    Colchicine may be of therapeutic benefit in prurigo pigmentosa

    Prurigo pigmentosa (PP) is a rare inflammatory skin disease. Neutrophil-mediated inflammation is considered to be responsible for the etiopathogenesis of PP. We consider that colchicine may be an effective agent in the treatment of PP since it exerts an antiinflammatory effect by inhibiting neutrophil chemotaxis. Further studies are required to verify whether colchicine is an effective treatment option for PP.

    The Diagnostic Value of Lesional Skin Smears Performed by Experienced Specialist in Cutaneous Leishmaniasis and Routine Microbiology Laboratory

    Objective: Leishmaniasis is a common vector-borne infection affecting 12 million people in 98 countries. The most frequently used method in diagnosis is the microscopic investigation of the leishmania smears. The diagnostic value of this method varies according to the experience of the evaluator. In this prospective study, it was aimed to emphasize the importance of experience in the evaluation of lesional smears used in the diagnosis of cutaneous leishmaniasis. Methods: In this study, patients who were admitted to Dicle University Medical Faculty Hospital Dermatological and Venereal Diseases Outpatient Clinic between January and December 2016 and who had lesions with suspicious cutaneous leishmaniasis were included. For all the cases, both in the routine microbiology laboratory and in the diagnosis and treatment of cutaneous leishmaniasis, separate smears were performed by an experienced specialist and evaluated independently from each other. Results: In 70 of 98 cases studied, the diagnosis of cutaneous leishmaniasis was confirmed by laboratory evaluations. The rate of positivity was significantly higher in the smears analyzed by experienced specialist in the clinical and diagnosis of cutaneous leishmaniasis (95.7%) than in the smears analyzed by the routine microbiology laboratory (42.9%) (p<0.001). Conclusion: The data in our study showed that smears should be performed and evaluated by experienced specialists in the clinical and diagnosis of cutaneous leishmaniasis.

    The effect of pentavalent antimonial compounds used in the treatment of cutaneous leishmaniasis on hemogram and biochemical parameters

    Objective: The aim of this study was to analyze the effect of pentavalent antimonials used in the treatment of cutaneous leishmaniasis (CL) on hemogram and biochemical parameters. Material and methods: The study consisted of 168 patients diagnosed with CL after microscopic examination and treated with either systemic sodium stibogluconate (SSG) or meglumine antimonate (MA) 20 mg/kg/day for 14 days. The patients were divided into two groups as SSG and MA patients. Neutrophil count, leukocyte count, lymphocyte count, hemoglobin concentration, platelet count, amylase, lipase, alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen and serum creatinine levels were compared before and on the 14th day of the treatment. Results: There was a statistically significant decrease in the neutrophil, lymphocyte, leukocyte, platelet counts, and hemoglobin and blood urea nitrogen levels on the 14th day of the treatment when compared to the pre-treatment values. A statistically significant increase was found in the ALT, AST, amylase and lipase levels. No significant change was found in the serum creatinine levels. Conclusion: According to the results of our study, pentavalent antimonials given standard doses in the treatment of CL can lead to an increase in the pancreatic enzymes and transaminases and bone marrow suppression. We do not recommend any change in the treatment if these conditions are not corroborated by clinical findings.

    Increased mRNA expression of ADAMTS1 and ADAMTS4 in peripheral blood mononuclear cells of psoriasis patients developed psoriatic arthritis

    Background and purpose: Psoriatic arthritis (PsA) is a chronic inflammatory disease associated with psoriasis (PsO) affecting both skin and joint. ADAMTS (A disintegrin-like and metalloproteinase domain with thrombospondin-1 repeats) is a large family of proteoglycanase enzymes and the expression levels of ADAMTS proteases are upregulated in arthritis. In this study, we aimed to determine mRNA expression levels of ADAMTS1, ADAMTS4 and ADAMTS5 and identifying the key signaling pathways involved in the regulation of these proteases in peripheral blood mononuclear cells (PBMCs) of patients with PsO who later developed PsA. Materials and methods: 25 PsA patients, 25 PsO patients and 25 healthy individuals were included in this study. PBMCs were isolated from venous blood and mRNA expression levels of ADAMTS1, ADAMTS4 and ADAMTS5 were measured through Real-time quantitative PCR (RTqPCR). Results: mRNA expression levels of ADAMTS1 and ADAMTS4 were found to be increased in PsA patients compared with control and PsO patients. In response to TNF- a stimulation, the expression of ADAMTS1 in PsA patients was determined to be reduced in a Erk1/2 activity dependent manner, whereas p38 and JNK activities were shown to induce the expression of ADAMTS4 in PsA patients. The reduced ADAMTS1 expression in PsA patients induced by IL -1 b stimulation was revealed to be dependent on NFkB activity. Conclusions: mRNA expressions of ADAMTS1 and ADAMTS4 regulated by MAPKs and NFkB were increased in PBMCs of PsA patients. This study supports the hypothesis that ADAMTS1 and ADAMTS4 mRNA level might be diagnostic markers for identifying psoriatic patients who are more likely to develop PsA and a future drug target for PsA treatment.

    Lichenoid drug eruption induced by colchicine: case report

    Lichenoid drug eruption (LDE) is a common cutaneous side effect of drugs including antimalarials, antihypertensives, nonsteroids, anti-inflammatory drugs and diuretics. The physiopathologic relationship between colchicine treatment and LDE is unclear. There is very little documentation of LDE induced by colchicine in the literature. In this report, we present a case that developed LDE on the abdomen and the legs during the colchicine treatment.

    Premature or nevoid sebaceous hyperplasia? Two case reports of a rare disease

    Early or familial cases of sebaceous hyperplasia (SH) with multiple lesions, thought to be associated with genetic predisposition, have been reported in young individuals. Rarely reported variants include the giant, diffuse, zosteriform, and/or linear (nevoid) form. However, premature SH, defined as benign proliferation of sebaceous glands that occurs during or after puberty, is histologically similar to SH. Whether the definitions of nevoid SH or premature SH are the same remains unclear. We report two sporadic cases of premature SH.
