Öğe Cranial MRI findings in epileptic children(2003) Ozateş M.; Acar M.; Başak F.PURPOSE: To investigate the cranial MRI findings in childhood epilepsy and to detect the most frequent cerebral lesions in epileptic children in our region. MATERIALS AND METHODS: In this study, 98 epileptic children were examined with 1 T MRI equipment between January 1997 and March 2001. T1-weighted sagittal, T2 and proton weighted axial and T2-weighted coronal MR images were obtained. Contrast-enhanced T1-weighted axial, sagittal and coronal MR images were also examined in patients with suspicion of infection or intracranial mass. RESULTS: Among a total of 98 patients, 65 (66.3%) had no pathological findings on MRI. The pathological findings were diffuse cerebral atrophy (18.4%), delay in myelination and dismyelination (12.4%), ischemia and infarct (8.2%), arachnoid cysts (6.2%), tumoral mass (5.1%), mesial temporal sclerosis (1.0%) and sequelae of kernicterus (1.0%). CONCLUSION: MRI has been found to be effective in the evaluation of cerebral structures and imaging the pathologies in the etiology of epilepsy and other lesions in childhood focal epilepsy. However, MRI does not always appear to be successful in detecting the epileptic focus.Öğe The evaluation of paraoxonase 1 activity in patients with gestational diabetes(Acta Medica Mediterranea, 2013) Atay A.E.; Sakar M.N.; Culcu N.S.N.; Simsek H.; Akbas H.; Acar M.; Isik B.Material and methods: Sixty five patients with GDM and 66 healthy pregnants were enrolled. Paraoxonase activity, insulin levels, HOMA-IR, demographic features and anthropometric measurements were evaluated. Results: The mean paraoxonase activity was significantly diminished in patients with GDM (p:0,004). Insulin level and HOMA-IR were significantly higher in GDM patients (p:0,004 and p:0,001; respectively). The mean interval between present and previous pregnancy was significantly shorter in patients group (p:0,004). There was a significant correlation between PON1 activity and serum LDL, HDL levels and weight gained during pregnancy (p:0,001,p<0,001 and p:0,002; respectively) but not with weight and parity. However HOMA-IR was significantly correlated with weight and parity (p:0,001 and p:0,002; respectively). Discussion: Diminished PON1 activity and enhanced oxidative stress accompany to insulin resistance in the pathogenesis of GDM. Oxidative stress is associated with weight gained during first 2 trimesters of pregnancy rather than obesity.Öğe The evaluation of the case with bradycardia and syncope that caused by hypothyroidism(2005) Kara I.H.; Özdemir Ö.; Acar M.Syncope is defined as transient loss of consciousness due to reduced blood flow. It may occur suddenly or can be associated with alarming symptoms like weakness, confusion, nausea, visual blurring. The purpose of this report is to makes primary care physicians to pay more attention of case of bradycardia and syncope that caused by hypothyroidism. The patient who experienced a faint period lasting half an hour after wake up at the morning and remained unconscious for 10-15 minutes, complaining dizziness, confusion and vomiting at the end of the faint admitted to Family Practice Outpatient Clinic of Dicle University, Medical Faculty. From medical history, it was learned that patient had total thyroidectomy operation in 1997 and had been using thyroxin tb 2x1 regularly, except the last month. Physical examination revealed heart rate 56/min and rhythmic. At ECG sinus bradycardia and low T3, T4 and high TSH levels were detected. The syncope was attributed to hypothyroidism resulting sinus bradycardia because of thyroidectomy in post medical history not using thyroxin replacement for last month. At central visit it was seen that patient restarted thyroxin replacement and has no clinical problem.Öğe The relation of coronary artery disease with Doppler flow velocity and resistance index in cases without significant carotid artery stenosis(2001) Ulgen M.S.; Bilici A.; Acar M.; Onder H.; Sucu M.; Toprak N.OBJECTIVE: Atherosclerosis causes functional vasomotor changes as well as well as atheromatous carotid plaques and luminal stenosis resulting in mechanical effect. The relation between functional vasomotor changes in carotid arteries and extent of coronary artery disease is unknown. In our study, the probable relationship between carotid arterial flow velocities and resistance indexes (RI) with the extent of coronary artery disease (CAD) in patients who do not have significant carotid luminal stenosis was evaluated. METHODS: One hundred and fourteen patients (74 males, mean age 53+/-10 years, range 33-72 years) were studied. All patients underwent color Doppler sonography before coronary angiography. Peak systolic flow velocity, end-diastolic flow velocity and resistance index (RI) of right and left common carotid and internal carotid and internal carotid arteries were measured by color Doppler sonographic technique. Doppler parameters were correlated with the extent of CAD and left ventricular ejection fraction. RESULTS: Patients were classified on the basis of presence of significant CAD and the number of affected coronary arteries. Thirty-three patients did not have (normal group) and 81 patients had significant coronary arterial stenosis (22 patients with one-vessel disease, 27, with two-vessel disease and 32 patients with three-vessel disease). Flow velocities were the highest in normal group but the lowest in CAD patients, especially when 3 coronary arteries were affected. Correlation analysis demonstrated negative relationship of age, ejection fraction and number of affected coronary arteries with end-diastolic flow velocity, but positive and significant correlation with RI value. CONCLUSION: Our study is the first on this object. The results suggest that presence and extent of CAD changes flow velocities and RI values of common and internal carotid arteries. However, further investigations are required before these parameters can be applied as diagnostic criteria.
