Öğe ASSESSMENT OF THE LABORATORY TRANSMISSION OF BRUCELLOSIS IN AN ENDEMIC REGION(Carbone Editore, 2014) Deveci, Ozcan; Tekin, Recep; Aslan, Emel; Hosoglu, Salih; Bozkurt, Fatma; Dayan, Saim; Demirpence, OzlemObjectives: Laboratory-acquired brucellosis (LB) has been one of the most commonly reported laboratory-associated bacterial infections in both endemic and non-endemic countries. Brucellosis is endemic in Turkey. The aim of this study is to describe the risk factors of LB among laboratory healthcare workers. Material and method: A regional survey study was conducted by face-to-face interview in 7 hospitals from Diyarbakir, Mardin and Batman province, in southeaster Anatolia in Turkey. A structured survey questionnaire was administered to the Laboratory healthcare workers, employed in infectious diseases clinics and microbiology departments, who were at risk of Brucella infection. Result: Of the 136 laboratory workers, 13 (9.5%) had a history of laboratory-acquired brucellosis. Logistic regression analysis identified factors independently associated with an increased risk of LB including lack of biosafety cabinet (P<0.005) and a lack of compliance in the use of the same (P<0.005). Using a biosafety cabinet (P<0.005), existence of biosafety cabinet (P<0.005), full adherence to glove use (P<0.005) and male gender(P<0.005) were found to be protective factors. Conclusion: Increased adherence to personal protective equipment and use of biosafety cabinets should be priority targets to prevent LB.Öğe High-sensitivity C-reactive protein and high mobility group box-1 levels in Parkinson's disease(Springer-Verlag Italia Srl, 2019) Baran, Aslihan; Bulut, Mahmut; Kaya, Mehmet Cemal; Demirpence, Ozlem; Sevim, Bunyamin; Akil, Esref; Varol, SeferVarious immunologic and inflammatory factors are contributed to pathogenesis of Parkinson's disease (PD). High mobility group box-1 (HMGB1) is a protein that plays certain roles in inflammation, DNA repair, transcription, somatic recombination, cell differentiation, cell migration, neuronal development, and neurodegeneration. The aim of the present study was to evaluate the serum levels of HMGB1 and high-sensitivity C-reactive protein (hs-CRP) among patients with Parkinson's disease and healthy controls. This study includes 30 patients with PD and 30 healthy controls, matched sex, age, body mass index, and smoking status. HMGB1 and hs-CRP serum levels were compared between the groups. The diagnostic performance of HMGB1 and hs-CRP was evaluated with receiver operating characteristic (ROC) curve analysis. HMGB1 levels were significantly higher in PD patients than in controls. Hs-CRP levels were significantly higher in PD patients than in controls There was a moderate correlation between hs-CRP and HMGB1 levels in the patient group. The cut-off value of HMGB1 level for the prediction of PD was determined as 32.8ng/mL with 80% sensitivity and 60% specificity (p=0.006). The cut-off value of hs-CRP level for the prediction of PD was determined as 0.63mg/L with 66.7% sensitivity and 77.7% specificity (p=0.007). This study demonstrates for the first time the association between HMGB1, hs-CRP, and PD. We found that HMGB1 and hs-CRP levels to be significantly higher in the PD patients than in the normal controls. As a result of the ROC curve analysis, HMGB1 and hs-CRP levels may be fair markers in the diagnosis of PD.Öğe Increased High Mobility Group Box1 (HMGB1) level in major depressive disorder(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2015) Demir, Suleyman; Bulut, Mahmut; Kaya, Mehmet Cemal; Sevim, Bunyamin; Demirpence, Ozlem; Ibiloglu, Aslihan Okan; Gunes, MehmetObjective: It was reported that High Mobility Group Box 1 (HMGB1), also known as the nuclear transcription factor, is a late mediator of inflammation. It was thought that HMGB1 has a prominent role in the activation of Tumor Necrosis Factor-a (TNF-alpha), Interleukin (IL)-1 beta and IL-8 which are proinflammatory mediators during inflammation. HMGB1 plays a role in progress, diagnosis and prognosis of immune system illnesses. Besides suppressing the immune system, Major Depressive Disorder (MDD) was indicated to cause changes in inflammatory processes. Biological determinants affecting the diagnosis, therapy, and prognosis of depression are quite limited. Therefore, new etiological models are needed to explain the pathophysiology of depression. There is no study in the literature investigating level of HMGB1 in MDD of the humans. This study aims to examine the role of inflammation in the etiology of depression based on the HMGB1 in patients with MDD. Methods: A total of 30 patients diagnosed with MDD were included in the study. The control group consisted of 30 healthy subjects without any psychiatric disorders. A socio-demographic information form, Hamilton Depression Rating Scale (HDRS) and Clinical Global Impression Scale (CGIS) were administered, and blood was taken for measurement of HMGB1 levels. Results: Significantly higher HMGB1 values were identified with the patient group when compared to the control group (p<0.05). Conclusion: Our study is the first in which HMGB1 level was investigated in MDD ot the humans. The findings of the study reveal that HMGB1 tends to be higher in patients with MDD, and a high HMGB1 value supports the view that inflammation might have a critical role in the etiology of MDD.Öğe Neutrophil-lymphocyte ratio in patients with major depressive disorder undergoing no pharmacological therapy(Dove Medical Press Ltd, 2015) Demir, Suleyman; Atli, Abdullah; Bulut, Mahmut; Ibiloglu, Aslihan Okan; Gunes, Mehmet; Kaya, Mehmet Cemal; Demirpence, OzlemStudies attempting to clarify the relationship between major depressive disorder (MDD) and the immune system have been increasing in recent years. It was reported that increased production of the main proinflammatory cytokines, such as interleukin-1, interleukin-6, and tumor necrosis factor-alpha, and that of acute phase reactants may play a role in the etiopathogenesis of depression. Stress and depression were reported to increase leukocyte and neutrophil counts and to decrease lymphocyte count. Biological determinants affecting the diagnosis, therapy, and prognosis of depression are quite limited. Therefore, new etiological models are needed to explain the pathophysiology of depression. In recent years, neutrophil-lymphocyte ratio (NLR) was determined to be a good indicator of inflammatory status. There is no study in the literature investigating NLR in MDD. This study aims to examine the role of inflammation in the etiology of depression based on the NLR in MDD patients who are undergoing no pharmacological therapy. A total of 41 patients diagnosed with MDD, who received no antidepressant therapy within the past 1 month, were included in the study, which took place between January and March 2015. The control group consisted of 47 healthy subjects with no psychiatric disorders. A sociodemographic information form and a Beck Depression Scale were administered, and the blood was taken for biochemical analysis. Significant differences were identified in the NLR, neutrophil count, lymphocyte percentage, and leukocyte values of the patient group when compared with the control group (P < 0.05). Our study is the first in which NLR was investigated in MDD. The findings of the study reveal that NLR tends to be higher in patients with MDD, and a high NLR value supports the view that inflammation is a critical factor in the etiology of MDD.Öğe The role of High Mobility Group Box 1 (HMGB1) in colorectal cancer(Int Scientific Literature, Inc, 2014) Suren, Dinc; Yildirim, Mustafa; Demirpence, Ozlem; Kaya, Vildan; Alikanoglu, Arsenal Sezgin; Bulbuller, Nurullah; Yildiz, MustafaBackground: HMGB1, the most important member of the high mobility group box protein family, is a nuclear protein with different functions in the cell; it has a role in cancer progression, angiogenesis, invasion, and metastasis development. We studied the expression of HMGB1 and whether it is a prognostic factor in colorectal carcinoma. Material/Methods: The study included 110 cases that were histopathologically diagnosed with colorectal carcinoma from the tissue samples acquired by surgical resection and biopsy in Antalya Education and Research Hospital between 2008 and 2012. HMGB1 expression was examined via immunohistochemical method. Results: HMGB1 expression was evaluated as negative in 32 (44.4%) of the patients and as positive in 40 (55.6%) patients. There was no relation between the HMGB1 expression and sex, age, tumor invasion depth, and histological type. However, a significant relation was detected between the HMGB1 expression and lymph node status, metastasis status, and stage (p:<0.001, p:<0.001, p:<0.001, respectively). Similar results were obtained for the relations between the HMGB1 and histological grade, perineural invasion, lymphovascular invasion, and lymphocytic response (p<0.001, p<0.001, p<0.001, and p<0.001, respectively). Conclusions: The results of our study demonstrate that HMGB1 overexpression has a significant role in tumor progression (especially migration of tumor cells) and tumor ability to metastasize in colorectal cancers; thus, it corroborates the idea that it might be an important prognostic factor.Öğe Seroprevalence of HAV, HBV, HCV and HIV in People Admitted to Batman State Hospital(Galenos Publ House, 2012) Demirpence, Ozlem; Isik Tezcan, Sehavet; Degirmen, Elif; Mert, Duygu; Gumus, Alper; Celen, Mustafa KemalObjective: At the present time viral hepatitis and HIV infection are important global health problems. The aim of this study was to investigate the rates of HAV, HBV, HCV and HIV seropositivity in people those admitted to Batman State Hospital between Macrh 2010 and March 2011. Materials and Methods: Blood samples were searched for HAV IgG, HBsAg, AntiHBs, Anti-HCV and Anti-HIV with CMIA (Chemiluminescent Microparticle Immunoassay) method (Architect I2000 SR, USA). Results: For anti-HBs, 10957 samples were analyzed and 5266 (48.06%) of these samples were positive. For anti-HCV 9666 samples were analyzed and 186 (1.9%) of these samples were positive. For HAV IgG 2606 samples were analyzed and 2447 (93.9%) of these samples were positive. For anti-HIV 1/ 2 6442 samples were analyzed and only 1 sample was positive (0.015%). Conclusion: In conclusion; despite the relatively lower rate of HIV seropositivity, hepatitis viruses, particularly HBV remains to be a significant health problem in our province. Effective vaccination coverage and public awareness about infection control measures would be expected to decrease the emergence of new cases.Öğe Serum paraoxonase, TAS, TOS and ceruloplasmin in brucellosis(E-Century Publishing Corp, 2014) Demirpence, Ozlem; Sevim, Bunyamin; Yildirim, Mustafa; Nurlu, Nilhan Ayan; Mert, Duygu; Evliyaoglu, OsmanIt is possible that brucellosis may be related to increase free radical production and antioxidant depletion. Thus, in the present study we aimed to evaluate the oxidative status in patient with brucellosis and healthy controls. Methods: This study includes the patients with brucellosis diagnosed by clinical findings and positive agglutination titer. The paraoxonase, ceruloplasmin, total antioxidant capacity and total oxidant status values were measured from the samples taken. The oxidative stress index value was calculated through the total antioxidant capacity and total oxidant status values. Results: A total number of 93 people, 40 women (43%) and 53 men (57%) were included to the study. The levels of ceruloplasmin were found higher in patients when compared to the control group (p < 0.001). The total antioxidant capacity level was found significantly higher in the patients group when compared to the control group (p < 0.001). The oxidative stress index value was significantly lower in the patients group when compared to the control group (p < 0.001). The paraoxonase-1 level was not different in control and patient groups (p = 0.077). Conclusions: Brucellosis is an infection that is frequently seen in Mediterranean countries. This infection breaks the oxidant and antioxidant balance. In this disease, oxidant-antioxidant system indicators such as ceruloplasmin, total antioxidant capacity, total oxidant status and oxidative stress index can be used for showing the role of the brucella infection and for the monitoring of the treatment results.
