dc.description.abstractAmaç: Smear ve HPV-DNA testinde anormallik saptanan hastalarda serviksin kolposkopik olarak incelenerek preinvaziv-invaziv neoplazi bulunma olasılığı yuksek olan hasta grubunu tespit etmeyi amaçladık. Gereç ve Yöntem: Çalışmamızda; 01.06.2015-31.05.2016 tarihleri arasında, eş zamanlı HPV DNA ve smear bakılan 1753 hasta retrospektif olarak değerlendirildi. Smearler sıvı bazlı sitoloji ile alınmıştır. Smear sonuçları, Bethesda Sistemi 2001 ile yorumlanmıştır. HPV DNA Genotiplendirme çalışması Roche marka Cobas 4800 sisteminde, Cobas 4800 HPV Test kullanılarak Real Time PCR(polimeraz zincir reaksiyonu) yöntemiyle çalışılmıştır. Kolposkopik muayeneler; LEİSEGANG marka BG/LED model, teaching monitor bağlanılabilen binoküler kolposkop ile serviks incelenmiştir. Verinin istatistiksel analizi SPSS (Statistical Package for the Social Sciences) 18 istatistik paket programında yapılmıştır. Çalışmamızdaki bütün değerlendirmeler % 95 güvenle yapılmıştır. Anlamlılık düzeyi p=0.05 olarak öngörülmüştür. Kategorik verilerin incelenmesinde Pearson ki-kare testi, ki-kare test kullanılmıştır. Tanımlayıcı istatistiklerle demografik bulgular özetlenmiştir. Bulgular: Kadın doğum jinekoloji polikliniğine başvuran ve eş zamanlı HPV DNA ve smear bakılan toplam 1753 hasta çalışmaya dahil edildi. Hastaların yaş ortalaması 41,89±12,19 bulunmuştur. Çalışmaya dahil edilen 1753 hastanın %93,3 benign (n:1639), %6,7 anormal smear (n:114) sonucu tanısı almıştır. Anormal smear sonuçlarının %53,5’i LSIL (n:61), %13,1’i HSIL (n:15), %29’u ASCUS (n:33), %2,6’sı ASC-H (n:3), %1,8’i AGUS (n:2) tanısı almıştır. Hastaların %87,7’si HPV DNA negatif (n:1538), %8,7’si HPV DNA pozitif (n:152), %3,6’sı geçersiz-hücre yetersizliği (invalid) (n:63) sonuç almıştır. HPV-DNA pozitif hastaların 46 tanesi HPV DNA 16+ (%30,3), 13 tanesi HPV-DNA 18+ (%8, 5), 93 tanesi HPV-DNA diğer HR (yüksek risk) (%61,2) tespit edilmiştir. Hastaların sadece 72 (%27) tanesine kolposkopik biyopsi yapılmıştır. Kolposkopik biyopsi sonuçlarının %34,7’si benign (n:25), %45,8’i LSIL (n:33), %16,7’si HSIL (n:12), %2,8’i karsinoma in situ (n:2) olarak tespit edilmiştir. Sonuç: Düşük gradeli lezyonlarda preinvaziv bir lezyonu atlamamak veya gereksiz invaziv uygulamalardan kaçınmak için; sitolojiye ek olarak HPV DNA testi ve kolposkopi eşliğinde muayene yapılmalıdır. dc.description.abstractObjective: In patients who is subjected to the abnormal cervical smeare and abnormal HPV-DNA test and by colposcopic examination of the cervix, We am aimed to determine the patient group with a high probability of having preinvasive -invasive neoplasia. Method and Material: We am retrospectively evaluated 1753 patients with performed tests whch is HPV DNA and smear test, between 01.06.2015 and 31.05.2016 in our study. The smears were examined and checked by liquid-based cytology. Smear results of patients were interpreted with Bethesda 2001. HPV DNA genotyping work was performed on a Roche brand Cobas 4800 system by using the Cobas 4800 HPV test and studied by using the REAL TİME PCR(polymerase chain reaction) method. Colposcopic examinations; the cervix was examined with a binocular colposcope that could be connected to a teaching monitor having LEISEGANG brand BG / LED model. Statistical analysis of the data was made in the SPSS (Statistical Package for the Social Sciences) 18 statistical package program. All the evaluations that we made in our study were performed with 95% confidence. The level of significance was predicted as p =0.05. Pearson Chi square test and Chi square test were used when categorical data were examined. Demographic findings were summarized with descriptive istatistics. Results: Totally 1753 patıents who apply to gynecology polyclinic and were examined at the same time by performed HPV DNA and smear test. The average age of the patients was 41,89±12,19. Of the 1753 patients included in the study,% 93,3 had benign (n:1639) and %6,7 (n:114) had abnormal smear outcome diagnosis. 53,5 % of the abnormal smear results were diagnosed LSIL (n:61) and remaining as follows: %13,1 were HSIL (n:15), %29 were ASCUS (n:33), %2,6 were ASC-H (n:3), %1,8 were AGUS (n:2). %87,7 of the patients HPV-DNA negative (n:1538), %8,7 HPV DNA positive (n:152), %3,6 invalid (n:63) were diagnosed. 46 of the HPV DNA positive patients were HPV DNA 16+ (%30,3), 13 of them HPV DNA 18+ (%8,5) and finally 93 of them HPV DNA other HR (high risk) (%61,2) were determined. Colposcopic biopsy was performed on only 72 patients (%27). %34,7 percent of colposcopic biopsy results were benign (n:25) and remaining as follows: %45,8 of them LSIL (n:33), %16,7 of them HSIL (n:12), %2,8 of them carcinoma in situ were detected. Conclusions: In the low-grade lesions, in order to not miss out a preinvasive lesion or in order to avoid unnecessary invasive applications; in addition to cytology, HPV DNA test and colposcopy examination should be performed.
