Öğe Chylothorax with Down syndrome: Unusual case report(2009) Avci A.; Ülkü R.; Onat S.Trisomy 21 or Down syndrome (DS) is the most common chromosome aberration in the newborn. Its high incidence has led to better understanding of congeniatal anomalies and diseases in association with this syndrome. The presence of protein, triglycerides, and lymphocytes in the pleural effusion is diagnostic of chylothorax. Lymph vessel abnormalities or a leaky thoracic duct are thought to be origin of congenital chylothorax, which is repoted many times in the literature. But, there is no information in the english literature about 1-year-old baby with DS who is complicated with chylothorax. Aplasia of the twelfth rib is rare but found to be more common in DS than in normals. We report on a 1-year-old girl with DS refered to our clinic with signs and symptoms of respiratory distress. Diagnosis and treatment of chylothorax, pleural emphyema, and pericardial effusion were obtained in algorithm.Öğe Penetran thoracic wall traumas due to sharp devices(2007) Avci A.; Sayhan M.B.; Eren T.Ş.Penetran thoracic wall traumas due to sharp devices are important injuries because of the intrathoracic vital organs and blood vessels anatomic locations. Because of these, these types of traumas should be examinated carefully, and might be needed surgical operations. Cases of intrathoracic organ injury, thoracic wall injury, been of foreign body are the many causes of the surgery. Delayed diagnosing or misdiagnosing increase the mortality and morbidity in such traumas. We would like to report our delayed diagnosed, transthoracic and extrapleural penetran trauma due to large sized broken glass.Öğe Pleomorfic carcinoma: A rare case of lung cancer(2008) Özçelik C.; Avci A.; Kilinç N.; Onat S.Pleomorphic carcinoma is a rare epithelial malignant tumor. Pulmonary pleomorphic carcinoma was introduced by the 1999 World Health Organization classification as a new peculiar type of lung carcinoma showing concurrent malignant epithelial and sarcomatoid spindle cell elements. Rarely seening, advange age, male sex dominance, upper lobe involvement, big sized (more than 7 cm.) at time of diagnosis , and poor prognosis are the characteristics of it. We are reporting 63-year-old man with pleomorfic carcinoma in the left lower lobe who was treated by left pneumonectomy.Öğe Primary yolk sac tumor in the thoracic wall: Case report(2009) Avci A.; Ulku R.; Onat S.; Ozcelik C.Malignant extragonadal germ cell tumors primary to the thoracic wall are quite uncommon lesions, but pure yolk sac tumor is even more exceptional. This is believed to be the first reported case of yolk sac tumor of the chest wall. A 33-year-old female patient with a primary extragonadal giant yolk sac tumor presenting with thoracic wall mass, complaint of back pain.Öğe A Rare Hydatid Cyst Involvement: Bilateral Pulmonary Artery (Case Report)(2010) Abakay A.; Gümüş H.; Tanrikulu A.C.; Atilgan Z.A.; Avci A.Hydatid cyst is a parasitic disease that may involve many organs. Hydatid cyst with pulmonary artery involvement is a very rarely encountered form. It was aimed to present a rare bilateral pulmonary artery involvement determined in a 61-year-old male case with hydatid cyst. In our case, the lesions were determined in many segments of bilateral lungs, right atrium lateral wall of the heart and in many branches of bilateral pulmonary artery. Pathological evaluation of the patient underwent surgery for the cardiac mass revealed hyda-tid cyst. The patient, who refused the proposal of surgery for the lesions in lungs and pulmonary artery, was given medical drug therapy. We think that it may be helpful to include hyda-tid cyst in prediagnosis list for the patient applied to a doctor with multiple cystic or nodular images.Öğe The relationship between coagulation factor XI levels and cardiac syndrome X in patients with diabetes mellitus(Cardiology Academic Press, 2014) Yazicioglu M.V.; Yuksel M.; Batgerel U.; Acar G.; Avci A.; Simsek Z.; Tabakci M.M.Cardiac syndrome X (CSX) is a characterized with normal epicardial coronary arteries and exertional angina pectoris. The aim of this study was to measure the levels of coagulation factor XI (F XI) and XII (F XII) to compare the intrinsic coagulation pathway activities in DM patients with and without CSX. A total of 50 Type 2 DM patients with CSX diagnosed by non-invasive tests accompanied with coronary angiography, and control group of 51 Type 2 DM patients matching with the CSX group in terms of age and sex were included in the study. F XII levels were tended to be higher in patient group than controls [107 ± 26 vs 99 ± 21 IU/dL; p = 0.124, respectively]. Moreover F XI levels were found to be significantly higher in CSX patient group compared to the controls [130 ± 23 IU/dL vs 118 ± 17 IU/dL; p = 0.005, respectively]. This study demonstrates that intrinsic coagulation pathway of DM patients with CSX is more active than those without CSX. It is also suggested that hypercoagulability may play an important role in the pathogenesis of CSX.Öğe Resection of intercostal hemangioma with involved chest wall and ribs: In an 11-year-old girl(2010) Ulku R.; Onat S.; Avci A.; Ozmen C.A.We report a case of an 11-year-old girl who presented with a slowly enlarging mass in the right posterolateral chest wall. Computed tomography showed a soft-tissue mass 8.5 × 7.5 × 5.5 cm in size, arising from the right posterolateral 9th, 10th, and 11th intercostal spaces. Magnetic resonance imaging confirmed a vascular mass. The patient underwent complete resection of the tumor, together with the right 8th, 9th, 10th, 11th, and 12th ribs and their intercostal muscles. Reconstruction of the chest wall was performed with methyl methacrylate and Marlex mesh. Histopathologic examination of the tumor confirmed an intercostal cavernous hemangioma. At last examination, 6 months after the operation, the child was doing well, with no evidence of recurrence. © 2010 by the Texas Heart® Institute, Houston.Öğe Traumatic sternal fracture: Three years experience of thoracic surgery department(2008) Ülkü R.; Avci A.; Onat S.; Sayhan M.B.; Özçelik C.Traumatic sternal fractures are becoming common injuries in developed and developing countries. We would like to report our three years experience with sternal fracture. Emergency room management and clinical examinations are reviewed in this study.
