa; Dasdag, Suleyman; Erbatur, Serkan; Yegin, Korkut; Durgun, Suna Ogucu; Uzun, Cosar; Ogucu, GolgeFractures in the maxillofacial region are among the most frequent incidences in patients exposed to trauma. The aim of this study was to evaluate the effect of 2100MHz electromagnetic radiation on the healing of mandibular fractures in rabbits. The study was carried out on 12 mature male New Zealand rabbits. After producing a left mandibular osteotomy, the fracture was repaired by using a plate screw. The rabbits were divided into two groups - control and experimental. The experimental group was exposed for 3h daily to a 2100MHz radiofrequency (RF) for 28 d. Biomechanical, histopathological and radiological tests were performed during the healing of the mandibular bone. According to the biomechanical tests, the energy absorption capacity, toughness and maximum strength of the mandibular bone were increased compared to the control group (6.84vs. 2.57, 0.135vs. 0.041 and 22.21vs. 10.32, respectively). No significant differences between the two groups were found during the radiological assessment. However, the histopathological examination revealed that the fracture healing score was higher in the experimental group when compared to the control group (8.0vs. 2.75, respectively). In conclusion, the results of this study showed that a 2100MHz RF radiation positively affected fracture healing.Öğe The Effect of Hyperbaric Oxygen Treatment on the Healing of Burn Wounds in Nicotinized and Nonnicotinized Rats(Oxford Univ Press, 2013) Selcuk, Caferi Tayyar; Ozalp, Burhan; Durgun, Mustafa; Tekin, Alicem; Akkoc, Mehmet Fatih; Alabalik, Ulas; Ilgezdi, SavasThe importance of oxygen in wound healing and the negative effects of cigarette smoking have been demonstrated in various studies. In this study, our aim was to investigate the effect of hyperbaric oxygen (HBO2) treatment on wound healing in nicotinized and nonnicotinized rats. The study was conducted on 32 Sprague Dawley rats. The rats were divided into four groups, with eight rats in each: group 1, nonnicotinized rats; group 2, nonnicotinized rats treated with HBO2; group 3, nicotinized rats; and group 4, nicotinized rats treated with HBO2. To prepare the nicotinized groups, the rats were given nicotine for 28 days. At the end of day 28, standard, deep, second-degree to third-degree burns were created on the rats. The HBO2-treated groups underwent HBO2 treatment once a day for 7 days after the creation of the burn damage. All rats were killed 21 days after injury, and the burns were subjected to macroscopic, histopathological, and microbiological evaluation. During this evaluation, the smallest necrotic areas and the lowest rate of fibrosis were observed in group 2. The largest necrotic areas and the highest inflammation and fibrosis rates were observed in the nicotine-treated group 3. When the nicotinized and nonnicotinized groups were compared separately, there was a significant difference in favor of the groups treated with HBO2. Bacterial growth was the highest in the nicotinized group 3, whereas no statistically significant difference was observed among the other groups. We conclude that HBO treatment accelerates the recovery of burn wounds and provides more effective healing by reducing the development of scars both in nicotinized and nonnicotinized rats.Öğe Evaluation of the Effect of Thymoquinone Treatment on Wound Healing in a Rat Burn Model(Lippincott Williams & Wilkins, 2013) Selcuk, Caferi Tayyar; Durgun, Mustafa; Tekin, Recep; Yolbas, Iyas; Bozkurt, Mehmet; Akcay, Cemal; Alabalik, UlasThymoquinone (TQ) is a plant extract that has been shown to have antimicrobial, anti-inflammatory, and antioxidant effects. Because of these activities, the authors hypothesized that TQ would reduce inflammation and oxidative stress and accelerate wound closure in a rat model of deep second-degree burns. For the purposes of this study, 40 Sprague-Dawley rats were divided into five groups of eight rats each. Group 1 was the control group, group 2 was the silver sulfadiazine group, group 3 was treated with systemic TQ, group 4 received topical TQ, and group 5 was administered topical and systemic TQ. After the deep second-degree burn damage was created, daily dressing changes and TQ administration were continued in the study groups for a period of 21 days. Systemic TQ was administered intraperitoneally at a dose of 2 mg/kg/day, whereas the topical treatment was applied using a 0.5% solution. The changes in the wound site were observed macroscopically, histopathologically, microbiologically, and biochemically in all groups. The smallest necrotic areas were observed at the end of the study in the groups that were administered a combination of systemic and topical TQ, or solely topical TQ (6.1 +/- 1.6 cm(2) and 6.7 +/- 0.4 cm(2), respectively), whereas the largest necrotic areas were observed in the control group (11.2 +/- 1.2cm(2)). The total antioxidant state levels in the control group were significantly lower than in the other groups (P < .05), whereas the total oxidative stress levels were lower in the TQ groups compared with the control group (P < .05). The lowest bacterial counts were observed in the groups treated with both topical and systemic TQ (P < .05). TQ given systemically and/or topically reduced inflammation and oxidative stress and accelerated the rate of wound closure or reepithelialization.Öğe Fluorine/phenyl chelated boron complexes: Synthesis, fluorescence properties and catalyst for transfer hydrogenation of aromatic ketones(Elsevier Science Sa, 2014) Kilic, Ahmet; Aydemir, Murat; Durgun, Mustafa; Meric, Nermin; Ocak, Yusuf Selim; Keles, Armagan; Temel, HamdiThe synthesis of salen/salan ligands (L-1 and L-2) and their fluorine/phenyl chelated boron complexes [L(1,2)BF2] or [L(1,2)BPh2] is described in this paper. The fluorine/phenyl chelated boron complexes were synthesized from the reaction of BF3 center dot OEt2 or BPh3 with the corresponding ligands in different solvent. The boron complexes display high stability and can be handled in air due to the presence of coordinative B <- N and covalent B-O bonds in their structures. The salen/salan ligands (designated as salan, a saturated version of the corresponding salen ligands) and their fluorine/phenyl chelated boron complexes have been characterized by H-1, C-13 NMR and F-19 NMR spectra, elemental analysis, FT-IR spectra, UV-vis spectra, LC-MS spectra, melting point and fluorescence spectroscopy. The fluorescence efficiencies of BF2-chelate boron complexes are greatly improved compared to those of the BPh2-chelate boron analogs based on the same salen/salan ligands, probably due to the enhanced conjugation degree of the diphenyl boron chelation, which can effectively prevent molecular aggregation. The boron complexes [L(1,2)BF2] or [L(1,2)BPh2] were also applied to the transfer hydrogenation of aromatic ketones to the corresponding alcohol derivatives in the presence of iso-PrOH as the hydrogen source. Catalytic studies showed that all complexes are good catalytic precursors for transfer hydrogenation of aryl alkyl ketones in 0.1 M iso-PrOH solution. This transfer hydrogenation is characterized by low reversibility under these conditions. (C) 2014 Elsevier B.V. Hastalar ve Yöntemler: Ocak 2006 - Mart 2011 tarihleri arasında Fujimori gate flep ile rekonstrükte edilmiş 19 hasta (7 kadın, 12 erkek; ort. yaş 60.3 yıl; dağılım 41-79 yıl) bu retrospektif çalışmaya alındı. Tüm hastaların demografik özellikleri ve ameliyat sonrası uzun dönem fonksiyonel ve estetik sonuçları incelendi. Bulgular: Ortalama defekt boyutu 34.2 mm (dağılım, 10-60 mm) idi. Toplam 27 flep eleve edildi. Ameliyattan sonra hiçbir komplikasyon izlenmedi. Dokuz hastaya uzun dönemde minör revizyon uygulandı. Hiçbir hastada lokal nüks meydana gelmedi. Sonuç: Fujimori gate flep alt dudak tümörlerinin cerrahisi sonrası gelişen doku kayıplarının rekonstrüksiyonunda kullanı- labilir.Öğe Geriatrik hastalarda bası yaralarına yaklaşım(Dicle Üniversitesi Tıp Fakültesi, 2012) İnözü, Emre; Özakpınar, Hülda Rıfat; Durgun, Mustafa; Eryılmaz, Avni Tolga; Selçuk, Caferi Tayyar; Tellioğlu, Ali TeomanAmaç: Bu çalışmanın amacı bası yarası nedeniyle kabul edilen geriatrik hastalara gerekli beslenme desteği sağlandıktan sonra planlanan tedavi ve sonuçlarını değerlendirmektir. Gereç ve yöntem: Bu çalışmaya 2006 ile 2011 yılları arasında bası yarası nedeniyle servisimize kabul edilen geriatrik hastalar dahil edildi. Hastaların vücut kitle indeksleri hesaplandı, albumin değerleri kaydedildi. Hastalarda protein-enerji malnütrisyonu değerlendirilerek gerekli beslenme desteği verildi. Malnütrisyonu düzelen hastalarda tedaviye başlandı. Bulgular: Kliniğe kabul edilen hastaların ortalama albumin değerleri 2,53 ± 0,25 g/dL iken beslenme desteği sonrası 3,95 ± 0,42 g/dL idi. Hastaların %75’inin genel durumu uygun olduğunda bası yaraları cerrahi olarak onarıldı, kalan %25’i ise eşlik eden sistemik sorunlarına bağlı olarak yüksek riskli oldukları için ameliyat edilmedi ve konservatif yöntemlerle takip edildi. Ameliyat sonrası ortalama taburculuk süresi 12 (8-21) gündü. Gerek cerrahi, gerekse konservatif yöntemler kullanılarak hastaların %78,6’sının bası yaraları başarı ile tedavi edildi. Sonuç: Geriatrik hastalarda bası yarasının tedavi başarısı, eşlik eden malnutrisyon gibi patolojilerin beslenme desteğiyle düzeltilmesine bağlıdır. Operasyon öncesi yapılan beslenme desteği ile sağlıklı granülasyon dokusu oluşumuyla birlikte aynı zamanda hızlı ve güvenilir yara iyileşmesi sağlanmıştır. In this study, the repair of the upper extremity tissue defects due to high-voltage electrical injuries using groin flaps is discussed. Methods: Ten patients who developed upper extremity tissue defects following high-voltage (> 1000 V) electrical injuries between April 2009 and October 2011 were enrolled in our study. All the patients underwent reconstructions with pedicled groin flaps. All the patients were male. The areas on which the reconstructions were performed were hand and wrist areas where structures like the bones, tendons and nerves are accessible. The separation of the pedicles was performed on days 20-27 (average period: 23 days) after the pedicle ligation examination revealed no problems. Result: Adequate soft tissue coverage was obtained in all patients following the reconstruction. All the flaps healed without problem. No donor area morbidity was observed. No articular rigidity related to the operation was observed in any of the patients. Conclusion: We are of the opinion that in soft tissue defects that form in the upper extremities subsequent to high-voltage electrical injuries, when local flaps or free tissue transplants are risky or inapplicable, reconstruction with pedicled groin flaps constitutes an ideal treatment alternative.Öğe Inverted Nipple Correction with Dermal Flaps and Traction(Springer, 2014) Durgun, Mustafa; Ozakpinar, Hulda Rifat; Selcuk, Caferi Tayyar; Sarici, Murat; Ceran, Candemir; Seven, ErginInverted nipple occurs when part of or the entire nipple is abnormally located below the areola. Surgical repair of severe cases involves suture or flap techniques. Complications include recurrence, lactation problems, hypopigmented scar formation in the areola, and loss of sensation. We describe an alternative repair technique using a dermal flap and traction, which leads to less apparent scarring and preserves lactation function and sensation. Between January 2010 and January 2013, we treated 28 inverted nipples in 16 patients using two areola-based triangular dermal flaps and traction. The scar was aligned with the junction of the nipple and the areola. Postoperatively, traction was applied through an apparatus prepared from a 50-cc syringe. Patients were followed up for 8-24 months (mean = 16.5 months). Adequate projection was achieved in all patients and no wound dehiscence or complications such as infection occurred. Unilateral recurrence occurred in one patient on the 10th postoperative day. This patient was reoperated on successfully using the same method. No loss of sensation was observed in any of the patients during the postoperative period. This dermal flap technique for treating inverted nipple was effective and preserved lactation function. The alignment of the scar with the junction of the nipple and the areola led to a more aesthetic appearance with no apparent scarring. The traction method helped maintain traction for a longer period, which in our opinion increased the success rate of the surgery.
