Öğe Crescentic glomerulonephritis requiring hemodialysis and elevated MPO-ANCA level and vasculitis allergica cutis in a 21-year-old girl.(2002) Kara I.H.; Yilmaz M.E.; Sari Y.; Gür A.; Büyükbayram H.Microscopic polyangiitis (mPA) is a systemic necrotizing nongranulomatous vasculitis that affects small blood vessels. Clinical features include constitutional symptoms such as fever, anorexia, fatigue, and weight loss. Skin lesions include purpura and splinter hemorrhages, which occur in 50% of patients. Another characteristic feature is rapidly progressive glomerulonephritis, which often affects the kidneys in the early stages of the condition. Diagnosis is based on typical clinical features, biopsy, and presence of antineutrophil cytoplasmic antibodies (ANCA). This disorder tends to involve middle-aged and older persons, with a predilection for males; it is very rare in children. A 21-year-old female patient with mPA who did not respond well to treatment, required hemodialysis, developed vasculitis allergica cutis (VAC) later, and had a relatively short-term survival is reported herein.Öğe Double thyroglossal duct cyst located in the hyoid region and the tongue base: an unusual coexistence.(2011) Yorgancilar E.; Yildirim M.; Gün R.; Büyükbayram H.; Topçu I.Thyroglossal duct cysts originate from persistent epithelial remnants of the thyroglossal duct that are present during the descent of the thyroid gland from the foramen cecum to its final position in the anterior neck. Thyroglossal duct may persist anywhere along the descent from the tongue to the thyroid. Lingually localized thyroglossal duct cysts are congenital lesions of the pharynx, which are very uncommon and may cause morbidity and mortality if not treated expeditiously. Double thyroglossal cysts and the complete failure of the obliterative process of thyroglossal duct are very rare in the literature. In this article, we describe a case of two cysts derived from the same thyroglossal duct, one in the hyoid region and the other in the tongue base. To our knowledge, this coexistence has not been described previously in the literature.Öğe Effectiveness of fibrin tissue adhesive for colocolic anastomosis reliability(2001) Yilmaz H.G.; Odabaşi M.; Büyükbayram H.; Baç B.Leakage from a colonic anastomosis remains a major complication of surgery for colon emergencies and is associated with a significant increase in postoperative hospital stay and mortality rate. The purpose of our study was to investigate whether fibrin sealant, a well known tissue adhesive, will augment healing of bowel anastomosis in an experimental peritonitis model. Forty-eight male Sprague-Dawley rats were randomized into four groups. All rats underwent conventional sutured colo-colic anastomosis. The rats in group 3 and 4 had been exposed peritonitis before that procedure. In groups 2 and 4 fibrin sealant was added to the suture line. The measurements of bursting pressure and histopathological evaluations of suture lines revealed that peritonitis was deleterious to healing of colonic anastomosis. It was also seen that fibrin sealant augments healing of colonic anastomosis both in normal and peritonitis models.Öğe The effectiveness of hemopoietic growth factors in sepsis(2001) Yilmaz G.; Aldemir M.; Yilmaz R.; Diken H.; Büyükbayram H.; Akgün Y.In this experimental study, consist of 54 Sprague-Dawley rats, we tried to observe the effectiveness of haemopoietic growth factors such as G-CSF and GM-CSF in treatment of sepsis and see if they have any effects on phagocytic activity of macrophages when are administered after establishment of sepsis. In first phase of this study, twenty one rats were randomly divided into three groups of 7 animals each. Cecal ligation and perforation were carried out in each rat and sepsis made up. The Control group received 2 x 0.2 cc %5 dextrose injection subcutaneous (s.c.).. The G-CSF group received 2 x 1 g G-CSF with 0.2 cc %5 dextrose s.c. The GM-CSF received 1 x 2 g GM-CSF with 0.2 cc %5 dextrose s.c. Seventh day survival was considered as criterion in the three groups. In second phase of this study, thirty three rats were randomly divided into three groups of 11 animals each. The same procedures were carried out also in these groups. Leukocyte counts and peripheric spread were analyzed in postoperative 24th and 72th hours, alveolar and peritoneal macrophages were Investigated in postoperative 72nd hour. There was significantly neutrophilic leukocytosis in the G-CSF group according to the control group. Nevertheless, there was no change in the phagocytic activity of alveolar and peritoneal macrophages. GM-CSF brought about positive effect of phagocytic activity of macrophages without change of leucocyte count in the sepsis, but it caused neutrophilic, monocytosis and lymphocytopenia. The seventh day survival rates in control, G-CSF, GM-CSF groups were as; 42.8%, 71.4%, 28.5% respectively. As a result, we saw that G-CSF has no effect on the phagocytic activity of macrophages, while increases the survival by enhancing the count and probably the function of neutrophils. GM-CSF fails to increase survival while effects the phagocytic activities of macrophages positively and enhances the peripheral neutrophil and monosit counts without changing the total number of leukocytes.Öğe The effects of epidermal growth factor deficiency on rat gingival epithelia(2001) Ketan? M.A.; Dağ A.; Zeng?ngül A.?.; Büyükbayram H.; Özbağ D.Epidermal growth factor (EGF) is a low molecular mass polypeptide with 53 amino acids and is known to stimulate cell proliferation and differentiation in a wide range of tissues. EGF is thought to have important functions in epithelial growth and differentiation and in wound healing. In the present study, the teratogenic effects of sialoadenectomy on rat gingival epithelia were investigated histologically. Twenty adult female Wistar albino rats were divided into two groups (N = 10), a control and an experimental group. The experimental group was subjected to sialoadenectomy in order to create EGF deficiency. After 60 days of sialoadenectomy (EGF deficiency), control group rats were killed using pentobarbital and their maxilla removed. The sections were stained with haematoxylineosin and Mason triple for evaluation by using a light microscope. Astatistically significant reduction in body mass was noted in rats in the experimental group when compared to the control group. Decreasing the thickness of keratization layer, irregularity and disappearance of microscopic papilla, intraepithelial focal cystic lesions resulting from EGF deficiency, and a decrease in saliva, were noted. As a result, epidermal growth factor deficiency performed by sialoadenectomy caused body mass reduction and gingival epithelia abnormalities.Öğe The effects of topical hypothermia, methylprednisolone and BN 52021 on hepatic ischemia/reperfusion injury(Turkish Surgical Society, 2002) Taçyildiz I.; Aldemir M.; Girgin S.; Büyükbayram H.; Baç B.This study was undertaken to investigate the effectiveness of topical hypothermia, methylprednisolone and BN 52021 on ischemia/reperfusion induced liver injury. The liver is one of the organs most vulnerable to ischemia because of its high metabolic state. Occlusion of the portal triad is often required in complex hepatic surgery in order to decrease parenchymal bleeding. Liver failure due to ischemia-reperfusion injury is a serious problem during liver surgery. Fifty male Sprague-Dawley rats (200-225 gr) were divided into 5 groups each containing 10 rats. Midline laparatomy was fulfilled on all the rats under ketamine anesthesia. In group 1 (Sham), the hepatoduodenal ligament was dissected without any drug administration. In group 2 (İ/R), the ischemia (for 45 min)/ reperfusion (for 45 min) was performed and received no further treatment. In group 3 (Hypothermia), the ischemia plus topical cooling (for 45 min) and reperfusion (for 45 min) was fulfilled. In the group 4 (Steroid), the İ/R was performed and methylprednisolone (30 mg/kg) was administered. In group 5 (BN 52021), the İ/R was performed and BN 52021 (100 mg/kg) was given before ischemia. Liver, ileum and blood samples (for MDA, ALT, AST, LDH and ileal mucosal injury score) were obtained. The levels of ALT,AST, LDH, MDA(ileal, hepatic) and ileal injury score in group İ/R were significantly higher than those of group sham (p>0.0001). When group İ/R was compared with group hipothermia, there were no significant differences with regard to AST, ileal MDA and ileal injury score (p>0.05), whereas levels of ALT, LDH and hepatic MDA were significantly higher (p<0.0001, 0.0001, <0.0001, respectively). When group İ/Rwas compared with group steroid and group BN 52021, it was determined that those agents had significant protective effects on hepatic and ileal injuries (p<0.0001). It was determined that topical hypothermia decreased hepatic İ/R injury, however had no protective effect on ileal injury. We determined that Methylprednisolone and BN 52021 decreased both hepatic İ/R and ileal injury due to splanchnic congestion.Öğe Ganglioglioma in the nasal cavity: a case report.(2010) Yorgancilar E.; Yildirim M.; Gün R.; Büyükbayram H.; Meriç F.Ganglioglioma is a tumor containing both astrocytic and neuronal components. It may occur any where in the central nervous system and spinal cord but is only encountered rarely. Nasal glial heterotopia (also known as ''nasal glioma''), is a rare developmental abnormality seen in a wide age group. Gangliogliomas may also manifest as a nasal glial heterotopia, and neurogenic tumors should be considered in the presence of a nasal mass. In this article, we present a case of ganglioglioma located in the right-nasal cavity. The mass was excised totally through an endoscopic approach. The ganglioglioma developed on a nasal glial heterotopia base. To our knowledge, a ganglioglioma arising from the nasal cavity has not been described previously in the literature.Öğe Is there an association between immunohistochemical parameters of breast cancer and metabolic parameters obtained with 18F-fluorodeoxyglucose PET / CT?(DergiPark, 2019) Tuzcu S.A.; Gezici A.; Taşdemir B.; Kaplan İ.; Büyükbayram H.; Çetin F.A.Introduction: The aim of our study to investigate relationship between 18F -fluorodeoxyglucose PET/CT (18F-FDG PET/CT) metabolic parameters and immunohistochemical factors in breast carcinomas. Material and method: Patients with breast carcinomas who underwent 18F-FDG PET/CT imaging at our department between May 2018 and November 2019 were included in this study. A total of 146 female patients were included (aged 49.1 ± 13.4years; range, 26-87 years). PET scanning was performed in 3D mode from the skull ceiling to the middle of the thigh. Metabolic parameters such as TLG (Total lesion glycolysis), MTV (Metabolic tumor volume), SUVmean and SUVmax values were calculated. We obtained the histopathological findings, including the size of invasive cancer, histological type, histological grade, ER and PR status, epidermal growth factor receptor (HER2) and Ki-67 of the primary tumor by reviewing the pathology reports. Result: SUV max and SUVmean of Oestrogen receptor negative group were statistically higher than Oestrogen receptor positive group (p=0.009). SUVmean of progesterone receptor negative group were statistically higher than progesterone receptor positive group (p=0.05). Ki-67 of the Oestrogen receptor negative group and progesterone receptor negative group were statistically higher than Oestrogen receptor and progesterone receptor positive group (p=0.001, 0,001 respectively). Both SUVmax and SUVmean of Ki-67 positive group were statistically higher than Ki-67negative group (p=0.0001). Conclusions: 1-SUV max, SUVmean and Ki-67 of Oestrogen receptor negative group were statistically higher than Oestrogen receptor positive group. 2-SUVmean and Ki-67 of progesterone receptor negative group were statistically higher than progesteron receptor positive group. 3-HER2 positive and/or triple negative breast cancers were not associated with 18F-FDG PET/CT metabolic parameters © 2019 Published by International Archives of Medical Research. All rights reservedÖğe Patients with Hodgkin's disease: Clinical and pathological evaluation(2006) Altintaş A.; Çil T.; Kaplan M.A.; Atay A.E.; Işikdoğan A.; Büyükbayram H.; Ayyildiz O.Hodgkins' disease accounts 25% of all lymphoid malignancies and 1% of all cancers. In this study, 150 patients with Hodgkins' disease were reviewed retrospectively and compared according to age, gender, histopathologic subtype and stage of disease. 102 (68%) of patients were male and 48 (32%) were female with a median age of 41 (16-73) years. The frequencies of histopathological subtypes, mixed cellularity, nodular sclerosis, lymphocyte-rich and lymphocyte-depletion, were 63 (42%), 37 (24.6%), 25 (16.6%) and 21 (14%), respectively. Histological subtype was not determined in 4 (2.7%) patients. Distribution of patients according to stages were; 18 (12%) patients were in stage I, 35 (23.3%) patients were in stage II, 52 (34.7%) patients were in stage III and 43 (28.7%) patients were in stage IV. Sufficient staging procedure was not performed in two patients. SPSS 11.0 program was used for statistical analyzes. Patients were divided into two groups; older and younger than 40 years age and no statistical significancy was detected between groups. Also there was no significant relation between histopathological subtype and age. In conclusion; Hodgkin disease predominantly seen in males (M/F: 2.1). Mixed celularity HD was the most frequent subtype and 63.7% of our patients were in advanced stage (stage III ve IV).Öğe Primary gastrointestinal lymphomas: A clinicopathologic study of 55 cases(2001) Yilmaz F.; Arslan A.; Ayyildiz O.; Uzunlar A.K.; Büyükbayram H.; Keleş C.Purpose: To study primary gastrointestinal (GI) non-Hodgkin's lymphomas (NHLs) in our region of the southeast Turkey, and to compare our results with those found in the relevant literature. Materials and methods: Fifty-five patients with surgical resection of primary GI NHLs were reviewed retrospectively, and their histologies were classified according to the Working Formulation. Clinical data obtained from pathology and endoscopy reports was also reviewed. Results: The cases of thirty-two male and 23 female patients were studied. GI NHLs were most frequent in the first and fifth decade of life (mean 30.9 years, range 3-75 years). Abdominal pain, weight loss and bleeding were the most common presenting symptoms. The primary sites of the tumors were: 16 in the stomach, 26 in the small intestine, 3 in the iliocecal region, and 10 in the large intestine. Eight (15%) of the 55 GI NHLs were of low-grade malignancy (5 lymphocytic and 3 follicular small cleaved cell), 38 (69%) were of intermediate-grade malignancy (9 diffuse small cleaved cell, 6 diffuse mixed, 23 diffuse large cell) and 9 (16%) were of high-grade malignancy (1 immunoblastic, 3 diffuse small noncleaved cell and 5 lymphoblastic). Conclusion: Clinicopathologically, our cases of primary GI NHLs seem to resemble the "Mediterranean" type of NHLs.Öğe A rare cause of anemia in a patient with end stage renal disease: Pulmonary hemorrhagic syndrome: Case report(Turkiye Klinikleri, 2007) Şit D.; Kadiroğlu A.K.; Kayabaşi H.; Büyükbayram H.; Yilmaz M.E.; Işikoğlu B.The association of pulmonary hemorrhagic syndrome and end stage renal disease is a rare condition. Severe anemia that occurred suddenly in a 27 years old male patient who was undergoing chronic HD program was investigated. Platelet count was normal but bleeding time was prolonged. Pulmonary hemorrhagic syndrome was considered secondary to platelet dysfunction. The diagnosis was confirmed by pulmonary biopsy. The patient was successfully treated with conservative approach.
