Tedavide hastalık modifiye edici ilaçlar ve biyolojik ajanlar kullanılmaktadır. Çalışmalarda obezitenin biyolojik ajanlara cevabı azalttığı gösterilmiştir. Visseral yağ obezitenin neden olduğu metabolik ve inflamatuar olaylardan sorumlu tutulmaktadır. Bu çalışmanın amacı RA’lı hastalarda visseral yağ oranını ve hastalık aktivitesine etkilerini belirlemektir. Dicle Üniversitesi Tıp Fakültesi Hastanesi Fiziksel Tıp ve Rehabilitasyon polikliniğine başvuran 101 RA’lı hasta ve 45 sağlık kontrol grubu çalışmaya alındı. Vücut yağ doku yüzdesi, visseral yağ yüzdesi Biyoempedansmetre tekniği ile çalışan OMRON BF 508 marka vücut analizi aleti ile ölçüldü. RA’lı hastalar muayene edilerek artrit ve hassas olan eklemleri belirlenerek DAS 28 değerleri hesaplandı ve hastalık aktiviteleri DAS 28 ve VAS ile belirlendi. RA’lı hastalarda ilaç alt grubu oluşturularak yapılan değerlendirmede anti TNF kullanan grupta total yağ ortalaması 43,1, Disease Modifying Anti Rheumatısmal Drug (DMARD) grubunun total yağ ortalaması 38,5, kontrol grubunu total yağ ortalaması 36,9 idi. Total yağ açısından anti-TNF ve kontrol grubu arasında istatiksel olarak anlamlı fark saptandı (P=0,031); diğer gruplar arasında total yağ açısından anlamlı fark saptanmadı (p = 0,113 ve 0,705). Anti TNF kullanan hastalarda visseral yağ ortalaması 11,1 DMARD grubunun visseral yağ ortalaması 8,6, kontrol grubunun visseral yağ ortalaması 8,9 idi. Anti TNF grubunun visseral yağ oranı kontrol ve DMARD grubuna göre yüksek olup istatiksel olarak anlamlı fark saptandı (p=0,018 ve 0,003). Romatoid artrit hastalarında visseral yağ oranı daha yüksek olduğu tespit edildi. Anti TNF kullanımı ile hem total ve hemde visseral yağ oranlarının daha fazla arttığı tespit edildi ve bununda tedavi yanıtını etkileyen bir faktör olabiliceği düşünüldü. Methods Forty-eight patients with BD and 29 age-and gender-matched healthy control subjects were included in the study. Serum pentraxin 3 levels were measured using enzyme-linked immunosorbent assay (ng/ml), and alpha 1-acid glycoprotein levels were measured using nephelometry (mg/dl). Disease activity was assessed using the BD Current Activity Form (BDCAF). Results Serum alpha-1 acid glycoprotein levels were significantly higher in patients with BD compared to the control group (P = 0.048). There were no significant differences between two groups in terms of levels of pentraxin 3 (P = 0.697). According to Pearson's analysis, alpha 1-acid levels are significantly positively correlated with erythrocyte sedimentation rate, C-reactive protein, and skin lesions and arthritis of BDCAF scores. Pentraxin 3 levels did not correlate with erythrocyte sedimentation rate, C-reactive protein, or any domains of BDCAF scores. Conclusion The results of the present study demonstrate that serum levels of alpha-1 acid glycoprotein were significantly higher in patients with BD relative to the control group. Alpha-1 acid glycoprotein is greatly associated with skin lesions and arthritis in patients with BD. We did not find high serum levels of PTX3 in patients with BD compared to healthy controls, and pentraxin 3 is not associated with disease activity in BD.Öğe Visceral Fat Reflects Disease Activity in Patients with Ankylosing Spondylitis(Canadian Soc Clinical Investigation, 2014) Aydin, Mesut; Aydin, Fatma; Yuksel, Murat; Yildiz, Abdulkadir; Polat, Nihat; Akil, Mehmet A.; Bilik, Mehmet Z.Purpose: Response to infiiximab treatment diminishes as body mass index (BMI) increases in patients with ankylosing spondylitis (AS). The purpose of the study was to determine if diminished response to in fliximab treatment in patients with AS could be associated with increased visceral adipose tissue rather than increased BMI. Methods: Twenty six AS patients (21 males and five females) who fulfilled the modifid New York criteria and who were currently receiving infiiximab treatment were enrolled in the study. Pain was measured by the visual analogue scale (VAS). The disease activity and functional status were assessed by the Bath AS Disease Activity Index (BASDAI) and the Bath AS Functional Index (BASFI). The Bath AS Metrology Index (BASMI) was used to evaluate mobility restrictions. Weight and visceral body composition were measured without shoes in light indoor clothes using a bio-impedance meter. Results: There was a significant correlation between visceral adipose tissue amount and disease activity under infiiximab treatment. In correlation analysis, visceral fat showed significant correlations between BASDAI (r=0.545, p=0.004) and VAS (r=0.458, p=0.019). Total body fat also showed a significant correlation with BASDAI (r=0.463, p=0.017). Conclusion: A significant correlation was found between visceral adipose tissue amount and disease activity in patients with AS.
