Özet
Bu çalışmanın amacı; megaloblastik anemi tanısı konulan yedi
hastanın klinik ve laboratuvar bulgularını değerlendirmektir.
Polikliniğe başvuran ve megaloblastik anemi tanısı konulan yedi çocuk
hastanın klinik ve laboratuvar bulguları değerlendirildi.
Beslenme öykülerinde hastaların tümünün hayvansal ürünleri ya hiç
tüketmediği ya da az olarak tükettiği öğrenildi. İştahsızlık, halsizlik, baş
ağrısı, içe kapanıklık ve ayaklarda uyuşma başvuru yakınmalarıydı. Fizik
incelemelerinde dördünde hiporefleksi, glossit, birinde ataksi saptandı. Üç
hastada ortalama eritrosit hacmi (MCV) normal değerlerde idi. Hastaların
tümünde B12 vitamin serum düzeyi düşük, folik asit düzeyi normal idi.
Hastaların tümünün periferik kan yaymasında nötrofil hipersegmentasyonu,
iki hastada lökopeni, bir hastada ise trombositopeni saptandı.
Sonuç olarak, beslenme öyküsünde hayvansal ürünleri az ya da hiç
tüketmeyen hastalarda B12 vitamin eksikliği düşünülmelidir. Ayrıca basit
bir yöntem olan periferik kan yaymasında (PY) nötrofillerde
hipersegmentasyon saptanması da megaloblastik anemi tanısı için
önemlidir.
Aim of the study: to rewieved the clinical and laboratory properties of seven cases with megaloblastic anemia. Clinical and laboratory findings of seven cases with megaloblastic anemia are described. İt is determined that all of the patients received little or no animal products by nutritional history. Clinically apatite, malasia, headeche, otism, and parestheia in the lower extremities and foods were present in patients. On physical examination; four patients had glossit, four had hyporeflexia, one had ataxia. Folat level was normal and B12 vitamin level was low in all patients. The MCV (mean corpuscular volume) was normal in three patients. Hypersegmentation of neutrophil was observed in all patients, leukopenia in two, and trombocytopenia was observed in one patient. Conclusion: it is suggested B12 vitamin deficiency in the patients that received little or no animal products by nutritional history. However, hypersegmentation of neutrophil in peripheral blood sample is an important finding for diagnosis of megaloblastic anemia.
