Öğe Akut böbrek yetmezliğinde inflamatuar belirteçlerin biyokimyasal parametrelerin ve demografik özelliklerin mortalite üzerine etkileri(2017) Atay, Ahmet Engin; Yılmaz, M. EminAmaç: Çalışmamızda ABY'li hastalarda demografik özelliklerin, biyokimyasal parametrele rin, proinflamatuar ve antiinflamatuar belirteçlerin mortalite üzerine etkilerini araştırdık. Materyal ve Metod: Dicle Üniversitesi Tıp Fakültesi Nefroloji kliniğinde ABY tanısı ile takip edilen (28 erkek, 31 kadın) 59 hasta çalışmaya dahil edildi. Daha önceden böbrek hastalığı anamnezi olmayan bireylerde 48 saat içinde 0,5 mg/dl'den daha fazla artış olanlar ile daha önceden böbrek hastalığı anamnezi olanlarda 48 saat içinde 1 mg/dl'den daha fazla artış olduğu saptanan hastalar çalışmaya dahil edildi. Hastaların demografik bilgileri kaydedildi. Biyokimyasal parametreleri,hemogram,sedimantasyon, lipit profili, sitokin düzeylerinin ölçümleri için kan örnekleri 48 saat aralıklarla alındıktan sonra çalışıldı. Hastalar klinik son durumlarına göre yaşayanlar (grup 1: n = 46) ve exitus ( grup 2: n = 13) olanlar olarak iki gruba ayrıldı. BulgulanTüm hastaların ortalama yaşı 52.3 ± 17.9 yıl ( mini 6 yıl, mak 78 yıl). 46 hasta (% 77.9 ) hayatta kaldı ve 13 hasta (% 22.1) exitus oldu. Grup 2'de erkek cinsiyetin baskın olduğu görüldü (p= 0.016). IL-2R ve TNF-a exitus grubunda yaşayanlar grubuna göre anlamlı olarak daha yüksek saptandı (p=0,000 ve p=0,020). Total kolesterol, HDL ve LDL düzeyleri exitus grubunda yaşayanlar grubuna göre anlamlı olarak daha düşük saptandı ( sırasıyla, p=0,041, p=0,020 ve p=0,042). Multipl lineer regresyon analizinde de proinflamatuar sito- kinlerin mortalite üzerine etkileri anlamlı olarak saptandı (r square = 0,341, p=0,005).Hasta- nede kalman gün sayısı ile mortalite arasındaki ilişki anlamlı idi(p=0,016). Yaş ve mortalite oram arasında anlamlı düzeyde bir ilişki saptanmadı(p=0,206) Sonuç: ABY'nin klinik gidişinde erkek cinsiyet, inflamatuar sitokinlerden IL-2R ve TNF-a ile kötü beslenme belirteçlerinin varlığının prognostik ve prediktif öneme sahip olduğu düşünülmektedir.Öğe The association of endothelial nitric oxide synthase gene G894T polymorphism and serum nitric oxide concentration with microalbuminuria in patients with gestational diabetes(Dustri-Verlag Dr Karl Feistle, 2014) Atay, Ahmet Engin; Akbas, Halit; Tumer, Cemil; Sakar, Mehmet Nafi; Esen, Bennur; Incebiyik, Adnan; Simsek, SeldaAim: Gestational diabetes mellitus (GDM) is a glucose intolerant condition that affects 14% of all pregnancies. Diabetes mellitus (DM) occurs in 30 - 70% of patients with GDM after delivery. DM and GDM are associated with structural and functional deterioration of the renovascular system. Our aim is to investigate the association Glu-298Asp polymorphism of the endothelial nitric oxide synthase (eNOS) gene with serum nitric oxide levels and microalbuminuria in patients with GDM and healthy pregnancies. Material and methods: Serum nitric oxide (NO) levels, urinary excretion of albumin and Glu298Asp polymorphism of the eNOS gene were analyzed in 68 patients with GDM and 73 healthy controls. High performance liquid chromatography (HPLC-Griess) method was used to analyze serum NO levels. Microalbuminuria was evaluated by rate nephelometry method. The Glu298Asp polymorphism of the eNOS gene was determined by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP). Results: Nitric oxide, glucose, creatinine, and microalbuminuria were significantly different between the patients and the control subjects (p = 0.001, p = 0.001, p = 0.002, and p = 0.005, respectively). There was a significant difference between groups in terms of the ratio of GG/GT+TT of eNOS gene Glu298Asp (p = 0.02). The patients with GT+TT genotype had significantly higher microalbuminuria levels and lower NO concentrations (22.16 vs. 9.51, p = 0.005, and 10.56 vs. 12.73, p = 0.021, respectively). The presence of T allele of eNOS gene is an independent predictor of microalbuminuria (OR: 2.346, 95% confidence interval: 1.247 - 5.238, p = 0.02) as well as serum glucose and NO concentration. Conclusion: The G894T polymorphism of eNOS gene and decreased NO concentration seem to be independent predictors of increased urinary excretion of albumin in patients with GDM. Methods: Twenty-eight female Sprague-Dawley rats were enrolled in the study and the rats were divided into four equal sized groups randomly (Group N: the rats exposed only to nicotine, group HB: the rats received only HBO, group N+HB: the rats that underwent to nicotine injection and subsequently received HBO, group C: the control group that neither exposed to nicotine nor received HBO). The rats were sacrified by decapitation method and all were enucleated immediately after scarification. Tissue samples from crystalline lens, lens capsule, and the retina from the right eyes of the rats were examined by light microscopy. Results: While the histological appearances of the retina and the lens was similar in group HB, group N+HB, and the control group; group N showed some pathological changes like decrement in the retinal ganglion cell density, atrophy of the retinal nerve fiber layer, congestion of the vessels in the optic nerve head, thinning of the internal plexiform layer, thinning of the lens capsule, and transformation of the anterior subcapsular epithelium into squamous epithelia. Discussion: Subcutaneous injection of nicotine was found to be related with some pathological changes in the retina and lens of the Sprague-Dawley rats. However HBO caused no significant negative effect. Furthermore, the histopathological changes related to nicotine exposure in the lens and retina of the rats recovered by the application of HBO.Öğe The Effects of Pegylated Interferon Alpha-2a and Alpha-2b Therapy on Chromosomal Aberrations and Mitotic Index in Patients with Chronic Hepatitis B(Ortadogu Ad Pres & Publ Co, 2012) Akbas, Halit; Yalcin, Kendal; Isi, Hilmi; Simsek, Selda; Atay, Ahmet Engin; Budak, TurgayObjective: We aimed to prospectively evaluate the effects of pegylated interferon alpha-2a and alpha-2b therapy on chromosomal aberrations and mitotic index in patients with chronic hepatitis B. Material and Methods: Fifty patients with chronic hepatitis B who underwent pegylated interferon alpha-2a or 2b therapy were evaluated for chromosomal aberrations and mitotic index before the treatment and at the end of 6 months of therapy. Results: Cytogenetic examinations revealed out that there was no significant difference between pre- and post-treatment frequencies of mitotic index and chromosomal aberrations. Conclusion: Interferon alpha-2a and alpha-2b therapy which is associated with common hematologic adverse effects has no significant cytogenetic effect.Öğe THE EVALUATION OF PARAOXONASE 1 ACTIVITY IN PATIENTS WITH GESTATIONAL DIABETES(Carbone Editore, 2013) Atay, Ahmet Engin; Sakar, Mehmet Nafi; Culcu, Nurdagul Serife Nurani; Simsek, Hakki; Akbas, Halit; Acar, Murat; Isik, BirgulMaterial and methods: Sixty five patients with GDM and 66 healthy pregnants were enrolled. Paraoxonase activity, insulin levels, HOMA-IR, demographic features and anthropometric measurements were evaluated. Results: The mean paraoxonase activity was significantly diminished in patients with GDM (p:0,004). Insulin level and HOMA-IR were significantly higher in GDM patients (p:0,004 and p:0,001; respectively). The mean interval between present and previous pregnancy was significantly shorter in patients group (p:0,004). There was a significant correlation between PON1 activity and serum LDL, HDL levels and weight gained during pregnancy (p:0,001,p<0,001 and p:0,002; respectively) but not with weight and parity. However HOMA-IR was significantly correlated with weight and parity (p:0,001 and p:0,002; respectively). Discussion: Diminished PON1 activity and enhanced oxidative stress accompany to insulin resistance in the pathogenesis of GDM. Oxidative stress is associated with weight gained during first 2 trimesters of pregnancy rather than obesity.Öğe Evaluation of seroprevalence of Hepatitis B Virus and Hepatitis C Virus in reference Center Hospital in Turkey's Southeast Anatolia among patients referred for major or minimally invasive surgery: Last decade experience in a previously high prevalence area(Kowsar Publication, 2021) Ekin, Nazım; Uçmak, Feyzullah; Atay, Ahmet EnginBackground: There is a high worldwide prevalence of chronic Hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) infections, among the significant causes of liver-related morbidity and mortality. Objectives: We aimed to determine the prevalence of HBV and HCV in a referral center hospital in Southeast Anatolia among patients that applied for major or minimally invasive surgery. Methods: In a tertiary referral state hospital for general purposes, patients undergoing surgical procedures and serologic examinations for HBV and HCV were included in the study between January 2011 and September 2020. Results: In the general population, hepatitis B surface antigen (HBsAg) and anti-HBs were examined in 220,724 patients, and anti-HCV was examined in 186,017 patients. The mean age was 42.3 +/- 20.2 years with a 51.8% male distribution. The frequency of positive HBsAg and anti-HCV in all patients was 9.4 and 0.9%, respectively. On the other hand, the frequency of positive HBsAg and anti-HCV was 4.2 and 0.7%, respectively, among patients admitted for a surgical procedure. The mean age was 46.1 +/- 21.1 years with a slightly male predominance (54 vs. 46%). In this group, the frequency of positive HBsAg was higher in males (5.1%) while the lowest was in the 1 - 10 age range (0.4%) and the highest in the 41 - 50 age range (5.7%). Between 2011 and 2019, the prevalence of HBsAg positivity decreased from 6.4 to 4.0%, while anti-HCV positivity was similar in both genders, and its frequency increased with age. We aimed to evaluate the predictive role of APPs in the response to antiviral therapy. Methods: Forty-five patients underwent antiviral therapy. Serum ferritin, C-reactive protein (CRP), transferrin, albumin, alpha-1 acid glycoprotein (A1AG), and alpha-2 macroglobulin (A2MG) levels were examined at the initial evaluation and at the 4th, 12th, and 48th weeks. HCV RNA levels were examined at the initial evaluation and at the 12th and 48th weeks. Results: Ferritin, transferrin, A1AG, and A2MG levels were significantly higher in the patient group (p<0.05). CRP, ferritin, A1AG, and A2MG levels were significantly increased from baseline to the 4th week (p<0.05). The responders and nonresponders to antiviral therapy had insignificantly but remarkably different levels of CRP, ferritin, transferrin, A1AG, A2MG, and alanine aminotransferase (ALT) both at the initial evaluation and at the 12th week. Conclusions: Variations in ferritin, A1AG, A2MG, albumin, CRP, and transferrin levels are not alternatives to virological and biochemical parameters for predicting an early response to therapy in patients with CHC. However, the investigation of ALT levels and hepatitis C virus RNA in combination with acute-phase reactants may provide supplementary data for evaluating responses to antiviral therapy. Hemangioma of the rib and chest wall tuberculosis may be misdiagnosed as chest wall tumors. We herein report a case of hemangioma of the rib with a tuberculosis infection of the pleura.Öğe Use of Adjunctive Mitomycin C in External Dacryocystorhinostomy Surgery Compared With Surgery Alone in Patients With Nasolacrimal Duct Obstruction: A Prospective, Double-Masked, Randomized, Controlled Trial(Elsevier, 2009) Ari, Seyhmus; Gun, Ramazan; Surmeli, Serdar; Atay, Ahmet Engin; Caca, IhsanBACKGROUND: The most common Cause for the failure of external dacryocystorhinostomy (DCR) surgery is the formation of granulation tissue at the osteotomy site or common canaliculus. OBJECTIVES: The alms of this study were to assess the efficacy of intraoperative adjunctive mitomycin C (MMC) treatment in external DCR surgery and to compare this procedure with the standard DCR procedure alone in the long term (1 year). METHODS: In this prospective, double-masked, randomized, controlled trial, patients with primary acquired nasolacrimal duct obstruction were randomized (using a random number table) into 2 groups based on surgical procedure. In the MMC group, intraoperative adjunctive MMC 0.2 mg/mL was applied to the osteotomy site for 30 minutes. The control group underwent standard DCR procedure only. The results of the DCR surgeries were assessed using objective findings (eg, cessation of excessive tearing via nasolacrimal duct irrigation and the improvement in height of tear meniscus) and subjective symptoms (asking patients to describe the degree of tearing improvement). Both the patients and the researchers who were assessing the study outcomes were masked to treatment group. RESULTS: One hundred eyes of 100 Turkish patients were assessed and equally randomized to the MMC (27 women, 23 men; mean [SD] age, 47.0 [7.6] years) and control (26 women, 24 men; mean age, 46.6 [8.8] years) groups. The follow-up period was not significantly different between the MMC and the control groups (13.1 [1-1] vs 13.2 [1.4] months). Significantly more eyes in the MMC group than the control group remained symptom-free throughout the 1-year follow-up period (45/50 [90%] vs 33/50 [66%]; P = 0.005). Significantly more patients in the control group than the MMC group had an improvement in symptoms at the I-year follow-up (8/50 [16%] vs 2/50 [4%] eyes; P = 0.005). Based on the patency of the drainage system, the success rate was significantly greater in the MMC group than the control group (48/50 [96%] vs 42/50 [84%]; P = 0.005). Based on nasolacrimal duct irrigation, significantly fewer patients in the MMC group than the control group had an enclosed nasolacrimal duct (2/50 [4%] vs 8/50 [16%]). No adverse effects (eg, abnormal nasal bleeding, mucosal necrosis, infection) or any other surgical adverse events were observed. CONCLUSIONS: In the management of these patients with primary acquired nasolacrimal duct obstruction, adjunctive intraoperative MMC application with standard DCR surgery had a significantly higher success rate than did standard DCR surgery alone.
