Özet
Bu çalışmada antiepileptik olarak tek başına uzun süreli karbamazepin
kullanan epilepsi hastalarında karbamazepinin kan serum lipit düzeylerine
olan etkisinin araştırılması amaçlanmıştır. Çalışma grubu epilepsi tanısı
alan ve bu nedenle en az bir yıldır karbamazepin kullanan 10'u kadın, 20'si
erkek 30 hastadan, kontrol grubu ise yaş
ve cinsiyet olarak uyumlu 30 sağlıklı
denekten oluşmuştur. Tüm deneklerin serum kolesterol (total kolesterol,
HDL, LDL ) ve trigliserit düzeyleri ve LDL/HDL oranları belirlenmiştir.
Çalışma grubunun serum HDL kolesterol, trigliserit düzeyleri kontrol
grubuna göre istatistiksel olarak anlamlı düzeyde yüksek, serum LDL
kolesterol düzeyi ve LDL/HDL oranlarının kontrol grubuna göre anlamlı
düzeyde düşüktü. Total kolesterol düzeylerinin ortalaması çalışma
grubunda kontrol grubuna göre daha düşük olmasına karşın, bu farklılık
istatistiksel olarak anlamlı değildi. Uzun süreli karbamazepin kullanımının
kolesterol düzeylerine olan etkisi nedeniyle ateroskleroz ve koroner kalp
hastalığının gelişme riskini azaltmaya katkıda bulunabileceği düşünülmüştür.
Yapılacak uzun süreli ileriye dönük izlem çalışmaları bu konunun kesin
biçimde aydınlatılmasına yardımcı olacaktır.
The aim of this study was to determine the effect of carbamazepine on serum lipid levels in epileptic patients who were on long-term carbamazepine monotherapy. The study group were comprised of 30 epileptic patients (10 female, 20 male) who have been on carbamazepine monotherapy for at least one year whereas control group consisted of 30 age and sex matched healthy controls. Serum cholesterol (total cholesterol, HDL cholesterol, LDL cholesterol) and triglyceride levels were measured and LDL/HDL ratio was calculated in all subjects. Serum HDL cholesterol and triglyceride levels of study group were significantly higher than control group whereas serum LDL cholesterol levels and LDL/HDL ratios of study group were lower than control group. Mean total cholesterol level of study group was lower than control group, however the difference did not reach statistical significance level. Because of its effect on cholesterol levels, long term carbamazepine could possibly have a positive influence in decreasing the risk of developing aterosclerosis and coronary heart disease. Long term prospective follow-up studies would be helpful to us in enlightening this issue definitely.
